What this means for you: when you open an account, we will ask you for your name, address, date of birth, and any other information that will allow us to identify you. We will ask for your driver s license and another piece of valid identification or other identifying documents. PERSONAL CHECKING ACCOUNTS * Personalized check prices for all checking accounts vary with style and design. The cost of checks will be deducted from your account. HEARTLAND FREE CHECKING ACCOUNT This is a non-interest bearing account. REQUIREMENTS/FEES You must deposit $50.00 to open this account. There is no minimum balance requirement. There is no monthly service charge. There are no activity charges. Unlimited ATM transactions at all Heartland Bank locations at no charge. A charge of $1.00 will be assessed for any ATM debit transactions at non-heartland Bank ATMs. Miscellaneous fees may be assessed against your account. Please refer to the schedule of fees for a complete listing of these charges. Please refer to our Rules and Regulations for information regarding foreign ATMs. Page 3

PERSONAL SAVINGS ACCOUNTS HEARTLAND BASIC SAVINGS ACCOUNT RATE INFORMATION The interest rate on your account is % with an annual percentage yield of %. At our discretion your interest rate and annual percentage yield may change at any time. Interest is paid on balances above $50.00. REQUIREMENTS/FEES You must deposit $25.00 to open this account. A monthly maintenance fee of $2.00 will be assessed on this account for a balance less than $200.00 on any day of the month. You may make no more than six (6) withdrawals or transfers per statement cycle. An activity charge of $.50 per debit item will be assessed for over six (6) items per statement cycle. Unlimited ATM transactions at all Heartland bank locations at no charge A charge of $1.00 will be assessed for any ATM debit transactions at non-heartland Bank ATMs. Miscellaneous fees may be assessed against your account. Refer to the Schedule of Fees for a complete listing of these charges. Monthly maintenance fees, activity charges, and miscellaneous fees may reduce your earnings on this account. BALANCE COMPUTATION METHOD We use the daily balance method to calculate the interest on your account. This method applies a daily periodic rate to the principal in the account each day. COMPOUNDING AND CREDITING Interest for your account will be compounded monthly and credited to your account every month. Your monthly statement will reflect all interest earned. ACCRUAL OF INTEREST ON DEPOSITS OTHER THAN CASH Interest begins to accrue on the business day you deposit non-cash items (For example: checks). Page 4

ACCOUNT AGREEMENT CERTIFICATES OF DEPOSIT This section contains the terms of our agreement with you. These terms cannot be varied orally. Handwritten and typewritten terms are presumed to be consistent with printed terms, but will control if clearly intended to vary the printed terms. DEFINITIONS: "We" means this financial institution "You" means the depositor(s) "Certificate" means both this original instrument as well as the deposit it evidences "Transfer" means any change in: ownership, withdrawal rights, or survivorship rights, including (but not limited to) a pledge of this certificate as security for any obligation PRIMARYAGREEMENT: You agree to keep your funds with us in this account until the maturity date. (An automatically renewable certificate matures at regular intervals.) You may not transfer this certificate without first obtaining our written consent. You must present this certificate when you request a withdrawal or transfer. This certificate is void if the deposit is made by any method requiring collection (such as a check) and the deposit is not immediately collected in full. If the deposit is made or payable in a foreign currency, the amount of the deposit will be adjusted to reflect final exchange into U.S. dollars. No additional deposits to this certificate are permitted during the term, unless allowed by product type. If any notice is necessary, you all agree that the notice will be sufficient if we mail it to the address that appears on the signature card. You must notify us of any change. WITHDRAWALS AND TRANSFERS: Only those of you who sign the permanent signature card may withdraw funds from this account. (In appropriate cases, a court-appointed representative, a beneficiary of a trust or a pay-on-death account whose right of withdrawal has matured, or a newly appointed and authorized representative of a legal entity may also withdraw from this account.) The specific number of you who must agree to any withdrawal is written in the section bearing the title Number of Endorsements. This means, for example, that if two of you sign the signature card but only one endorsement is necessary for withdrawal, then either of you may request withdrawal of the entire account at any time. These same rules apply to define the names and the number of you who can request our consent to a transfer. PLEDGES: Any pledge of this certificate (to which we have agreed), must first be satisfied before the rights of any joint account survivor, pay-on-death beneficiary, or trust account beneficiary become effective. For example, if one joint tenant pledges the certificate for payment of a debt then dies, the surviving joint tenant s rights in this certificate are subject first to the payment of the debt. ACCOUNT TYPES: The following paragraphs explain how the various types of accounts listed below operate. Only the paragraph corresponding to the account type selected will apply. Individual Account - Such an account is issued to one person who does not intend (merely by opening this account) to create any survivorship rights in any other person. Page 5

Joint Account With Survivorship (And Not As Tenants In Common) - Such an account is issued in the names of two or more persons. Each of you intend that upon your death the balance in the account (subject to any previous pledge to which we have consented) will belong to the survivor(s). If two or more of you survive, you will own the balance in the account as joint tenants with survivorship and not as tenants in common until you inform us of a change in ownership or close this account. Multiple Depositors - No Survivorship (As Tenants In Common) -Such an account is issued to two or more persons but none of you intend (merely by opening this account) to create any right of survivorship in any other person. We encourage you to agree and tell us in writing of the percentage of the deposit contributed by each of you. This information will not, however, affect the number of endorsements necessary for withdrawal. Trust Account (subject to this form) and Pay-On-Death Account - If two or more of you create such an account, you own the account jointly with survivorship. Beneficiaries acquire the right to withdrawal only if: 1) all persons creating the account die, and 2) the beneficiary is then living. If two or more beneficiaries are named and survive the death of all persons creating the account, such beneficiaries will own this account in equal shares, without right of survivorship. The person(s) creating either of these account types reserves the right to: 1) change beneficiaries; 2) change account types; and 3) withdraw all or part of the deposit at any time. Trust Account Subject to Separate Agreement - We will abide by the terms of any separate agreement which clearly pertains to this certificate and which you file with us. Any additional consistent terms stated on this form will also apply. Non-Personal Account - Such an account is issued in the name of a legal entity such as a corporation. We reserve the right to require the governing body of the legal entity to give us a separate authorization telling us who is authorized to act on its behalf. We will honor such an authorization until we actually receive written notice of a change from the governing body. SET-OFF: Each of you who has the right to withdraw from this account agrees that we may set-off any debt you owe us now or later against the amount of money you could withdraw from this account. For example, if any one of three of you can withdraw all the money from this account, then the debt of any one of you can be set-off against the balance in this account (even though the others are not obligated on the debt). We may exercise this right of set-off, without notice to you, any time your debt is in default. Under certain conditions, we will not exercise this right of set-off. For example: 1) If this certificate is part of an I.R.A. account or a qualified retirement plan, 2) if the debt arises from a consumer credit transaction under a credit card plan, 3) if your debt is personal and your right of withdrawal arises solely in a representative capacity (or vice versa), or 4) if such exercise is expressly prohibited by law. However, if your debt is in default, we reserve the right to exercise any other remedies available to us under the law or separate agreement. NOTICE OF PENALTY FOR EARLY WITHDRAWAL Heartland Bank s deposits are subject to all applicable Federal and State Laws and Regulations and Rules and Regulations of this Bank as are in force or as they may hereafter be changed or amended. Except as mentioned below, you cannot withdraw any principal from this account before a maturity date and we will charge a penalty. We can only consent to an early withdrawal at the time you request it. Federal Regulations require a minimum penalty on certificates of deposit redeemed prior to maturity. The penalty for time deposits with original maturities of 90 days or less is a forfeiture of one month s interest that would have been earned if the deposit had been held until maturity. The penalty for time deposits with original maturities, 91 days but less than one year, is a forfeiture of three months interest, on time deposits with original maturities one year or greater but less than three years is a forfeiture of six months interest and on time deposits with original maturities three years or greater, a forfeiture of twelve months interest. These penalties on the amount withdrawn are regardless of the length of time the funds withdrawn have remained in the account and therefore, could result in a loss of a portion of the principal of your deposit. Under current Federal Regulations, penalties will not be imposed in situations where a depositor dies or is declared mentally incompetent. We reserve the right to deny early redemption of a time deposit even though we have never exercised the right. Page 7

Your certificate will mature on. The APY assumes interest remains on deposit until maturity. A withdrawal will reduce earnings. REQUIREMENTS You must deposit $500.00 to open this account. BALANCE COMPUTATION METHOD We use the daily balance method to calculate the interest on your account. This method applies a daily periodic rate to the principal in the account each day. COMPOUNDING AND CREDITING Interest on your account will be compounded and credited to your certificate monthly. ACCRUAL OF INTEREST ON DEPOSITS OTHER THAN CASH Interest begins to accrue on the business day you deposit non-cash items (For example: checks). TRANSACTION LIMITATIONS After the account is open, you MAY NOT make deposits into or withdrawals from the account until the maturity date. EARLYWITHDRAWAL PENALTY If you withdraw any principal before the maturity date, the following penalties will apply: Certificate Maturity Interest Penalty 90 days or less One (1) month 91 days to less than one (1) year Three (3) months One (1) year or greater but less than three (3) years Six (6) months Three (3) years or greater Twelve (12) months The penalty we may impose on the entire principal of this certificate will equal months of interest. RENEWAL POLICY This account will automatically renew at maturity. You have a grace period of ten (10) calendar days after the maturity date to withdraw the funds without being charged a penalty. Overdraft Protection Service Disclosure Heartland Bank offers our consumer customers an Overdraft Protection Service of $100.00* at account opening to protect you from the embarrassment of returned checks, ACH s (automated clearing house items) ATM and Debit card transactions for insufficient funds. While the Overdraft Protection Service is a cumulative amount for all items, in order for us to pay your ATM/debit card transactions you must agree to it by giving your consent on the accompanying Opt-In form. All items are paid at the bank s discretion. It is your obligation to keep your account in good standing. Heartland Bank reserves the right to cancel this service upon written notification to you. Each item presented against the account with insufficient funds will incur a fee of $31.00 for each item that is paid. Any item that is returned is also charged a $31.00 fee. Repayment of the overdraft balance is expected immediately. Any deposits made will first be applied to any overdraft balance. However depending on the situation, you may have up to 30 days to pay any overdraft balances.
