Pre-existing condition means a condition resulting from an Injury or Sickness for which the Covered Person is diagnosed or received treatment within 6 months prior to the Covered Person s Effective Date. Loss of Earnings with 12 month work incentive Lump sum payment equal to three months of net benefits paid to an eligible survivor if you are receiving a benefit and have been disabled for 12 months. If you become disabled for the same condition within 12 months following your prior disability, you will not need to satisfy a new 180 day elimination period. You must work a minimum of 35 hours per week. None Basic Life Amount of Insurance 1.5 times annual earnings up to maximum of $300,000 Basic Life Reduction Schedule Accelerated Death Benefit Basic Annual Earnings Waiver of Premium Conversion and Portability Contributions Age 65-69: 1.0 times annual earnings, $200,000 max Age 70-74: 0.75 times annual earnings, $150,000 max Age 75+: 0.5 times annual earnings, $105,000 max If you have a life expectancy of 12 months or less you may request up to 75% of your coverage amount while still living to a maximum of $500,000. The minimum that can be requested is 10% of your coverage amount or $5,000. Base annual salary In the event you become totally disabled prior to age 70, you may be eligible to continue your group life insurance. You may apply for the waiver after 6 months of being totally disabled and waiver will term at age 70. If all or part of your coverage ends, you may port your coverage into an individual term life policy or convert your policy to an individual whole-life insurance policy.

Basic Life Insurance: Part-time employees Eligibility Class 1: Active part-time faculty Class 2: Active part-time staff Employees who are covered by a collective bargaining agreement and independent contractors are excluded. You must be deemed health and welfare benefits eligible by meeting the minimum requirements outlined below. Minimum Hours Waiting Period Class 1: Taught at least six 4-credit hour courses, or the equivalent, (at least four courses for the Law School) during the 12 month measurement period. Class 2: Worked at least 1,000 hours of service during the 12 month measurement period. Initial Eligibility: The Office of Human Resources reviews an employee s hours of service credited during his or her first 12 months of employment to determine the employee s eligibility. Ongoing Eligibility: Beginning with the October 3 following a part-time employee s date of hire, the Office of Human Resources reviews an employee s hours of service earned in the 12-month measurement period between October 3 and October 2 to determine the employee s eligibility to enroll in the health and welfare benefit options on an annual basis. You must work a minimum of 35 hours per week. None An amount equal to 1, 2, 3, 4, or 5 times your annual earnings not to exceed $500,000. The non-medical maximum is 1, 2, or 3 times your annual earnings. If you elect for more than 3 times your annual earnings, you will be required to submit a medical questionnaire and be approved by Liberty. Late Applicant: Waiver of Premium: Conversion and Portability Dependent Life Insurance and AD&D Coverage Amount: If you declined coverage before and wish to enroll now, you will need to submit a medical questionnaire and be approved for any amount you elect. In the event you become totally disabled prior to age 70, you may be eligible to continue your group life insurance. You may apply for the waiver after 6 months of being totally disabled and waiver will term at age 70. If all or part of your coverage ends, you may port your coverage into an individual term life policy or convert your policy to an individual whole-life insurance policy.
