The Patient Reference Group is a very informal feedback session. Good as it is!! Change of music Kelsall Medical Centre New Appointment System From Monday 11 May 2015 Kelsall Medical Centre will be trialling a new appointment system. The GP s have decided to implement an improved appointment system to help meet the increasing demand on a day-to-day-basis. When you telephone the surgery you will be asked if you feel you could be dealt with via a telephone consultation with a GP. If you feel you need to see a GP face to face please advise the receptionist who will arrange an appointment. You will still be able to speak to a doctor for a review telephone consultation. These slots will be booked before the GP s afternoon surgery. We will no longer book appointments at 5.30pm for the following morning surgery. If you feel your problem is acute for the day you can ring at 8.00am for a telephone consultation or appointment. Would you like to join our Patient Participation Group? Are you interested in how your health centre is run and what services it provides? We are currently looking for new members to join our Patient Participation Group This is a group of patients who meet with practice staff during the year to discuss how our patients view the practice s performance and to discuss what plans we put in place to improve our performance. The group is also contacted during the year if specific views are sought on various topics from the group. It is not an onerous task but allows you to understand better how a GP practice operates and gives you an opportunity to have a say on how the practice performs. please email info@kelsallmedicalcentre.org.uk or ask at Reception for more information. Our next meeting is Monday 7 September 2015 at Kelsall Community Centre 5.45 6.45pm Named Accountable GP From 1 st April 2015 we are required under our NHS contract to allocate a named accountable GP to all patients who will be responsible for your overall care. All registered patients at this practice have now been allocated a Named Accountable GP. New patients joining the surgery will be advised of their Accountable Named GP at the point of registration. In order to facilitate this we have allocated the GP s to patients on a surname basis as follows: Patients with a surname beginning with A-H Patients with a surname beginning with I-P Patients with a surname beginning with Q-Z Dr Hinds Dr Daniels Dr Donovan If you are registered aged over 75 and registered before 1 April 2015 you will have already been allocated a Named Accountable GP this named GP will remain the same and will not change under the surname arrangement. Please be aware that you may still see any GP of your choice at the surgery and do not have to see your Named Accountable GP, nor does this mean that you will only be offered appointments with your named GP.
Where a patient expresses a preference as to which GP they have been assigned, the practice will make reasonable efforts to accommodate this request. Men ACWY vaccine The Men ACWY vaccine protects against four different causes of meningitis and septicaemia meningococcal (Men) A, C, W and Y diseases. All 17 and 18 year olds in school year 13 and first time university students up to the age of 25 are eligible for the Men ACWY vaccine as part of the NHS vaccination programme. How do you get the vaccine? GP practices will automatically send letters inviting 17 and 18 year olds in school year 13 to have the Men ACWY vaccine. Students going away to university or college for the first time as freshers should contact their GP to have the Men ACWY vaccine, ideally before the start of the academic year. Students in their second year or above of university do not need the vaccine and are not included in this programme. Younger teenagers (school year 9 or 10) will be offered the Men ACWY vaccine in school as part of the routine adolescent schools programme alongside the 3 in 1 teenage booster, and as a direct replacement for the Men C vaccination. Kelsall Medical Centre will no longer offer a Travel Clinic Service. The clinic is held on Friday mornings from 9.30am-12.45pm. The services are provided by Specialist Nurse Norma Evans. Liverpool School of Tropical Medicine 0151 705 3223 Cheshire & Warrington Carers Centre CARERS Do you look after someone who is ill, frail, disabled or mentally ill? If so, you are a carer. We are interested in identifying carers, especially those people who may be caring without help or support. We know that carers are often hidden looking after a family member or helping a friend or neighbour with day to day tasks and may not see themselves as a carer. We feel that caring for someone is an important and valuable role in the community, which is often a 24 hour job that can be very demanding and isolating for the carer. We further believe carers should receive appropriate support by way of access to accurate information on a range of topics such as entitlement to benefits and respite care and not least, a listening ear when things get too much. As a carer, you are entitled to have your needs assessed by Adult Care Services. A carer s assessment is a chance to talk about your needs as a carer and the possible ways help could be given. It also looks at the needs of the person you care for. This could be done separately, or together, depending on the situation. There is no charge for an assessment. If you are a carer, this is an opportunity to let the practice know so that we can update our records and pass on your details to the Carers Service who can provide relevant information and advice, local support services, newsletter and telephone linkline. CARERS IDENTIFICATION AND REFERRAL FORM DO YOU LOOK AFTER SOMEONE WHO IS ILL, FRAIL, DISABLED OR MENTALLY ILL? If so, you are a carer and we would like to support you. Please complete this form and hand it in to reception. If you are agreeable, we will enter the details on your medical record With this you will have been given a leaflet from Carers in Cheshire which is country wide organisation providing relevant information and advice, local support services, newsletter and telephone link line for carers. We will also refer you, with your permission, to have your needs assessed by Adult Care Services. A Carers Assessment is a chance to talk about your needs as a carer and the possible ways help could be given. It also look at the needs of the person you care for. This could be done separately, or together, depending on the situation. There is no charge for an assessment. How do l let my GP Surgery know I am a Carer? Simply complete Form 1 Registration of Carer overleaf and hand it in to your GP Surgery. The surgery will make a note on your medical records and (where applicable on the records of the person you care for), that you are a Carer. If you would like your details to be forwarded to Cheshire & Warrington Carers Centre, please also sign the bottom of this form. The Centre will send you information such as regular newsletter, how to register for a Carers Emergency Card and help access grants and funds. They will inform you of any events happening in your area. What about the person I care for? If the person you care for would like your GP surgery to know that you are their carer, please ask them to complete Form 2 Registration of the person you care for overleaf. You can then hand both forms to your GP reception.

Appointments will be bookable up to two weeks in advance. Patients are able to ring to book an appointment on 01244 385422 between 8:00am 6:00pm. The service will be provided by local GPs and Practice Nurses, many of whom may already work for the Out of Hours service. Clinics The clinics will operate between: Monday to Friday: 18:30 to 21:30 in Chester and Ellesmere Port 18.30 to 20:00 in Helsby (2 evenings Monday and Tuesday) 18:30 to 20:00 in Tarporley (3 evenings Wednesday Thursday Friday) 18.30 to 20:00 in Malpas and Neston (2 evenings Monday and Thursday) (start date to be arranged) Saturday: 09:00 to 14:00 in Chester, Ellesmere Port, Tarporley 09:00 to 12:00 in Malpas and Neston (start date to be arranged)
