MEMBERSHIP AGREEMENT & CLUB TERMS AND CONDITIONS/CODE 1. Welcome - Some things you need to know at the start of your membership 1.1 What types of membership can you choose from? Your membership: Your membership allows you to use the club in line with these terms and conditions and club code. Your type of membership sets the times and accessibility for club facilities that you can use. Membership Access Type VIP (Ages 18+) 24hr Gym Access Public Holiday Access Includes All Group Fitness Classes Includes Les Mills GRIT Group Training Sessions Includes Pool Facility Access During Designated Hours Standard PLUS (Ages 12-18) Gym Access During Staffed Hours Only Includes All Group Fitness Classes Includes Les Mills GRIT Group Training Sessions Includes Pool Facility Access During Designated Hours Membership Commitment Type 12 Months Direct Debit: Sign up for a minimum of 52 weekly payments via direct debit from a nominated bank account or credit card. Your agreement can only be cancelled during this time, if you cancel within your 48 hour cooling off period policy (see 1.5) or for other reasons outlined in this booklet (see 3.3). After the minimum 52 payments your membership will continue to run from week to week until you decide to cancel it. The 30 days cancellation notice period still applies as per your membership agreement. Pay upfront: Sign up for a 12 month membership and pay the full amount on the date you join. Your agreement can only be cancelled during this time within the 48 hour cooling off period policy (see 1.5) or other reasons in this booklet (see 3.3)

6 Months 3 Months 1 Month Direct Debit: Sign up for a minimum of 26 weekly payments via direct debit from a nominated bank account or credit card. Your agreement can only be cancelled during this time, if you cancel within your 48 hour cooling off period policy (see 1.5) or for other reasons outlined in this booklet (see 3.3). After the minimum 26 payments your membership will run from week to week until you decide to cancel it. The 30 days cancellation notice period still applies as per your membership agreement. Pay upfront: Sign up for a 6 month membership and pay the full amount on the date you join. Your agreement can only be cancelled during this time under the 48 hour cooling off period policy (see 1.5) or other reasons in this booklet (see 3.3) Direct Debit: Not applicable Pay upfront membership only Pay upfront: Sign up for a 3 month membership and pay the full amount on the date you join. Your agreement can only be cancelled during this time under the 48 hour cooling off period policy (see 1.5) or other reasons in this booklet (see 3.3) Direct Debit: Not applicable Pay upfront membership only Pay upfront: Sign up for a 1 month membership and pay the full amount on the date you join. Your agreement can only be cancelled during this time under the 48 hour cooling off period policy (see 1.5) or other reasons in this booklet (see 3.3) 24 Hour Membership 24 Hour Access Only members with 24 hour eligibility (see 1.1) may access the facility outside staffed hours no exceptions. (see 4.4) Membership Reduced Rate Options Family Memberships Discounted rate available on select membership types to immediate family. Please see the club for more information. Concession Receive a discounted membership on select membership types if you are a full-time student, seniors card holder or on a full-time pension If you are a student, each year, prior to your membership anniversary, you must show us proof that you qualify for a concession. If you no longer satisfy the criteria for a student membership, you will be required to upgrade your membership to the standard membership.

1.2 What is the Start Up Fee? Applies when you join or renew Our Start Up fee applies when you join or renew your membership. It is not refundable, even if you cancel during the 48 hour cooling off period (see 1.5). The amount will be detailed at time of sign up. 1.3 How old do you have to be? You must be at least 12 years old Members must be at least 12 years old to join on any of our membership packages. It is highly recommended that any member between the ages of 12 and 16 years of age also be accompanied by an adult and/or guardian. See in club for what special membership offers we can arrange to satisfy this recommendation. Members under 16 If you are under the age of 18 you only qualify for a Standard PLUS Membership. If you are under 16, it is advised a parent or guardian must complete your pre-exercise questionnaire and an adult must supervise you when you are using our facilities. The club advises you must be at least 16 to do weight-based group exercises or resistance training activities. If a person under the age of 16 chooses to do weight-based training or attend a weigh-based group fitness class they do so under their own risk and cannot hold the club liable for any resulting injuries or health concerns. It is recommended if a member under the age of 16 wishes to do weight-based resistance training they do so under a pre-scribed exercise program from a health & fitness professional whom is qualified to do so. Members under 18 If you are under the age of 18 you only qualify for a Standard PLUS Membership. If you are under 18, we strongly recommend that you undertake a fitness assessment with one of our qualified personal trainers so they can develop you a suitable and safe exercise program. This will be offered to you at a discounted rate. We may specify programs, classes and activities you can do without your parent or guardian s supervision. We may charge extra fees for these, as the law may require us to supervise them more closely. You must be at least 18 years old to be eligible for 24 hour membership. 1.4 When does your agreement start? Signing your agreement You have an agreement with us when you have signed it and

we have accepted it. Terms in this booklet and your agreement apply If this booklet or your agreement differs from anything you are told at the club or over the phone, the terms in this booklet and your agreement will apply unless written confirmation is received from a Forest Lake Fitness authorised employee (i.e. Management). Our rights If we do not enforce our agreement rights at any time, it does not mean we have waived those rights, no matter how long we wait. If there is a miscalculation on your membership application and agreement, we still reserve the right that after the agreement is signed to correct it. 1.5 Can you change your mind? 48 hour cooling off period policy All membership agreements are entitled to a 48 hour cooling off period - This is the first 48 hours from the time you sign the membership agreement. To cancel your membership within the 48 hour cooling off period you must provide the club a written letter of cancellation notice. Once cancellation notice is received the club will then cancel your agreement and a) Upfront Membership: refund your initial payment, less Start Up fee ($99), a $89 cancellation fee and a guest visit rate which is outlined by current club signage within the club per day from commencement of membership. b) Direct Debit Membership: Charge an $89 cancellation fee plus $20 per day from the commencement of membership. No refund will be issued for any Start Up, Access Card or Direct Debit Fees paid - this includes Debit Success Admin Fees. A refund of any fees will be refunded to the member within 21 days of terminating the agreement. 1.6 What about your health? Orientation It is a strict condition of this agreement that you participate in a member orientation program. The orientation will focus on exercise safety, equipment safety and club safety procedures especially for those members with 24 hour entitlements. The club may terminate this agreement in the event that you choose not to participate in the member orientation program due to health & safety reason unsatisfactory completion of the club orientation prior to the commencement of exercise. The member orientation program may be done at the time of signing your agreement or organised by a club staff member for an alternative time if time and availability at time of sign

up is restricted or limited. Guidance When you join the club, we may offer you personal training at a discounted rate. If you choose not to take up the personal training offer, we will take this as an indication that you will have no need for exercise guidance and the club cannot be held accountable. Additional guidance is recommended for members under the age of 16. See section 1.3 Your promise On the day you sign your agreement and each time you use our facilities, you promise to us that: a) You are in good physical condition b) You know of no medical or other reason why you cannot do active or passive exercise that may damage your health, safety, comfort or physical condition. Seek expert advice Our staff are not medically trained. They are not medically qualified to assess if you and your guests are in good physical condition and can exercise without damaging your health, safety, comfort or physical condition. If you or your guests are in any doubt, we strongly urge you to seek expert advice before starting an active or passive exercise program. Before any commencement of exercise at the club you will however be required to fill out a brief medical questionnaire to assess your health and condition. Staff reserve the right to decline anyone access to the facility or furthermore request a medical clearance if they deem the member or guest is in an unfit condition to exercise. 2. Things you need to know during your membership 2.1 When do you pay membership fees? Direct debit ongoing memberships You pay fees for ongoing membership in advance each week, by direct debit from a transaction account or credit card After your direct debit minimum payment term your membership will continue to run week to week as per your membership agreement until you decide to cancel it (see 3.4) Direct debit term memberships You pay fees for ongoing membership in advance each week, by direct debit from a transaction account or credit card After your direct debit minimum payment term payments are complete your membership will end. At this time if you wish to continue to be a member of the club you will be required to renew

your membership agreement (see 3.1). Upfront payments Instead of ongoing memberships, you can pay upfront for a VIP or Standard PLUS access term membership. Please see in club for a list of upfront membership types available When your membership is due to end, you can renew it before the end date please see the club for current renewal rates. For more information on upfront membership renewals (see 3.1). 2.2 Your Direct Debit Agreement 2.2.1 How will your account be debited? Debit Success Initial terms Payment days Debit Success is the debiting agency that handles all direct debit membership payments on behalf of Forest Lake Fitness. By Signing up to any VIP or Standard PLUS access direct debit membership you also agree to the terms of agreement from Debit Success. This will be outlined on your Debit Success Payment Plan. Debit Success will regularly debit your weekly club fees from your nominated account at a specific time, as agreed in your membership application and agreement. Debit Success will debit fees: o On the designated day as agreed in your membership application and agreement each week. o If a payment day falls on a public holiday, Debit Success will debit your account on the next business day. Please note that debits may take up to 5 business days to come out of your nominated account. Privacy Statement Please visit http://forestlakefitness.com.au/privacypolicy.pdf for the clubs full privacy statement. 2.2.2 What are your responsibilities for direct debits? Your responsibilities Your are responsible for: o Making sure your account can accept direct debits (your financial institution can confirm this) o Making sure there is enough money in your account on the scheduled payment day up to and including the 5 following business days. o Inform us in advance (at least 48 hours before your next direct debit) if you are transferring or closing your

Changes by you account o Inform us in advance (at least 48 hours before your next direct debit) about any changes to your credit card, such as expiry date or number. Please inform both the club and your financial institution if you want to change your direct debit arrangements. To stop or for cancellation of direct debits and membership please refer to 3.2. What if you have a question? If you query a payment with us, we will do our best to respond within 7 days. If you do not receive a satisfactory response from us, you may contact your financial institution or Debit Success on 1800 148 848. They will handle your query in line with their policy. 2.2.3 What happens if your payment fails? Club access will be suspended If you do not pay your fees on the due date, we will suspend your club access until your payments are up to date. Debit Success will also charge you a late payment fee of $10.00 for a failed payment. This will be debited from your account, and you authorise Debit Success to do this as part of your membership agreement. Debit Success will debit your account Debit Success will continue to debit your nominated account without notice, until they have received the total amount owing. Debit Success will make a reasonable effort to let you know before Debit Success do so either by phone, via SMS or in writing to the address you last gave us. For membership/s in your name, you must ensure that the payment method you choose on your agreement continues throughout your agreement term, including nominated third party accounts. If the account details you supply us with are incorrect and payment is declined, you will be liable for all the resulting fees and debts. Please note that continual failure of payments can result in Debit Success referring you to debt recovery which can lead to excessive fees. 2.3 Can we change the terms of your agreement? Changes Club terms and conditions, membership conditions and code are subject to change. This includes but is not limited to changing the club s operational hours, its services, facilities, pool availability and membership fees. From time to time, to improve our facilities, we may choose to close the club for up to 3 consecutive days for refurbishment. The most up-to-date terms and conditions always apply. You

can request a copy at the club, email your request for a copy or download a copy at our website http://forestlakefitness.com.au/flfterms&conditions.pdf We will give you 6 weeks We will give you at least 6 weeks notice of any such changes by: o Publishing them in our newsletter, or o Placing a notice in the club, or o Writing to the address you last gave us, or o Contacting you by phone, or o Contacting you by email, or o Advising on our website, or o Advising on our Facebook page Suspension If we suspend the clubs operations or services for greater than 7 days you may be entitled to temporary membership suspension. 2.4 Can we increase fees? Changes We reserve the right to increase your fees at any time after the minimum period of your agreement has ended if not specified in your membership agreement s special conditions. How will we inform you? We will make a reasonable effort to tell you about this at least 6 weeks beforehand by correspondence (email, phone, text, mail). We will acknowledge that you have received our correspondence on the second business day after it is sent. Your authorisation Where we have made all reasonable efforts to inform you about any membership fee increase without your response, you authorise us to increase any debits from your nominated account in line with this increase. 2.5 Can you suspend your membership payments? Suspensions Conditions You can suspend your membership in multiples of 2 weeks, up to a maximum of 8 weeks for each membership period exceeding 26 weeks. Each suspension period must start and end on a fee payment day. VIP and Standard PLUS access members will be charged a $5 fee per week your membership is suspended. Note: If you are within the minimum agreement payment term, suspension payments are not classified as a full payment toward your minimum agreement payment period. Prior to suspension of your current membership, any fees due or outstanding must be up to date.

Upfront memberships Prior to any suspension requests can be processed by the club we must receive payment in advance of the requested suspension date. Direct debit memberships All members on direct debit will need to call Debit Success on 1800 148 848 to arrange their suspension. Backdated suspension We cannot back date any suspension requests; you remain responsible for requesting a suspension when it is required. 2.6 Using your access card Receiving your access card You will receive your access card at time of sign up. The cost of your access card will be included in your membership set up fee unless otherwise specified. Photo identity We will attach a copy of your photo identification to your account on our digital membership database. If you visit the club without a valid access card or photo identification, we may refuse you entry. No card lending Your access card is not transferable to anyone else You must not allow anyone else to use it. Lost or Damaged Cards Lost or damaged cards will incur a replacement fee of $25. 2.7 Protect Your Health Health Risks If you believe there is a risk to your health from doing any club fitness activities, you must tell us this in writing, with full details. You must also tell us if your medical condition changes after you join. We may choose to refuse/suspend your membership application, until: o Your doctor agrees in writing to a club exercise program, or o You show us proof that you have received medical advice from a qualified medical practitioner giving you clearance to participate in an unguided exercise regime. Infections and illness You will not use club facilities while suffering from: o An infection, or o A contagious illness, or o A physical ailment, such as open wounds, or o Where there is any other risk, however small, to other members and guests

2.8 Your feedback and complaints We welcome your feedback Your feedback and complaints are important in helping us to resolve your concerns quickly and effectively. If you have any feedback or questions about our club, our website or our service in general, please contact us via one of the methods listed below. Club Staff Our staff are happy to help you with any questions or feedback you have. If you would prefer to speak with a member of the club management please let our reception staff know. If they aren t immediately available the reception team will ensure you are contacted by them as soon as possible. Email Online Send a detailed email of your feedback or complaints to admin@flf24.com.au and a member of our management team will get back to you as soon as possible. All feedback or complaints are confidential and only seen by authorised club personnel. Alternatively go online and visit our website where you ll find our feedback form to complete. Every attempt will be made to contact you within 5 business days where possible (sometimes longer over a weekend or public holiday). We recognise that some enquiries are more complex than others and may require more time to resolve, but we will always let you know. 3. Things you need to know at the end of your membership 3.1 Renewing your membership Paid upfront If you paid for your membership upfront you may be contacted by the club in regards to your membership renewal and options available. You can renew your membership at any stage during your current membership term. Once renewal is complete your next membership period will start at the conclusion of your current membership period By choosing to renew your paid upfront membership by at least 7 days in advance you will be eligible for a discounted renewal rate based on the clubs current membership rates. Failure to renew your membership 7 days prior to your current membership expiry date will result in any special renewal offers to become void. Set to term direct debits If you have a direct debit membership and your payment term is not ongoing after you last minimum payment you will be required to renew your membership by either: o Signing up to another direct debit membership, or o Signing up to a pay upfront membership

If you choose to renew your membership by taking out another direct debit option you will incur any fees for the set up of this membership from Debit Success. 3.2 Ways of cancelling your direct debit membership How to cancel You can request to cancel your membership by contacting the club by any one of these ways: o Email us at Cancellations@flf24.com.au (preferred) o Call Debit Success direct on 1800 148 848 o Visit us in person o Phone us If you choose to visit us in person or inform us via phone we will still need written notification via email for record keeping. If you have not has a response to your email notification in 2 business days please contact the club to ensure the email was received. If you choose to cancel directly through Debit Success you will be contacted by a member of Forest Lake Fitness staff to confirm your request and email address and they will send you a Request to Cancel form to complete and return. Keep evidence of your cancellation request In all scenarios above please ensure you give us and utilise your email address so we can confirm your cancellation request via a reply email and send you a Request to Cancel form to complete and return (if applicable). Please keep a copy of any cancellation request you make. Please make sure that you also cancel any direct debit arrangements by contacting Debit Success on 1800 148 848. Request to Cancel Form After you have given notice of your cancellation by one of the methods listed in 3.2 you will be required to complete a Request to Cancel form before your cancellation notice period will commence. The Request to Cancel form will be emailed to the email address issued to Forest Lake Fitness. Please ensure your email address is up to date by checking what is listed with a member of Forest Lake Fitness Staff. It is your responsibility to fill out and return the Request to Cancel form either by emailing Cancellations@flf24.com.au or in person before the 30 days notice period commences on the cancellation. If a Request to Cancel form is not returned to Forest Lake Fitness the cancellation will not be processed. A Request to Cancel may be declined if the member wishing to cancel is in arrears of their membership payments. You will be required to contact Debit Success to arrange catch up payments and once the outstanding fees are paid the cancellation can be processed.

3.3 Can you cancel during the membership/minimum payment term? Cancelling and paying out your agreement You can cancel your direct debit membership at any time during the minimum payment term. A cancellation fee of $250.00, unless otherwise specified, will apply as per your membership agreement and conditions. 30 days notice period also applies see 3.4 Cancelling due to a terminal illness or physical incapacitation You can cancel your membership during the minimum term if you become terminally ill or physical incapacitated (see 5.9 for definitions). You must provide us with a certificate from a qualified medical practitioner stating your condition and that it is recommended you do not exercise for the remainder of your term. If the member has a paid upfront membership a portion of the fees will be refunded within 21 days of termination of the agreement also less any fees for fitness services supplied and a $89 administration fee of the total membership cost and a guest visit rate which is outlined by current club signage within the club per day from the commencement of the membership. For definition of 'Terminal Illness' and 'Physical Incapacitation' relating to these terms & conditions see 5.9. Transferring your membership to another person You (transferor) can transfer your membership to another person (transferee) within the minimum term of your agreement provided that the transferee: o Completes the new membership sign-up process; o Is eligible to take up the transferor s membership, e.g. a concession membership cannot be transferred to a person who would not qualify for that membership; and o Has not been previously banned by Forest Lake Fitness or failed to pay their membership fees when due o The transferee is not a current member We do not allow transfers once you have completed the minimum term of your membership agreement. Any complementary or promotional memberships are not transferable. The transfer is effective only after the transferee has completed the new membership sign-up process. A transfer fee of $65 plus any 24 hour access fee s will apply and is payable by the transferee. We do not allow transfers if any accounts are in arrears. All membership payments must be up to date before a member can transfer their membership. Cancelling for other reasons If you wish to cancel for any other reason and have more than 4 weeks remaining of your minimum term, you can pay the cancellation fee, as agreed in your membership application and agreement. Request to Cancel Form As with section 3.2 a Request to Cancel form is to be completed and returned for all cancellations to be processed.

3.4 How do you cancel direct debit memberships after the minimum term? Cancellation If your direct debit membership is ongoing, after the minimum payment term has surpassed you or we may cancel your membership at no cost. Please see 3.2 for ways of canceling your direct debit membership. 30 days notice 30 days notice is required to cancel. This notice is taken from the next available direct debit date, Should you wish to ensure your debit does not continue past your minimum term, you must give us this 30 days notice within your minimum term. Use one of the ways to cancel in 3.2 No suspensions You cannot suspend your membership payments during the 30 days notice period. You must pay any outstanding fees When you cancel your membership, you must first pay any fees owed to us, or we/debit Success may take action to recover them. Request to Cancel Form As with section 3.2 a Request to Cancel form is to be completed and returned for all cancellations to be processed. 4. Club Code Signs and handouts You must follow club code. Read, understand and follow the code in all signs and handouts. 4.1 What happens if you break the code? Refusal of entry We can refuse entry to anyone, including members Warnings If you act unreasonably, or break these terms and conditions or this club code, you risk having your membership cancelled immediately. Instant cancellation We can cancel your membership without warning if you behave in a way that is risky or seriously inappropriate, such as: o Threatening or harassing others including staff o Swearing or abusive behavior o Damaging property or equipment o Using illegal or performance-enhancing drugs o Selling additional services and supplements o Providing instruction to other members where you are not

authorised by us to do so. 4.2 Use equipment correctly Using equipment You are responsible for using our facilities and equipment correctly, including adjusting levels or settings. If you are not sure how to operate any equipment, please see a club staff member during staffed hours before you use it. If you feel there is any risk to you when using the equipment it is strongly recommended by the club you seek professional help from a club trainer. This can be in the form of Personal Training which may be offered to you at a discounted rate. 4.3 Respect club facilities Damage You are responsible for any damage to club facilities caused by you or your guests through a willful act or negligence. 4.4 24 Hour 24 Hour Access Unstaffed hours Only members with 24 hour eligibility (see 1.1) may access the facility outside staffed hours no exceptions. Any member who brings a non-member or member without 24 hour access into the club acknowledges that o They accept responsibility and liability on their personal behalf for any injury, loss or damage attributed to the non-member or member without 24 hour access whether or not caused through the negligence of Forest Lake Fitness. o The member s membership to Forest Lake Fitness will be terminated with no refund and a $150 noncompliance fee charged to the member s nominated bank account or credit card. o The member will be prohibited from re-joining or visiting Forest Lake Fitness. Forest Lake Fitness operates on a 24 hour basis and is accessible by 24 hour members only (See 1.1) outside staffed hours As a 24 hour member using the facilities during unstaffed hours you acknowledge that: o You enter and use the facilities at your own risk. o If you feel there is a risk to you, your personal property, health and safety, or you have any other concerns regarding the undue effect of exercising in an unstaffed facility, DO NOT sign this agreement or take out 24 hour membership. The security company and its representatives engaged by Forest Lake Fitness have authority to enforce these Terms and Conditions and Gym Code at any given time.

Staffed Hours 4.5 Lockers The staffed hours of the club are displayed at the entrance to the club and on the clubs website www.forestlakefitness.com.au The club has the right to make amendments to staffed hours at any stage without notice. Bags Bags are prohibited on the gym floor and any group fitness studios. Lockers are provided by the club for you to keep your belongings in. Keep your belongings safe We provide lockers for you to use whilst exercising. These lockers are for added security, so please keep valuable items with you or at home. We try to safeguard the contents of your locker, but thefts can occur. We do not accept responsibility for any loss or damage to any belongings not kept in a locker, or if someone breaks into your locker. Please feel free to bring along your own lock to use on the lockers whilst you are exercising to minimize risk of theft or damage. Belongings left in lockers If you leave belongings in a locker overnight, we may remove them. We give lost property to charity each month, including unclaimed possessions from lockers. It is your responsibility to ensure all items from your locker are removed before you leave the club. Reserving Lockers You cannot reserve any locker. This includes: o Leaving your lock on a locker, or o Leaving any items in a locker 4.6 Clothing and hygiene You must wear suitable clothes All members and guests must wear suitable clothes and enclosed sports shoes in any exercise areas, except for wet areas. We do not allow clothes with offensive images or inappropriate advertising. You must use a towel No Towel, No Workout! Please use a clean towel when you use equipment, including exercise mats. A towel does not include: o Face washer, or o Hand Towels, or o Tea Towel, or o Clothing

4.7 Wet areas Take care in wet areas Wet areas are unsupervised and you use them at your own risk. We do not allow running or jumping in wet areas. Children must be supervised. You must comply with all signage or code in these areas. 4.8 Vehicles You park at your own risk You park in the club s car park or on club premises at your own risk. We are not liable for any loss or damage to your vehicle or its contents. 4.9 Time limits Keep to time limits Please keep to set time limits for parking and using equipment. If not, we may charge you a fee decided by us from time to time. 4.10 Guests Bringing a guest/s to the club You can arrange to bring a guest/s into the club during the clubs designated staffed hours for a fee outlined on club signage. We decide how many club visits a guest can make at our discretion. Please call the club for more information. Any guest must: Be at least 13 years old Be accompanied by you and leave when you do Sign the guest register and show photo identity Complete a guest registration form and pre-exercise questionnaire on their first visit Follow the code and terms set out in the guest register, guest form, and any other club code on signs and handouts. Children in the club Child minding is available to all members and guests for a small fee. For security reasons, children can only wait in the club if accompanied by an adult who s seated with them. For child minding enquire at club reception for details. Children are strictly prohibited from entering the gym floor. At no time are children to be left unattended in the club s foyer whilst you re on the clubs premises. While your child is in child minding you must remain on the clubs premises. If you decide to use the pool please inform child minding staff 4.11 Cameras and mobile phones Courtesy As a courtesy to other members and safety, please keep taking

and receiving calls to an absolute minimum. Cameras aren t permitted in the club without club authorisation. At no times are cameras or mobile phones with cameras to be used in the clubs wet areas. 5. Additional information you need to know 5.1 Can we transfer your agreement? Transfers We can assign or transfer the benefit of your contract to a person, firm or company at any time. We will give you 6 weeks notice in writing. We can also sub-contract your obligations to someone else without notice but if we do, we are still responsible for ensuring those obligations are met. 5.2 What if the GST rate changes? Fees include GST All agreement fees include goods and services tax (GST). Your fees will change in line with any government GST rate changes, even if you are in your minimum term. 5.3 Keep us informed Your responsibilities Your responsibilities, including payment of membership fees, are not dependant on how often you use a club. Changes to details You must tell us about anything that affects your membership, and any changes to your contact and bank details Separate fees and agreements Extra services such as personal training and child minding are not part of our agreement. They involve separate fees, code and agreements please become familiar with them. 5.5 Security Video surveillance For security and safety purposes Forest Lake Fitness uses video surveillance equipment to monitor the club on a 24 hour basis. By signing your membership agreement you acknowledge that by accessing the club you will be subject to video surveillance and recording. Video surveillance of the club includes gym floor, group fitness classrooms, car park and any other accessible areas where video surveillance is permitted. There is no video surveillance within the clubs bathrooms or client screening room. 5.6 Privacy statement and acknowledgement Our privacy policy When you join and during your membership, we will have access

to personal information about you, such as about your health and financial situation. We will only use, disclose or deal with your information in accordance with our privacy policy.
