Steps to Becoming a Salem State University Direct Entry/MSOT Student Let us know you plan to enroll (matriculate) Email gradadmissions@salemstate.edu, including your name, ID and graduate program. Visit the Graduate Student Orientation page for information related to your program and information on services you may find helpful. Schedule an appointment with your program coordinator within one month from the date of your acceptance into your graduate program. You will work together to develop your plan of study which outlines which courses you will take and when, as well as any other program requirements, To set up an appointment with your program coordinator, Dr. Jeramie Silveira, please email her at jsilveira@salemstate.edu. YOU MUST MEET WITH YOUR PROGRAM COORDINATOR AND SIGN YOUR PLAN OF STUDY BEFORE REGISTERING FOR FALL COURSES. We strongly recommend that you meet with your program coordinator regularly to ensure you are making satisfactory progress toward your degree and are aware of any changes in requirements. Return your immunization records and physical exam must be submitted to the Office for Student Health. Additionally the information below needs to be submitted to Sandy Krushenick in the Occupational Therapy Department: Follow the instructions on the enclosed forms and return forms by Sept 1 to: Sandy Krushenick, Salem State University, Harrington Building, Room 301, 352 Lafayette Street, Salem, MA 01970 or by fax to (978) 542-2073 or by email skrushenick@salemstate.edu. All students will need to have the following: A. CPR certification for the Health Care Provider (A copy of current card must be submitted) B. Liability insurance, purchased at the Bursar s Office, Student Navigation Center, Central Campus Copy of receipt must be submitted to Sandy Krushenick C. Proof of current Health Insurance, copy of card D. HIPPA Certification (these are not needed upon admit, you will need this once you begin your fieldworks which is a year and a half into your program) E. AOTA Membership card: membership in the American Occupational Therapy Association is required (A copy of the card must be submitted) Register for your courses Go to Navigator and register for the following courses: OCT 724, OCT 700 and OCT 912 (if Statistics prerequisite was not completed) If you already have a Navigator account, use your current Navigator user ID and password. If you do not have a Navigator account, use S plus your student ID, which is on your admission letter (for example, S1234567). Your password is the first two letters of your first name, first two letters of your last name and your birth date: MMDDYY (for example, John Doe with a birth date of July 23, 1980 would be jodo072380). Please note the letters in your password are lowercase. 1

To speak with a representative, call 978.542.6112. Be sure to indicate that you will be a graduate student and ask for the graduate financial aid representative. Students who are conditionally accepted may not apply for federal financial aid until all of the conditions of acceptance have been met and they are fully accepted. Conditionally accepted students may apply for private/alternative loans and enroll in the payment plan. Please be sure to direct any questions to the graduate financial aid representative. Please note that federal financial aid and graduate assistantships are not available for the graduate certificate or licensure programs (except CAGS). However, if you are enrolled in a certificate program you may apply for private/alternative loans. You may also enroll in a payment plan, administered by the Bursar's Office. Immunizations - The Commonwealth of Massachusetts requires immunizations for full time students (students enrolled in at least 9 credits). Download and complete the Health and Immunization Form found on the webpage under the heading Student Supports. Health Insurance - The Commonwealth of Massachusetts mandates that all students have health insurance. Graduate students enrolled for 9 or more credits will be automatically assessed the health insurance fee by Salem State University. However, students who have comparable coverage can waive out of the plan. Student Health Insurance can be found on the webpage under the heading Student Supports. Please enroll in or waive out of the Salem State University Student Accident & Sickness Insurance Plan. Register for courses after you develop your personal plan of study with your program coordinator. Registration information can be found on the webpage under heading Registration. This website is updated each semester. Matriculated graduate students are not required to pay for their courses at the time of registration. Email notifications are sent out when an ebill is available for viewing. Set up your official Salem State email account after you have officially matriculated Emails pertaining to official School of Graduate Studies business will only be sent to your official Salem State email account. Your official email address will most likely be firstinitial_lastname@salemstate.edu (i.e., j_doe@salemstate.edu. A number may be added if there are multiple students with the same first initial and last name.) If you already have a Salem State email account, use your current logon information. To log into your new email account, follow these steps: 1. Set your password at salemstate.edu/reset. 2. Log into your new email at salemstate.edu/email. Use S + your seven digit student id + @salemstate.edu as your logon ID (i.e., S1234567@salemstate.edu), along with the new password you just set in step 1. Information regarding setting up your email account can be found on the webpage under the heading Student Supports, or by calling 978.542.2036. Obtain a parking pass - Parking pass information can be found on the webpage under the heading Additional Information. 2

Obtain a ClipperCard - Your ClipperCard is your official ID card. Clipper card information can be found on the webpage under the heading Financial. OT Graduate Student Handbook - This document contains information, policies and procedures for the occupational therapy direct entry-program. Information about the department s mission, philosophy, objectives, curriculum, fieldwork, general policies, graduation and student resources are included. First, suggestions are provided to help you connect with your program advisor and the support staff at the university who are here to help you. Second, the process for mapping out your academic plan, and the policies that serve as the rules of the road are provided to assist you in your process. Finally, information regarding available academic, professional, and personal resources is provided. We strongly suggest that you read this document. IMMUNIZATIONS/PHYSICAL 1. PHYSICAL EXAM: Updated physical within the past year prior to admittance into the Direct Entry program. 2. T.D. (Tetanus/Diphtheria): Within 10 years. If this outdates while you are here, the college requires a Booster immunization. 3. MMR (Measles, Mumps & Rubella): Students must have one MMR and a second Measles vaccine after 1980 or a titer for Rubella. 4. HEPATITIS B VACCINE: Proof of Hepatitis B Vaccine (3 doses) dates are submitted as part of the health record. Laboratory proof of immunity by positive titer will be accepted. 5. VARICELLA (CHICKEN POX): Proof of Chicken Pox immunity. Date of illness from physician, dates of vaccinations (2 doses), OR if neither of those two are available a Varicella titer is needed with or without Numerical level results. Varicella titer can be drawn at Salem State University Student Health Service for a fee of $10. If the titer is negative the Chicken Pox Vaccine (2 doses) is required. CLINICAL REQUIREMENTS: 1. Copy of Current CPR card 2. Current health insurance coverage either through the university insurance plan or by another policy. For more information on the student health insurance visit: http://www.salemstate.edu/chs/insurance.php. Health Insurance is required of all students by Salem State University. Clinical sites also require that you have health insurance to cover any illness or injury that you may incur during your clinical training experience. 3. Liability Insurance (renewal yearly, purchased through the Bursar s office for $15, a copy of the receipt must be kept on file in the Compliance Office HB220) 4. TB (PPD) (MANTOUX) TEST: Required every year. Students who have received BCG vaccine in their home countries and who have a positive PPD are assumed to have latent TB. Any student with a positive PPD MUST have a chest x-ray done and be evaluated by the Pulmonary Clinic at North Shore Medical Center Salem Hospital. Students with a negative chest x-ray and no symptoms of TB will be offered a 9-month course of INH. Students completing the course of INH require no further PPD or chest x-rays. Students with a negative chest x-ray who decline the INH treatment will be required to complete a questionnaire and be evaluated on a yearly basis by a physician or nurse practitioner in order to be cleared for clinical. Students who are symptomatic with a positive PPD and chest x-ray will be treated for active TB. 5. Flu Shot: Yearly Flu vaccinations are required for all Salem State occupational therapy students. Flu declinations can be completed for certain stipulations.. 6. Up to date Yearly physical is required to be submitted to the Office of Student Health. 4

LOCAL CPR COURSE INFORMATION Professional level CPR Certification: infant, child and adult technique is required for all students in the School of Nursing. The certification course must require both written exam and demonstrated proficiency on a mannequin. Internet CPR courses that certify individuals without practice on a mannequin are not acceptable. Please bring a photocopy of your CPR card (front and back) to Compliance Office. If you are not yet certified, the Preferred Vendor of Salem State is listed below.
