A copy of your Current Joint Commission Accreditation (JCAHO), Rehabilitation Accreditation Commission Certificate (CARF), Certificate of Accreditation (COA), Alcohol and Drug Abuse Division (ADAD) Accreditation or other national accreditation certificates. 5.
