When we receive your completed booking form we will phone you to confirm the date and time of the appointment and we will collect the fee for your appointment by debit/credit card over the phone. Without the relevant documents we cannot offer you an appointment. Any documents not in English must be submitted along with an official translation. *Alternative evidence examples:- letters from employers/schools/colleges/universities confirming dates of work/attendance, P60 s, wage slips (must be a complete set of either 52 weekly, 26 fortnightly or 12 monthly wage slips), National Insurance contributions letter, letters from Inland Revenue confirming payment of taxes (if self employed), letters from DSS confirming receipt of unemployment benefit, income support or other benefits including child benefit.
