Group life insurance is a life insurance scheme in which a single contract covers an entire group of people. Typically, in such schemes, the life insurance scheme owner is the employer or other registered groups as applicable. Group life insurance is often provided as part of employee benefits package because a single Policy is issued covering all members and in view of simplified procedures in risk assessment, the cost of group coverage is far less than what you would pay for a similar amount of individual protection. Rider option Group Accidental Death Benefit Rider, UIN 104B024V01 o Minimum age at entry 18 years o Maximum age at entry 64 years o Maximum cover ceasing age 65 years o Minimum Policy Term 1 year o Event covered Death due to accident o Minimum face amount - ` 5,000 o Maximum face amount ` 50,00,000 subject to rider sum assured not exceeding base sum assured at a scheme level o Minimum Premium - No such limit o Maximum Premium No such limit, subject to rider premium not exceeding 30% of the base plan premium at a scheme level Add on benefits (subject to underwriting) Spouse Cover Additional Insurance (Voluntary Top-up) Dependents Cover General Conditions The plan is a one year contract and can be renewed at the premium rates, terms and conditions mutually agreed by the Master Policyholder and Max Life Insurance Company Premiums can be paid annually, semi-annually, quarterly or monthly. Modal factors would apply as follows : Modal Factors Premium Mode Factor Annual 1.000 Semi-annual 0.520

Modal Factors Premium Mode Factor Quarterly 0.265 Monthly 0.090 Grace Period: A grace period of 15 days in respect of monthly mode and 30 days in respect of all other modes will be allowed within the policy term. During the grace period the policy will continue to be in force but in the event of any member suffering an insured event during the grace period, the company shall be entitled to deduct the aggregated unpaid premium in respect of the entire Group from the benefits payable under the policy. Provision for addition and deletion of members Provision for policy review period of 15 days. Sum Assured for a scheme or member of a scheme may be increased or decreased subject to underwriting. In the case of employer - employee schemes, cover in respect of a member will cease on ceasing of membership from the group. In the case of non employer - employee schemes, cover in respect of a member will cease on ceasing of membership from the group unless it is agreed to continue till the end of the period of cover. Employee Additions & Deletions New employees satisfying the eligibility criteria shall be allowed to join the scheme The cover commencement date shall be the date when his name is entered in the register of members and is communicated to Max Life Insurance Company The frequency of intimation to the employer for additions would be at least on monthly basis The insurance coverage will cease at the earliest of the following: o On termination of employee s service o o o o End of coverage term On retirement On Discontinuance of the policy On death of the member The proportionate premium for the unexpired period of risk would be refunded upon employee leaving the group or ceasing to be a member of the group: Tax Benefits You may be entitled to certain applicable tax benefits on your premiums and Policy benefits. Please note that all the tax benefits are subject to tax laws prevailing at the time of payment premium or receipt of benefits by you. Tax benefits are subject to changes in tax laws.

Premium paid by the employer would be treated as business expense under section 37(1) of the Income Tax Act, 1961. All death claim payments are exempted from tax under Section 10(10D) of the Income Tax Act, 1961 Service tax and educational cess would be applied on the premiums paid and recovered from the Policyholder Unique Features of this Scheme Free Cover Limit (No evidence Limit/ Automatic Acceptance Limit): It is the maximum sum assured under a group scheme up to which a member is covered without any evidence of health. It is an Automatic Acceptance Limit, which depends on the size of the Group and average sum assured, provided It is an Employer Employee Scheme It covers full time permanent employees All eligible employees are covered compulsorily. No individual selection is involved The member is Actively At Work on the effective date of coverage of scheme The member is below age 65 or the normal retirement age if less. However, in the case of schemes in lieu of Employee Deposit Linked Insurance, Free Cover Limit will be allowed up to 69 years. Where cover is not compulsory i.e. participation is voluntary, Free Cover Limit will also be allowed provided a certain minimum participation criteria are met. Cover for the Sum Assured above the Free Cover Limit may be considered subject to underwriting as per the Company underwriting guidelines. No Free Cover Limit will apply for non employer- employee groups. Cover in respect of members under non employer- employee groups is normally subject to underwriting, which depends on the level of Sum Assured and age; and premium rates will depend on occupation profile of the members and the past claims experience/ likely future claims experience Additional option: The following options can be exercised with this product depending upon the eligibility criterion of a specific group. Spouse and Dependent Cover Cover on the life of spouse/ dependants of a member can be opted either by the individual member/ employer. Cover on the life of spouse/ dependants is subject to evidence of insurability as per the Company underwriting guidelines and applicable premium rates. The cover in respect of the spouse will cease immediately on the exit of the member from the scheme either due to death, retirement, resignation, and termination or in the case of divorce/annulment of marriage of spouse Additional Insurance (Voluntary Top-up): In addition to the Sum Assured available under the scheme, employees can opt for additional Sum Assured, which will be considered on the basis of evidence of insurability as per the Company s underwriting guidelines. Types of Groups eligible for Insurance under this Plan: Employees of an employer all or defined categories. Other non employer-employee groups where a clearly evident relationship between the member and the group policyholder for services other than insurance exist Members of a professional association Members of a trade union. Members of a co-operative society. Borrowers of a Housing Society/ Bank/ NBFC

Members of groups of rural occupation such weavers, fisherman etc. Other groups with a common economic link and not formed with sole purpose of obtaining insurance cover Specimen Premium Rates for 1 Lakh Sum Assured per annum as given below for the Employees involved in the professional/ Managerial/ Clerical and Administrative Staff. Age in Years 20 25 30 35 40 45 50 55 60 Premium in Rs. 65.00 71.00 73.00 82.00 113.00 161.00 258.00 411.00 614.00 All Premiums are subject to applicable taxes and levies including service tax which shall be to the account of the Policyholder and shall be recovered following by the prescribed procedure. Revival of Policy: Revival may be allowed within the policy term of one year. A policy lapsed within the policy term of one year due to non-payment of model premium can be revived within three months within Policy Term of 1 (One) Policy Year from the date of first unpaid premium after the policy has lapsed provided: i) Policyholder forwards a written request for reinstatement to Max Life Insurance. ii) Policyholder can produce an evidence of insurability for the members acceptable to Max Life Insurance and bear cost for the same and iii) Policyholder pays all overdue Premiums with interest at such rates as may be determined by Max Life Insurance. The revival will take effect only after it is approved by Max Life and communicated to the Policyholder in writing. Upon revival, the Company shall pay all the benefits that become due and payable under the Policy. Suicide Exclusion: In case of employer-employee schemes where the cover is compulsory, suicide exclusion clause will not be applicable. In all other cases, if a member commits suicide, whether sane or not at the time, within one year from the effective date of coverage or the entry date as the case may be then the insurance in respect of the member shall come to an end simultaneously with the occurrence of such event, and the liability of the company shall be limited to refund of the premium(s) received in respect of the member, without interest.. Prohibition of Rebates: Section 41 of the Insurance Act, 1938 states: (1) No person shall allow or offer to allow, either directly or indirectly, as an inducement to any person to take or renew or continue an insurance in respect of any kind of risk relating to lives or property in India, any rebate of the whole or part of the commission payable or any rebate of the premium shown on the policy, nor shall any person taking out or renewing or continuing a policy

accept any rebate, except such rebate as may be allowed in accordance with the published prospectuses or tables of the insurer: Provided that acceptance by an insurance agent of commission in connection with a policy of life insurance taken out by himself on his own life shall not be deemed to be acceptance of a rebate of premium within the meaning of this sub-section if at the time of such acceptance the insurance agent satisfies the prescribed conditions establishing that he is a bona fide insurance agent employed by the insurer. (2) Any person making default in complying with the provisions of this section shall be punishable with fine which may extend to five hundred rupees. Full Disclosure & Incontestability: This Policy has been issued on the representation of the Proposer that he has made full disclosures of all relevant facts and circumstances. Any concealment, non-disclosure, misrepresentation or fraud by the Proposer shall render the Policy liable for cancellation and shall be the grounds for the Company to avoid all or any liability and if it deems fit, the Company may also forfeit the Premium(s) received, subject to the provisions of Section 45 of the Insurance Act 1938. Section 45 of the insurance Act, 1938 states that under the provisions of Section 45 of the insurance Act, 1938, No policy of life insurance effected before the commencement of this Act shall after the expiry of two years from the date of commencement of this Act and no policy of life insurance effected after the coming into force of this Act shall, after the expiry of two years from the date on which it was effected be called in question by an insurer on the ground that statement made in the proposal or in any report of a medical officer, or referee, or friend of the insured, or in any other document leading to the issue of the policy, was inaccurate or false, unless the insurer shows that such statement was on a material matter or suppressed facts which it was material to disclose and that it was fraudulently made by the policy-holder and that the policy-holder knew at the time of making it that the statement was false or that it suppressed facts which it was material to disclose: Provided that nothing in this section shall prevent the insurer from calling for proof of age at any time if he is entitled to do so, and no policy shall be deemed to be called in question merely because the terms of the policy are adjusted on subsequent proof that the age of the life insured was incorrectly stated in the proposal Nomination: As per Section 39 of the Insurance Act 1938, the person named by the member as the nominee/s at the time of commencement of the membership or the person who has been nominated at a later date, will be the person to whom the money is payable on death of the member. This nomination only indicates the person, who is authorised to receive the amount, on the payment of which the Company will receive a valid discharge of its liability under the policy. A change in nomination may be made by the policyholder at any time during the term of the policy. The details of the nominee and appointee, (where nominee is a minor) will be entered by the Policyholder in the Register of Member. Policy Review (Free Look) Period In case you are not satisfied with the terms and conditions of the policy, you have a period of 15 days from the date of receipt of the Policy to return the Policy stating the reasons for your objections, upon which you shall be entitled to refund of the premium paid subject to deduction of the proportionate risk premium for the

period of cover and the expenses incurred by the Company on medical examination and on account of stamp duty.
