The firm was initially founded by Maria J. Gutierrez, which continues to be involved in the day to day operations of the organization. Maria J. Gutierrez is a rare gem in the process serving industry that brings over two decades of legal experience with the nation s largest firms. She understands the issues that arise during a trial and has the experience to propose effective solutions on the fly. She has worked with National and International law firms since 1987 and was a former Litigation and Insolvency Paralegal. In 1993 Maria obtained her Bachelors of Arts Degree in English from the Florida International University. She then received her Master of Science Degree in Criminal Justice from the Florida International University in 1997 and graduated with Honors. She also graduated as an active member of the English Honor's Society. Maria Incorporated Judicial Process & Support, Inc. on April 20, 2000 and quickly became a member of multiple organizations that support our industry locally, statewide, and nationwide. Afterwards she became a Certified Process Server in Miami-Dade County on March 9, 2001 (CPS #1286) and a Special Process Server in Broward County on September 30, 2000 (SPS #463). In addition, she also obtained a Private Investigation License on August 30, 2002 and a Private Investigative Agency License on September 9, 2003 from the Florida Department of Agriculture and Consumer Services (A-2300228). 2. Contractor Management Plan A. Organization & Management Judicial Process & Support, Inc., is managed by Maria J. Gutierrez which is profiled in the executive summary. Our main office is in Miami, FL and employs a team of dedicated professionals whose sole goal is to provide a great experience for our customers. We understand that entrusting us with possible thousands of documents can be a daunting task, and we take that very seriously. Our in-house team of clerks, messengers, serve coaches, process servers, and management are here to assist you with anything that you need in order to get the job done. At any point in time we can deploy our in house messenger to your location to perform pick up and drop off activities as required. We employ twelve local process servers that each covers a specific geographic area in order to improve document delivery speeds and efficiency. We also only hire certified, appointed, and experienced process servers to get your documents served. B. Experience in General Over the last decade our corporation has served thousands of clients nationally and internationally. Judicial Process & Support, Inc., has assisted thousands of clients around the world with their service of process needs. Our extensive client portfolio includes the Department of Justice, the Securities and Exchange Commission, Ocean Bank, University of Miami, and hundreds of other law firms, insurance companies and/or collection agencies across the globe.

3. Contractors Technical Plan A. Technical Approach When a document comes into our office it goes through our proprietary Document-Check process which ensures that documents are not sent to a process server on the field until they have been reviewed for quality control. We handle everything throughout the process from start to finish. This way you can be at ease that everything will be handled properly by a group of professionals. In addition to a quality control review of each of document by a certified staff member, Maria personally reviews each document to ensure that each document is served correctly and efficiently. Then our job input manager deploys the Quick- Dispatch protocol and enters the job information into the system and dispatches it to a server. This process benefits you by ensuring that your documents will be on the field faster than ever. We have three standard turnaround speeds that we offer Standard, Next Day, and Same Day. They are attempted within 3-5 business days, one business day, or 24 hours respectively. You own personal Serve-Coach can assist you when you need answers related to one of your engagements with us. Either Maria J. Gutierrez or one of the in house coaches will offer you live honest, useful information that will answer your specific needs. Our office, which is located in Downtown Miami is open daily and has a team ready to take care of you. In addition we offer multiple ways to connect to support through the phone, e-mail, fax or even our online help desk software. This way you can rest assured that when you need to follow up on something, we will be here. Multiple ways to connect to use gives you more freedom and flexibility. Our Tracking-Star software gets updates sent to your team automatically by e-mail. Now review our turnaround times and pricing and select the turnaround that fits into your criteria. Lastly you will need to review our checklist and determine if you have everything you need. Part 2 Place your Order In step two you will need to completely fill out the order form. Either enter your credit card information on the order form or attach a company check. If it is a personal case please attach a money order. Congratulations, you re now ready to deliver your documents to us. You may call us and schedule a pickup, use the US Postal Service or any express carrier. Step 3 Updates and Service When your order is received at the Judicial Support headquarters it first goes through quality control to ensure that everything is correct and in order. Afterwards you will receive an e-mail that confirms the receipt of your document and that it has been inputted inside of our job tracking software. As soon as we have updates on your document we will send you an e-mail with the updated status. The updated status will advise you that your document has been served, the service has been attempted, or that the address provided is not the current address for the witness or defendant.

TrainServe Rush When you need your documents served quickly, you need our TrainServe service. We get your documents served within 1 business day or 2 business days in rural areas. JetServe Same Day Our JetServe service is our most elite service level. You receive our ExtremePriority support and Same Day Turnaround. If you have a critical document that needs to be served, you need JetServe!. Otherwise it takes up to 2 business days to make an attempt outside of Florida When does the turnaround time start? Answer: It starts when we receive the documents What is Sub-Service? Answer: When we can deliver/serve the documents to either a Registered Agent or any person at the location What is Personal Service? Answer: When we must deliver/serve the document to ONLY one person in particular

Subpoena Checklist The following items must be included when you send your Subpoenas to Judicial Support. 1 Copy of the Subpoena Per Witness Witness Fee Check (If Applicable) Judicial Support Order Form Payment (Unless Account is Open) Subpoenas Frequently Asked Questions 1) Do I need to include a witness fee check if I am only requesting documents, and not requesting the individual to appear in court? Answer: No witness fee is required in this situation. 2) What is the witness fee cost? Answer: It varies by type of case. Federal Court is $40 flat rate plus.35 cents per mile and Florida State Court is a $5 flat rate plus.06 cents per mile. Non-Florida State Court cases fees vary by state. Consult with the court in that state to determine the fee. These fees apply unless they fall into the category of the question below. 3) If the document is to be served in Florida and the witness resides in the same county that we re requesting him/her to appear in, is the fee the same? Answer: The fee is flat rate of $7.50 for State Court and $45.00 for Federal Court. 4) Who is the witness fee check payable to? Answer: Witness fee check is payable to the person being served. How to calculate the witness fee mileage check amount: 1) Go to the Map Quest website 2) Find the Directions section 3) In the starting location input the address of the person being served 4) In the ending location input the address of the court /address where they re going to appear 5) Then press Get Directions 6) On the following page find Total Estimated Distance 7) Now multiple the miles by the rate per mile the court demands

Florida Summons Checklist For Cases filed in Florida The following items must be included when you send your Summonses to Judicial Support.
