2014-2015 WILLIAM HOWARD TAFT HIGH SCHOOL ATHLETIC BOOSTER CLUB SCHOLARSHIP The William Howard Taft High School Athletic Booster Club will award scholarships to at least one senior male athlete and one senior female athlete each year. The scholarships will be awarded to athletes attending any post-secondary institution including two-year colleges, four-year colleges and universities, and trade schools. The purpose of the award is to recognize the Taft athlete who not only shows a high level of commitment, participation, and sportsmanship but also exemplifies the 10 IB attributes on the field, in the classroom, and around his or her community. The eligible applicant must be a senior athlete in a varsity or junior varsity sport, marching band member, student athletic trainer, or team manager who provides extraordinary assistance or support to the athletic program while attending Taft High School. Eligibility* requirements for the Taft High School Athletic Booster Club Scholarship are as follows: A minimum 2.0 grade point average. The applicant s GPA will be verified by his/her guidance counselor after submission of the application. Enrollment and attendance at a post-secondary institution the following school year. It is the responsibility of the applicant to ensure that all documents are complete and submitted. *Any applicant submitting false information will be disqualified. *Applicants will NOT be eligible for this scholarship if at the time it is awarded they have accepted a Full-ride (tuition, room and board and books) from a college, university or trade/technical school, per the NCAA. *Starting with the 2015-2016 school year, only students whose families are active members of the William Howard Taft Athletic Booster Club will be eligible for the scholarship.
2014-2015 WILLIAM HOWARD TAFT HIGH SCHOOL ATHLETIC BOOSTER CLUB SCHOLARSHIP Evaluation: Applicants will be evaluated based on the following criteria: Sportsmanship and Commitment to Excellence: 25% of evaluation score o attitude toward team members, coaches, equipment, officials and spectators o putting in extra work, being dedicated and striving to be well prepared Athletic Participation and Recognitions/Awards: 25% of evaluation score Leadership: 25% of evaluation score o positive example for others, being a good role model, exhibiting good sportsmanship Participation in Non-Athletic Activities: 25% of evaluation score o Community Involvement / Service / Activities, and School Activities and Awards Selection Committee: The selection committee will consist of the Athletic Director, Student Activity Director, (1) Building Administrator, (1) School Counselor and (1) Athletic Booster Board Member who does not have an eligible scholarship candidate. Application Packet: The applicant must include the following items with their application (in this order): 1. Scholarship Application Form 2. Athletic Participation Form 3. Activities, Honors and Awards Form 4. Coach s Recommendation Form in sealed envelope with coach s signature across seal. 5. A one page, typed, double-spaced size 12 font, character reference from a William Howard Taft High School faculty member. 6. A one page, typed, double spaced size 12 font, essay addressing the statement: How has being a part of a team positively influenced you and how has your participation contributed to the positive spirit and overall betterment of the team? Recommendation/Character Letters/Essay: 1. Keep all letters to a maximum of one, typed, double-spaced page size 12 font. 2. Include the student s ID number on the top right hand corner of the letter/essay. 3. Please mention applicant s name only once when writing the recommendation. The selection committee will use ID numbers for anonymity.
