Hunterdon County Division of Senior, Disabilities & Veterans Services... 908-788- 1361 PHARMACEUTICAL ASSISTANCE: Eligibility for programs: 65 years of age or over or recipient of Social Security Disability benefits; moderate annual income. P.A.A.D. (Pharmaceutical Assistance For the Aged and Disabled) New Jersey program offering prescriptions for $6.00 - $7.00. Senior Gold- New Jersey Program offering prescriptions for $15.00 co-pay, plus one half of the remaining cost. Once a cardholder incurs an annual out-of-pocket expense of $2,000 if single or $3,000 if married, in unreimbursed costs, prescriptions for the year cost only the $15 co-pay. Must participate in a Medicare Part D plan. -5- Pharmaceutical Assistance (cont'd) Medicine Program - A non-profit group which does paperwork involved in applying for various assistance programs through pharmaceutical manufacturers. Programs provide prescription medications without charge to physician for distribution to patient. Eligibility based on need and no other insurance or assistance for medications. Physician s cooperation required. Together Rx - A prescription savings program that provides savings to eligible Medicare enrollees on more than 170 widely prescribed medicines. Multiple pharmaceutical companies participate in Together RX, each with its individual savings program. You may save

approximately 20-40% off the amount you usually pay for prescriptions and, in many cases, substantially more. Only one who is competent may grant this power of attorney, and it remains valid only as long as the person remains competent. This can be accomplished by an attorney and in some cases a banker.
