SUNY Adirondack Sports Medicine Team Mission The SUNY Adirondack Athletics Department Head Certified Athletic Trainer (Dan Dennett MS, ATC) strives to provide the most efficient and effective interdisciplinary care available to help prevent and manage athletic related injuries and/or illnesses. Treatment of these conditions will be based on sound medical principles considering personal and team goals, and always delivered in a professional and ethical environment. It is the student athlete s responsibility to become medically eligible. Until an athlete has been cleared by the SUNY Adirondack Athletics Dept. requirements they are not considered eligible and are not to participate in any SUNY Adirondack Athletics Department team related physical activity. Certified Athletic Trainer (ATC) A Certified Athletic Trainer (ATC) is a highly educated and skilled professional specializing in athletic injuries. The Certified Athletic Trainer (ATC) works under the direction of the Team Physicians in the chain of command. The athletic trainer works collaboratively with the coaches, Physicians and other allied healthcare providers and functions as an integral member of the Sports Medicine Team. Dan Dennett is certified by the National Athletic Trainers' Association Board of Certification (NATABOC) and a member of the National Athletic Trainers' Association and the New York State Athletic Trainers' Association. SUNY Adirondack Athletic Contacts The SUNY Adirondack Athletic Training Room is Located in the lower hall Gymnasium. The Certified Athletic Trainer's office is located in the upper lobby of the Gym. Athletic Trainer's Office/Athletic Training Room: 518 743 2200 x2670 SUNY Adirondack Athletic Department: 518 743 2200 x2395 Athletic Director s Office: 518 743 2269 Athletic Training Room hours vary by season and sport (posted monthly) The athletic training room offers services to all student athletes on the varsity level. Athletic injuries and/or iiiness resulting from class, intramural or club sport participation are not always assessed or treated in the athletic training room. Non student athletes may receive immediate first aid treatment and recommendations as needed. Files for every varsity athlete are stored in the Athletics Department. These files contain all information on current athletes and past medical information. Introduction This information is provided to make the policies and procedures of the SUNY Adirondack Athletic Training Department more clear and concise. The following information will be updated on a regular basis. Statement of Purpose The purpose of the athletic training room at SUNY Adirondack is to provide the best medical care for the student athletes involved in intercollegiate competition. This medical care includes recognition, treatment, management, rehab and preventative education related to athletic injuries and illnesses. If referral, guidance or other forms of counseling are deemed necessary, these too shall be included in the overall medical care received. Statement of Function The athletic trainer (ATC) along with the coaches are directly responsible to the Athletic Director as it relates to emergency care. The team physicians will ultimately have the final decision for an athlete's return to play. The athletic trainer (ATC works under standing orders from the team physicians). The New York State Athletic Trainers' Association has laws that regulate the practice of athletic training. Within these laws are definitions that specify the scope of practice for a Certified Athletic Trainer. Copies of these orders are on file in the Athletic Trainer's Office. SUNY Adirondack s ATC practices athletic training as define of by the New York State Education Law Article 162 Athletic Trainers Act.

General Duties and Responsibilities The prevention, evaluation, care, rehabilitation and conditioning of all athletic injuries/illness to all SUNY Adirondack student athletes. This also includes the return of injured/ill student athletes to competition in optimum physical condition. Making sure the parents/guardians and/or emergency contact of an injured/ill student athlete are informed when they are sent for medical treatment. NOTE: If the student athlete is a minor then their parents/guardians will be automatically contacted. If the student athlete is over eighteen (18) years of age, the athletic trainer (ATC) must obtain permission from the student athlete before contacting their parents/guardians and/or emergency contact. The proper maintenance, organization, and supervision of the athletic training room. ATC will attend all home games when possible and/or will be on call on campus. When multiple events are schedule per diem ATCs may be used to provide athletic training services when available. Performing emergency care as needed to any student athlete at home events and to coordinate emergency services with local emergency squads. The selection, ordering and maintenance of all athletic training supplies and equipment. Provide advice on equipment purchases and equipment fittings. To communicate with coaches and physicians on the playing status of all injured/ill student athletes. Refer student athletes to a physician s care when the evaluation or treatment of the injury/illness is beyond the scope of Athletic Training. Maintain injury/illness and treatment records on file in the Athletic Trainer s Office. Services Rendered The certified athletic Trainer (ATC) at SUNY Adirondack provides service for the following persons: Any student athlete enrolled at SUNY Adirondack that has passed a physical on file and is on a team roster. Only eligible student athletes are permitted to participate in game events. Any visiting athlete who is injured while engaging in competition at SUNY Adirondack. Any coach, official, or other personnel directly involved with an athletic event at SUNY Adirondack. Any fan or other outside public will receive general first aid from the athletic trainer if needed otherwise EMS (911) should be used. Event Coverage SUNY Adirondack Certified Athletic Trainer (ATC) will attend all home games when possible and/or will be on call on campus. When multiple events are scheduled per diem ATCs may be used to provide athletic training services when available. The away game policy is as follows: The host school of the other sports traveling will be called in advance to let them know specific of instructions on needs of the team. A medical kit containing supplies for the visiting team needs will be provided by that team's athletic training staff.

Athletic Training Room Hours Athletic training services will be available at least one (1) hour prior to each home game report time. The athletic trainer (ATC) will be available until all home events are over. The athletic trainer (ATC) must be informed of practice/game schedules changes as soon as possible so athletic training services can be adjusted and provided. There will be times the athletic trainer (ATC) may not be able to provide service. In the event athletic training services are not provided all SUNY Adirondack Coaches will refer to the ATHLETIC MEDICAL EMERGENCY ACTION PLAN (EAP) regarding INJURY PROCEDURES. If SUNY Adirondack is closed due to weather related conditions, a decision made by the (President and/or Athletic Director), the athletic trainer (ATC) will not provide athletic training services until the College officially re opens. Injury Care Policies It will be the responsibility of the athletic trainer (ATC) to evaluate, treat and/or refer any injured/ill student athlete that is under his/her care. The evaluation will be made at the event, at the time of the injury, or may be done in the athletic training room. Once an evaluation is made, the athletic trainer (ATC) will make the decision as to the nature and extent of the injury/illness of the student athlete. The athletic trainer (ATC) will then make the decision as to the appropriate treatment rehabilitation, conditioning and/or physician referral. The coach, athletic director, and parent or guardian will be notified as needed. The athletic trainer (ATC) will inform the student athlete's coach on any updated status. Away Contest Injury/Illness If an injury/illness occurs while a team is at an away contest and the athletic trainer (ATC) is not traveling with them, the host school's certified athletic trainer should evaluate the injury/illness and offer his/her recommendation. The coach will make note of the injury/illness and contact the athletic trainer (ATC) and/or leave a voice message on cell phone and/or office phone indicating the name, contact information and injury of the athlete. When successful contact is made with athletic trainer (ATC), at that time instructions will be provided to athlete and coach regarding injuries/iiinesses and their procedures. When contact is unsuccessful with athletic trainer the coach will instruct the athlete to see the athletic trainer (ATC) the next available day for further evaluation. The athlete needs to be instructed if their symptoms should progress, to activate EMS and/or contact another responsible individual for transportation to seek medical attention for further evaluation. If the student athlete chooses to seek medical attention that information needs to be provide as soon as possible to SUNY Adirondack Athletic Trainer (ATC). The coach should only give ICE to the athlete for pain and swelling. NEVER give the athlete any type of medication, creams or ointments.

Injury/Illness Report/Status An injury/illness report will be completed by the supervising coach or communicated either by phone, email, voice mail, texting, fax, medical note or face to face conversation with the athletic trainer. If there is no report then it should be assumed that there is no significant injury... The injury reports are separated into four different playing status breakdowns. "OUT" This means the student athlete is not to practice at all. A physician and/or the athletic trainer (ATC) must clear the student athlete before returning to games and/or practice. The student athlete will have to go through some functional testing as well before returning to play. "QUESTIONABLE" This means the student athlete will have to be tested before he/she can resume practice. In some cases a student athlete will be under this listing because the athletic trainer (ATC) has not had any communication with him/her on that particular day. "GO AS TOLERATED" This is to let the coach know that a student athlete does still have an injury/illness and is or should be receiving treatment on a regular basis. Specific limitations are also addressed under the student athlete's name. Functional testing will be done prior to practice game and the results will be communicated to the coaches accordingly. Also, communication either by phone, email, voice mail, texting, fax, medical note or face to face conversation from a MEDICAL PHYSICIAN (i.e.: MD, DO, DDS) to SUNY Adirondack Athletic Trainer (ATC) and/or Athletic Director has permitted the athlete to return to participation with restrictions. "CLEARED" This means communication either by phone, email, voice mail, texting, fax, medical note or face to face conversation from a MEDICAL PHYSICIAN (i.e.: MD, DO, DDS) to SUNY Adirondack Athletic Trainer (ATC) and/or Athletic Director has permitted the athlete to return to full participation. Physician Referral An injured student athlete will be referred for further medical evaluation and diagnosis following these conditions: The athletic trainer (ATC) should evaluate the student athlete before the referral is made. If the athletic trainer (ATC) is not available in an emergency situation, the athlete should be sent to the nearest ER.. The injury/illness suffered by the student athlete requires care that exceeds the athletic trainers' (ATC) capabilities; the injury/illness is beyond the scope of athletic training or the facilities that are needed to treat the injury/iiiness are not avaiiable. The student athlete or the parents/legal guardian of the athlete request that the student athlete receive further medical evaluation. The student athlete maintains the right to seek a second opinion from another physician, but should communicate this to the athletic trainer (ATC). In the event the student athlete does not respond to treatment, normally 5 7 days, he/she may be referred to a physician. Proper follow up between the physician and the athletic trainer (ATC) will be carried out to ensure that the proper rehabilitation will be accomplished and that adequate information regarding the student athlete' s return to competition can be obtained.

Concussion Management Guidelines The Athletic Trainer (ATC) shall be empowered to determine management and return to play of any ill or injured student athlete, as he or she deems appropriate. Conflicts or concerns will be referred to our sports medicine physicians for remediation. The Athletic Trainer (ATC) has on file and annually updates an (EAP) Emergency Action Plan to respond to student athlete catastrophic injuries and illnesses, including but not limited to concussions, heat illness, spine injury, cardiac arrest, respiratory distress (e.g. asthma), and sickle cell trait collapses. The athletic trainer (ATC) shall review and practice the plan annually. SUNY Adirondack Athletic Trainer (ATC) will educate all student athletes about the signs and symptoms of concussions prior to the start of their sports season. SUNY Adirondack Athletic Trainer (ATC) will require student athletes to sign a statement in which student athletes accept the responsibility for reporting their injuries and illnesses to the athletic trainer including signs and symptoms of concussions. SUNY Adirondack Athletic Trainer (ATC) shall have on file a written concussion management plan that specifically outlines the roles of the Athletic Trainer (ATC) and the student athlete. In addition, the following components have been specifically identified for the collegiate environment: a. SUNY Adirondack Athletic Trainer (ATC) and other athletics healthcare providers will practice within the standards as established for their professional practice. b. When a student athlete shows any signs, symptoms or behaviors consistent with a concussion, the athlete will be removed from practice or competition, by either a member of the coaching staff or by the athletic trainer (ATC). If removed by a coaching staff member, the coach will refer the student athlete for evaluation by an athletic trainer (ATC). During competitions, on field injuries will be under the discretion of the official and playing rules of the sport. SUNY Adirondack Athletic Trainer (ATC) will follow such rules and attend to medical situations as they arise. Visiting sport team members evaluated by SUNY Adirondack Athletic Trainer (ATC) will be managed in the same manner as SUNY Adirondack student athletes. c. A student athlete diagnosed with a concussion will be withheld from the competition or practice and not return to activity for at least the remainder of that day. Student athletes that sustain a concussion outside of their sport will be managed in the same manner as those sustained during sport activity. d. The student athlete will receive monitoring for deterioration. Athletes may be provided with written home instructions upon discharge; preferably with a parent, guardian, roommate, or someone that can follow the instructions. e. The student athlete will be monitored for recurrence of symptoms both from physical exertion and also mental exertion, such as reading, phone texting, computer games, watching television and/or film, athletic meetings, working on a computer, classroom work, or taking a test. Academic advisors and professors will be notified of student athlete's concussion if permitted by the student athlete. f. The student athlete will be evaluated by a team physician or the physician's designee as outlined within the concussion management plan. Once asymptomatic and post exertion assessments are within normal baseline limits, return to play shall follow a medically supervised stepwise process.

*Example of Return to Play Protocol* 1. Specific Sport Stretching Exercises 2. Time limit stationary exercise bike, recumbent bike, treadmill, stair stepper, rowing machine and/or elliptical 3. Strength training: (ex. Hydro Fitness, Total Body Program and/or push ups, sit ups, squat thrusts, etc...) 4. Advanced cardiovascular and strength training: (ex. sprinting activities, plyometrics, sport specific movements, etc...) 5. Restricted, controlled return to practice (no risk of contact) Note: specific positions such as a goalie may require extra time 6. Full sports participation in a practice (with contact) Note: specific positions such as a goalie may require extra time Final authority for Return to Play resides with the team physician or the physician's designee. SUNY Adirondack Athletic Trainer (ATC) will document the incident, evaluation, continued management, and clearance of the student athlete with a concussion. Athletics staff, student athletes and officials will continue to emphasize that purposeful or flagrant head or neck contact in any sport should not be permitted. Athletic Training Room Rules No tobacco (including smokeless tobacco), food or energy drinks permitted on campus. If a student athlete requires some form of treatment they will sign in and be treated on a first come first serve basis. Please arrive early for practice/games. Treatments or tapings should not interfere with practice/games. No obscenities or offensive language. No friends of athletes are permitted in the athletic training room when treatments are provided without permission of ATC. No equipment (bags, cleats, etc...) in the athletic training room. Wear proper attire, shorts and a T shirt at minimum. Student athlete's injured area will need to be clean if getting an evaluation or rehab after practice. Do not take anything out of the athletic training room without permission from ATC. It is the student athlete's responsibility to report all injuries and/or illnesses to the athletic trainer. If a student athlete needs something from the athletic training room and the athletic trainer is not there, the student athletes' coach may let them in but the student athlete needs to be supervised. Under no circumstances should electrical modalities (ex: whirlpools, electric stimulation, and/or ultrasound) be used by anyone other than a certified athletic trainer. Be respectful at all times Only injured athletes are permitted in athletic training room unless permission by athletic trainer.

Student Athlete Medical Information Folder All student athletes participating in an intercollegiate sport at SUNY Adirondack will have a medical information folder on file at the Athletic Office and/or the Athletic Trainer's Office. The following information should be included in these folders. Physical form. The NJCAA requires all SUNY Adirondack student athletes to have an annual valid physical on file with the college prior to their involvement to their sports season. Student Athlete Medical History Questionnaire To be filled out completely prior to the student athlete's first scheduled practice. Proof of medical insurance (copy of medical insurance card) Student Athlete Concussion Statement and Concussion Fact Sheet Information ATC' s office To be read and sign prior to the student athlete's first scheduled practice at SUNY Adirondack. Multi Sport Student Athlete Medical Release Form (if applicable) Information on Medical Policies and Procedures Form ATCs office To be read and sign prior to the student athlete's first scheduled practice.
