It is not a contract. Terms and conditions of the coverage are set forth in policy number 2015I5A07 issued in Vermont for Champlain College. Please keep this material with your important papers. Form No.: 15-I5A07 (Bro.) OVERVIEW OF PLAN Champlain College offers its eligible students a comprehensive Student Health Insurance Program providing coverage for Accident and Sickness expenses. Premium - Annual Premium (including the cost of managing the plan) is $1,680 (per semester premiums are prorated). NOTE: All Allied Health students are required to purchase the insurance unless proof of coverage from a personal plan that is equal to or exceeds this plan is provided to the College by August 28, 2015. EFFECTIVE AND TERMINATION DATES Effective Dates: Insurance under the Policy will become effective on: 1. August 9, 2015 to August 8, 2016 for Annual Plan. 2. August 9, 2015 to December 31, 2015 for Fall Semester. 3. January 1, 2016 to August 8, 2016 for Spring Semester. 4. All eligible full-time students are automatically enrolled into the plan unless they submit proof of other coverage by completing the student insurance waiver online by August 28, 2015. Termination Dates: An Insured Person s insurance will terminate on the earliest of: 1. The date this Policy terminates for all insured persons; or 2. The end of the period of coverage for which premium has been paid; or 3. The date an Insured Person ceases to be eligible for the insurance; or 4. For students who graduate in December, or who are no longer enrolled at mid-semester, coverage expires on 12/31/15, and students entering in January are covered only for the semester beginning January 1, 2016; or 5. The date an Insured Person enters full-time active duty in the Armed Forces (upon receipt of proof of such active duty, a pro-rata refund of premium will be made.) Except for medical withdrawal due to a covered Injury or Sickness, any student withdrawing from the Policyholder s school during the first thirty-one (31) days of the period for which coverage is purchased shall not be covered under the Policy and a full refund of the premium will be made. If a claim has been paid there is no option for a refund of premium. Students withdrawing after such thirty-one (31) days will remain covered under the Policy for the full period for which premium has been paid. No refund will be allowed. PLAN COVERAGE Student Health Insurance Program Information This brochure is a brief description of the Student Health Insurance Program. This plan is underwritten by Companion Life Insurance Company. The exact provisions governing this insurance are contained in the master policy (referred to below as this policy or the policy ) issued to the College and may be viewed at the Finance Office. Please retain this brochure for future reference. The master policy shall control in the event of any conflict between this brochure and the policy. ELIGIBILITY All registered full-time traditional undergraduate students* (12 credits or more, with no more than 50% online courses) and MFA students will automatically be enrolled and billed for Student Health Insurance offered by Champlain College through Consolidated Health Plans unless the student completes an online waiver by August 28, 2015. Students enrolled in the Allied Health Program for at least 1 course, are required to participate in the Student Health Insurance Plan unless proof of personal coverage equal to or exceeding the school Plan is provided to the College by August 28, 2015. Students enrolled for less than 12 credits (unless in Allied Health Program) are not eligible to participate. *Of an eligible student s total credits per semester, no more than 50% can be online courses. COVERAGE The plan provides for reimbursement of certain eligible medical expenses incurred because of: 1) accidental bodily injuries occurring to the student, directly and independently causing a loss to the student while covered under the plan; and 2) a covered disease or illness contracted by the student while covered under the plan. COORDINATION OF BENEFITS (COB) This provision applies to persons covered by the Policy and one or more other medical plans. This Plan is excess to any other plan of medical insurance the Covered Person may have. No benefit is payable for any Covered Expense incurred, which is paid or payable by any other valid and collectible insurance. Covered Expenses does not include any amounts not covered by the primary carrier due to penalties for failure to comply with policy provisions or requirements. This provision will not apply to the first $100 of incurred Covered Expenses. 2 3

NON-DUPLICATION OF BENEFITS LIMITATION If benefits are payable under more than one (1) benefit provision contained in the Policy, benefits will be payable only under the provision providing the greater benefit. DEFINITIONS Accident means an, unexpected and unintended event, which is the direct cause of an Injury. The Accident must occur while the Covered Person is insured under the Policy. Co-payment means a fixed dollar amount that the Covered Person must pay before benefits are payable under the Policy. Covered Expenses means expenses actually incurred by or on behalf of a Covered Person for treatment, services and supplies not excluded or limited by the Policy. Coverage under the Policy must remain continuously in force from the date the Accident or Sickness occurs until the date treatment, services or supplies are received for them to be a Covered Expense. A Covered Expense is deemed to be incurred on the date such treatment, service or supply, that gave rise to the expense or the charge, was rendered or obtained. Covered Person means any eligible person who applies for coverage, and for whom the required premium is paid to Us. Covered Sickness means Sickness, disease or trauma related disorder due to an injury which: 1. causes a loss while the Policy is in force; and 2. which results in Covered Medical Expenses. Covered Sickness includes Mental Health Disorders and Substance Use Disorders. Deductible means the dollar amount of Covered Expenses that must be incurred as an out-of-pocket expense by each Covered Person on a Policy Term basis before benefits are payable under the Policy. Doctor means Doctor licensed to practice medicine. It also means any other practitioner of the healing arts who is licensed or certified by the state in which his or her services are rendered and acting within the scope of that license or certificate. It will not include a Covered Person or a member of the Covered Person's Immediate Family or household. Elective Surgery or Elective Treatment means those health care services or supplies that do not meet the health care need for a Sickness or Injury. Elective surgery or elective treatment includes any service, treatment or supplies that: 1. are deemed by the Insurer to be research, investigative, or experimental; 2. are not generally recognized and accepted medical practices in the United States. Essential Health Benefits means benefits that are defined as such by the Secretary of Labor in the following general categories, and the items and services covered within the categories: 1. Ambulatory patient services; 2. Emergency services; 3. Hospitalization; 4. Maternity and newborn care; 5. Mental Health and substance use disorder services, including behavioral health treatment; 6. Prescription drugs; 7. Rehabilitative and habilitative services and devices; 8. Laboratory services; 9. Preventative and wellness services and chronic disease management; and 10. Pediatric services, including oral and vision care. Experimental or Investigational means any procedure, treatment, facility, supply device, or drug that: 1. Is not generally accepted by the United States medical community as effective for diagnosis, care or treatment; or 2. Is subject to research protocols indicating that the procedure, treatment, facility, supply, device, or drug is experimental or investigational; or 3. requires the patient to sign a consent form which indicates that the procedure, treatment, supply, device, or drug is "experimental or investigational" or is part of a research or study program; or 4. requires the provider's institutional review board to acknowledge that the procedure, treatment, facility, supply, device, or drug is "experimental or investigational," and subject to the board's approval. Injury means accidental bodily harm sustained by a Covered Person that results directly and independently of disease and any bodily infirmity from a Covered Accident. All injuries sustained by one person in any one Accident, including all related conditions and recurrent symptoms of these injuries, are considered a single Injury. Usual and Customary Charge means the average amount charged by most providers for treatment, service or supplies in the geographic area where the treatment, service or supply is provided. We, Our, Us means Companion Life Insurance Company, Inc., or its authorized agent. REPATRIATION AND MEDICAL EVACUATION COVERAGE This plan through Companion Life Insurance Company offers Repatriation and Medical Evacuation coverage as shown below: REPATRIATION - $25,000 per person - In the event of the death of a covered person who is a student, the Company will pay the actual U&C expenses incurred up to a maximum benefit of $7,500 in connection with the preparation and transportation of the body including but not limited to, the cost of embalming and coffin, to the person s place of residence in his/her home country, provided the student is studying outside his/her home country. This benefit does not include the transportation expenses of anyone accompanying the body, nor does it extend to repatriation of spouses, same-sex spousal equivalents, or children. MEDICAL EVACUATION - $50,000 per person - The Company will pay U&C for the actual expenses incurred resulting from a covered injury or sickness, after hospitalization of at least 5 days, and upon the written certification of the attending physician, for air evacuation of an insured student, including physician or nurse accompaniment up to a maximum benefit of $10,000. Evacuation may be to his/her natural country or to a hospital operated pursuant to the law for the care and treatment of injured or ill persons. Any expenses in respect to medical evacuation require prior approval of Champlain College Student Health Center. This benefit does not extend to medical evacuation of spouses, same-sex spousal equivalents or children. Repatriation and Medical Evacuation coverage s are subject to the same Exclusions and Limitations set forth in the Student Health Insurance Policy issued by Companion Life Insurance Company, for the period and providing the other coverage as shown in this brochure. 4 5 6

STATE MANDATED BENEFITS This plan will also pay any applicable benefits mandated by Vermont State Insurance Law, the same as for any other covered sickness unless stated otherwise. All mandated benefits are subject to the terms and conditions generally applicable to other benefits provided under the policy. If any Preventive Services Benefit is subject to the mandated benefits required by state law, they will be administered under the federal or state guideline, whichever is more favorable to the student. CANCER SCREENING TESTS The Company will pay the charges incurred for the following cancer screening tests. 1. Screening mammogram performed according to the following schedule: a. A baseline mammogram for women age 35 through 39 inclusive; b. A mammogram every year for women age 40 and over. 2. PAP tests for women 18 years of age and older. 3. Prostate Cancer Screening that is consistent with the recommendations by the Centers for Disease Control and Prevention or if recommended by the Insured Person s health care provider. CLINICAL TRIAL COSTS FOR CANCER TREATMENT STUDIES We will pay the expenses incurred by an Insured Person for the costs of routine patient care services for patients who participate in all four types of approved cancer clinical trials (Phase I, II, III and IV) that are conducted under the auspices of cancer care providers. We will pay such benefits on the same basis as for other covered medical and surgical procedures. LOW PROTEIN MODIFIED FOOD PRODUCT BENEFIT Coverage will be provided for the Medically Necessary treatment of an inherited metabolic disease on the same basis as any other Covered Sickness. Such treatment will include the cost of low protein food products that are part of a diet prescribed by a licensed Physician and managed by a health care professional in consultation with a Physician who specializes in the treatment of metabolic disease. Such diet must be deemed Medically Necessary to avert the development of serious physical and mental disabilities or to promote normal development or function as a consequence of the inherited metabolic disease. We will provide coverage for this benefit only to the extent that the cost of Medically Necessary formulas and special food products exceeds the cost of a normal diet. DIABETES COVERAGE - We will pay Eligible Expenses for the equipment, supplies and outpatient diabetes self-management training and education. This includes medical nutrition therapy for the treatment of insulin dependent diabetes, insulin using diabetes, gestational diabetes and noninsulin using diabetes if prescribed by a Doctor. TREATMENT TO BONES OR JOINTS IN THE FACE, NECK OR HEAD The Company will pay the expenses incurred for the diagnosis and Medically Necessary treatment, including surgical and nonsurgical procedures, for a musculoskeletal disorder that affects any bone or joint in the face, neck or head. The musculoskeletal disorder must be the result of an accident, trauma, congenital defect, developmental defect or pathology. The benefits provided under this provision will be the same as that provided for any other musculoskeletal disorder in the body. Benefits will be provided for treatment prescribed or administered by either a Physician or a dentist. This provision does not provide benefits for dental services for the diagnosis or treatment of dental disorders or dental pathology primarily affecting the gums, teeth or alveolar ridge. CONTRACEPTIVE COVERAGE The Company will pay the expenses incurred for outpatient contraceptive services as follows: 1. Sterilizations, payable as an outpatient surgery benefit; and 2. If prescription drugs coverage is a benefit of this policy, the purchase of all prescription contraceptives and prescription contraceptive devices approved by the Federal Food and Drug Administration. 3. Any coinsurance factors or deductibles that apply to either outpatient surgery or prescription drugs will also apply to this benefit. HOME HEALTH CARE BENEFIT - If a Covered Student incurs medically necessary Doctor prescribed Home Health Care Expense, we pay 80% of such Eligible Expense incurred within 12 months from the date of the first Home Health Care visits, subject to: annual deductible of $50 maximum of forty (40) visits (four (4) hours of Home Health Aide Service will count as one (1) Home Health Care visit) coverage for maternity and childbirth is subject to a 9 month waiting period coverage will be provided if the covered person is not eligible for Medicare 7 8 9 Home Health Care Eligible Expenses include: part-time nursing care, by or supervised by a registered graduate nurse; part-time home health aide services which consists mainly of caring for the patient; physical, occupational, respiratory or speech therapy nutrition counseling; medical social services by a qualified social worker licensed by the jurisdiction where services are rendered; and medical supplies, prosthetic and orthopedic appliances, rental or purchase of durable medical equipment, drugs and medicines obtained by prescription only including insulin, but only to the extent that such charges would have been considered covered expenses had the Covered Student required confinement in a Hospital or in a skilled nursing facility. OFF-LABLE USE BENEFIT The Company will pay the expenses incurred for a prescription drug that is prescribed for a treatment other than those treatments stated in the labeling approved by the federal Food and Drug Administration, including the Medically Necessary services associated with the administration of the drug. This benefit is payable if the drug is approved by the federal Food and Drug Administration and includes another use of the drug if that use is prescribed by the Insured s treating oncologist and supported by medical or scientific evidence. COVERAGE FOR PRESCRIPTION DRUGS PURCHASED IN CANADA The Company will pay the expenses incurred for prescription drugs purchased in Canada, and used in Canada or reimported legally, or purchased through I-SaveRX program on the same benefit terms and conditions as prescription drugs purchased in the United States. For drugs purchased through the Internet, The Company will require evidence of the provider s accreditation by the Internet Mail-order Pharmacy Accreditation Commission (IMPAC) or similar organization. PROSTHETIC DEVICE The Company will provide coverage for the prosthetic device that is the most appropriate model that is medically necessary to meet the Insured Person s medical needs. The Company will also provide coverage for the medically necessary repair or replacement of a prosthetic device. AUTISM SPECTRUM DISORDER BENEFIT - We will pay benefits for the eligible expenses incurred for the diagnosis and treatment of Autism Spectrum Disorders to the same extent that coverage is provided for any other sickness. Coverage includes Applied Behavior Analysis supervised by a nationally board-certified behavior analyst,

for children between the ages of 18 months and six (6) years of age or entry into first grade, whichever comes first. Treatment of autism spectrum disorders means the following care prescribed, provided, or ordered for an individual diagnosed with one or more Autism Spectrum Disorders by a physician licensed pursuant to Chapter 23 of Title 26 or a psychologist licensed pursuant to Chapter 55 of Title 26 if such physician or psychologist determines the care to be medically necessary: Habilitative or Rehabilitative Care, Pharmacy Care, Psychiatric Care, Psychological Care or Therapeutic Care. TOBACCO CESSATION BENEFIT - We will provide coverage for at least one 3-month supply per year of FDA-approved Tobacco Cessation Medication, including Over the Counter (OTC) medication, when prescribed by a licensed health care provider. Tobacco Cessation Medication means all therapies approved by the federal Food and Drug Administration (FDA) for use in tobacco cessation. GENERAL ANESTHESIA FOR CERTAIN DENTAL PROCEDURES BENEFIT - We will cover charges incurred for general anesthesia administered by a licensed anesthesiologist or certified registered nurse anesthetist for dental procedures performed in a hospital or ambulatory surgical center setting on the following insureds: 1. An Insured Person s covered Dependent 7 years or younger who is unable to receive needed dental treatment in an outpatient setting due to the Dependent s age and the condition or problem is significantly complex to require a Hospital or Ambulatory Surgical Center to perform the dental procedure safely and effectively; 2. An Insured Person s covered Dependent 12 years or younger who has documented phobias or mental illness who has urgent and complex dental needs and a superior result is expected from dental care provided under general anesthesia that cannot be expected from dental care provided under local anesthesia that delaying or deferring can be expected to result in infection, loss of teeth, or other oral or dental morbidity; 3. An Insured Person who has exceptional medical circumstances or a developmental disability which places the Insured Person as serious risk. ANTICANCER MEDICATION BENEFIT - We will provide coverage for orally administered anticancer medications used to kill or slow the growth of cancerous cells on the same basis as intravenously administered or injected anticancer medication. EXCLUSIONS AND LIMITATIONS Any exclusion in conflict with the Patient Protection and Affordable Care Act will be administered to comply with the requirements of the Act. The plan does not cover nor provide benefits for: 1. Expenses incurred for dental care or treatment of the teeth, gums or structures directly supporting the teeth, including surgical extractions of teeth (except as specifically provided by the Policy). This exclusion does not apply to the repair of Injuries to sound natural teeth caused by a covered Injury, and except as specifically provided in the Hospitalization and Anesthesia for Dental Procedures expense benefit; 2. Expenses in connection with services and prescriptions for eye examinations, eye refractions, eye glasses or contact lenses (except as specifically provided by the Policy), or the fitting of eyeglasses or contact lenses, radial keratotomy or laser surgery for vision correction or the treatment of visual defect or problems; 3. War, or any act of war, whether declared or undeclared; service in the Armed Forces of any country. Loss which occurs during or as a result of committing or attempting to commit an assault, felony, or participation in a riot or insurrection, engaging in an illegal occupation; 4. Expenses incurred for Injury or Sickness for which benefits paid or payable under any Worker s Compensation or Occupational Disease Law or Act, or similar legislation; 5. Treatment, services or supplies, in a Veteran s Administration or Hospital owned or operated by a national government or its agencies unless there is a legal obligation for the Covered Person to pay for treatment; 6. Elective Surgery or Elective Treatment as defined by the Policy; 7. Expenses in connection with cosmetic treatment or cosmetic surgery, except: a. a covered Injury that occurred while the Covered Person was insured; b. a covered child congenital defect or anomaly; or c. as specifically provided for in the Policy. 8. Reproductive/Infertility procedures and fertility tests, including but not limited to: family planning, fertility tests, infertility (male or female), including any supplies rendered for the purpose or with the intention of achieving conception; premarital examinations. Examples of fertilization procedures are: ovulation induction; in vitro fertilization; embryo transplant; or similar procedures that augment or enhance the Covered Person s reproductive ability; impotence organic or otherwise; 9. Expenses incurred in connection with voluntary sterilization or sterilization reversal, vasectomy or vasectomy reversal; 10. Injuries arising out of playing or participating in any Club or Intramural sport contest or competition, traveling to or from such sport, contest or competition as a participant; or participation in any practice or conditioning program for such sport, contest or competition; 11. Routine physical examinations and routine testing; preventive testing or treatment; screening exams or testing in the absence of any Injury or Sickness, except as specifically provided by the Policy; and 12. Charges that are not Medically Necessary or in excess of the Usual and Customary charge. RIGHT OF REIMBURSEMENT If a Covered Person incurs expenses for Sickness or Injury that occurred due to the negligence of a third party: (a) We have the right to reimbursement for all benefits We have paid from any and all damages collected from the third party for those same expenses whether by action at law, settlement or compromise by the Covered person, Covered Person s parents, it the Covered Person is a minor, or Covered Person s legal representative as a result of that Sickness or Injury, and (b) We are assigned the right to recover from the third party, or his or her insurer, to the extent of the benefits paid for that Sickness or Injury. We shall have the right to reimbursement out of all funds that the Covered Person, the Covered person s parents, if the Covered Person is a minor, or the Covered Person s legal representative, is or was able to obtain for the same expense we have paid as a result of that Sickness or Injury. The Covered Person or the Covered Person s parents if the Covered Person is a minor is required to furnish any information or assistance or provide any documents that we may reasonably require in order to obtain our rights under this provision. This provision applies whether the third party admits liability or not. CLAIM PROCEDURE In the event of Accident or Sickness: 1. If at school, contact Student Health Services. 2. All claims should be mailed to the Claims Administrator at the address listed below and should include the student s name, address, insurance identification number and name of the 10 11 12

College under which the student is insured. A Company claim form is not required for filing a claim. 3. Itemized billings (Written Proof of Loss) should be submitted by your health care provider or the Covered Person within 90 days of treatment, or as soon as reasonably possible. The Insured Person must request an appeal in writing within 180 days of the date appearing on the EOB. The appeal request must include any additional information to support the request for appeal, e.g. medical records, physician records, etc. Please submit all requests to the Claims Administrator at the address on page 15. YOUR RIGHT TO AN EXTERNAL REVIEW If you have appealed a claim decision and exhausted all applicable internal review procedures you have a legal right to an independent external review. This review is available when requested in writing provided the decision to be reviewed requires the plan to expend at least $100 for the service and the decision by the plan is based on one of the following reasons: 1. The health care service is a covered benefit that we have determined not to be medically necessary; 2. A limitation was placed on the selection of a health care provider that is inconsistent with limits imposed by this plan and any applicable laws and rules; 3. The health care treatment has been determined to be experimental, investigational or an off-label drug; or 4. The health care service involves a medically-based decision that a condition is preexisting. Your request must be filed on a form provided to you by the Division and shall include either the $25 fee or a request for a waiver or reduction of the fee, a general release executed by you, the Insured, for all medical records pertinent to the appeal, identification of the health insurer by whom you are covered and which made the decision at issue, and a statement that all internal appeals have been exhausted.
