FINGERPRINTING LOCATIONS Boulder Police Dept. 1805 33 rd St. Boulder, CO 80301 303.441.3333 If you are a resident of or work in the City of Boulder, the police department can take your fingerprints for a variety of purposes. The police department does not do fingerprinting for immigration purposes. You will need to bring valid photo identification. Often, an applicant will be provided with a blank fingerprint card preprinted with important information relevant to the purpose for fingerprinting already filled in. The police department's main location at 1805 33rd St. offers fingerprinting on Tuesdays and Thursdays between 8 a.m. and 6 p.m. Fingerprinting is also available at the Community Police Center on 16th St., just south of Pearl St., Monday through Thursday, 10 a.m. to 2 p.m. The cost of this service is $11 per fingerprint card. This office is closed on holidays. The fees for fingerprinting are $10 per card only for persons who reside within Boulder County. Residency is determined by the address on the person s driver s license. Other ways to prove residency include residential lease agreements or utility bills containing the person s name. Persons who wish to be fingerprinted should bring their fingerprint card(s) with them. The Boulder County Sheriff s Office will provide fingerprint cards for people being fingerprinted; however, applicants are encouraged to bring in a fingerprint card from the requesting agency as most fingerprint cards have specific information which may not be available or known to the Records personnel.

One DesCombes Drive Broomfield, CO 80020 303.469.3301 The Court Security Unit offers fingerprinting to Broomfield citizens Monday and Tuesday from 9 a.m. to 4 p.m. There is no charge for this service to Broomfield residents. Proof of residency is required. For questions regarding fingerprinting, call 303.464.5722. Lafayette Police Dept. 451 North 111th St. Lafayette, CO 80026 303.665.5571 Fingerprinting is provided every Tuesday from 8:30 am-12 noon (walk-in). Lafayette residency or employment is required. A valid driver s license or ID card is also required. Fee is $15 per card. Louisville Police Dept. 749 Main St. Louisville, CO 80027 303.666.6565 Fingerprinting services are available by appointment only on Tuesdays from 8 AM - 3 PM. Cost is $10 for residents and $15 for non-residents. You must provide your own fingerprint card. The fee is double if you need four or more cards completed. The district does not carry insurance on private vehicles driven by parents or volunteers. It is Board Policy (EEAE) and therefore required that the owner carry a minimum of the following: $100,000/$300,000 liability, $50,000 property damage and Uninsured Motorist $25,000/$50,000. Board Policy EEAE is on the back of this form. Please read carefully before completing form. Thank you. We also need a copy of each driver s license and a signed copy of the BVSD Volunteer Agreement (non-coaching). *New law as of 8/1/10: Children under the age of eight shall be properly restrained in a child restraint system, according to the manufacturer s instructions.
