Mission The mission of Life Time Academy is to help students gain the skills and knowledge necessary to achieve academic, personal and professional success through obtaining career certification in a healthy way of life field. Admission Applicants must be accepted for admission in a Life Time Academy diploma program before enrolling in any courses. Once accepted for admission, a student may only enroll in courses in their selected diploma program. Application Applications are accepted on a rolling basis. Life Time Academy will be offering its Fitness programs at least once per quarter during the calendar year and its Yoga Teacher Training Programs at least twice per calendar year. Application is closed for the current program session if the classroom maximum capacity is met, or on the first day of class, whichever comes first. Applicants may apply at any time for the next program session. Life Time Academy reserves the right to reschedule or cancel a program session due to low application rates. Life Time Academy will notify each Fitness program applicant in writing of acceptance or rejection to their desired diploma program based on fulfillment of the following requirements: 1. Completed application form for the desired program. If any information provided on the application form is found to be false, the student will be subject to immediate dismissal. 2. Students applying for the Fitness Specialist or Professional Fitness Specialist programs must submit documentation of professional work experience, evidence of receiving college credit through a College Level Examination Program and/or a copy of their final college transcript as evidence a degree was conferred. 3. Students applying for the Fitness Specialist Certification program must submit documentation of their Certified Personal Trainer (CPT) and Corrective Exercise Specialist (CES) certifications. 4. Eligible Fitness Specialist applicants will need to complete a pre-test with an accuracy of 70% or higher. 5. Payment of a $25 application fee is due upon submission of application. Payment of $400 enrollment deposit is due upon submission of Enrollment Agreement for the Fitness programs. Life Time Academy will notify each LifePower Yoga Teacher Training program applicant in writing of acceptance or rejection to their certificate program based on fulfillment of the following requirements: 1. Completed application form for the desired program, including attestation of high school graduation or equivalency. 2. Payment of a $25 application fee is due upon submission of application. 3. A minimum payment of a quarter of the student s total tuition fee is due upon submission of Enrollment Agreement. In the event of rejection, any monies paid will be refunded in full. The date of acceptance by Life Time Academy shall be presumed to be the date of delivery of the notice of acceptance; and if delivered by mail, the postmarked date of the letter of acceptance. Statement of Non-Discrimination Life Time Academy admits students without regard to their race, color, sex, age, national or ethnic origin, religion, sexual orientation, ancestry, disability, veteran status, marital status, parental status, or any other protected status. Life Time Academy does not discriminate against individuals on the basis of race, color, sex, age, national or ethnic origin, religion, sexual orientation, ancestry, disability, veteran status, marital status, parental status, or any other protected status, in the administration of its educational policies, admissions policies, or other programs and activities. Immunization Requirements Minnesota law (M.S. 135A.14) requires that all students born after 1956 and enrolled in a postsecondary educational institution be immunized against measles, rubella, mumps and diphtheria, and tetanus after 12 months of age and within 10 years of first registration, allowing for certain specified exemptions. Students must submit a statement indicating the month and year of each immunization at the first registration for classes or no later than 45 days after the start of their first term. Students born in 1956 or before are not required to provide this information. Students who graduated from a Minnesota high school in 1997 or later are also exempt. Scholarships Life Time Academy is not an institution that is eligible for government aid or assistance. Life Time Academy offers the following institutional scholarship programs. Scholarships are non-cash. LifePower Member Scholarship All current Life Time Fitness members enrolling in the LifePower Yoga Teacher Training program that are active and in good standing are eligible for a $100 tuition discount if accepted into the LifePower Yoga Teacher Training program. In order to qualify, all admission requirements must be completed and applicants must provide proof of membership by submitting their membership identification number on their application form. This scholarship is not available to repeat students. LifePower Early Bird Scholarship A new student is eligible for a $300 tuition discount if the student applies for the LifePower Yoga Teacher Training program is accepted and paid in full before the Life Time Academy (MN January 2012 v4) Page 5 of 28

first day of class. This offer may be combined with the LifePower Member Scholarship. This discount does not apply to students who select the LifePower Yoga Teacher Training payment plan option. This scholarship is not available to repeat students. Fitness Program Member Scholarship All current Life Time Fitness members enrolling in a Fitness program, who are active and in good standing, are eligible for a $500 tuition discount if accepted into a Fitness program. In order to qualify, all admission requirements must be completed and applicants must provide proof of membership by submitting their membership identification number on their application form. Fitness Program Early Bird Scholarship A student is eligible for a tuition discount up to $400 (as outlined below), which varies based on the Fitness program selected and if accepted in to the Fitness program. This offer may be combined with the Fitness Program Member Scholarship. This discount applies if the student pays the Fitness program tuition in full prior to the beginning of the program session or if the student pays the first installment payment in full prior to the beginning of the program session and also makes on-time payments (as outlined in the Enrollment Agreement) for the entire duration of the program. Should an installment payment become delinquent, the scholarship will be revoked and the tuition and remaining payments adjusted to reflect as such. Program Tuition Discount Fitness Specialist Externship Certification $100 Fitness Specialist $100 Professional Fitness Specialist $300 Professional Fitness Trainer $400 Tuition For all students in the Fitness programs, the application fee of $25 is due with the delivery of the application and an enrollment deposit of $400, paid on-line to the Life Time Academy store, is due upon delivery of the Enrollment Agreement. The fee and deposit are only refundable if the student is rejected for admission or subject to the Cancellation and Refund Policy section. The balance of the program tuition remaining after paying the $400 deposit is to be paid in installments as stated in the student s Enrollment Agreement, two weeks prior to the student s conclusion of any Fitness program. For all students applying to the LifePower Yoga Teacher Training program, the application fee of $25 is due with the delivery of the application and minimally, one quarter of the student s total tuition payment is due with delivery of the Enrollment Agreement. If the student selects the LifePower Yoga Teacher Training payment plan the first quarter of the total tuition payment is due with the delivery of the Enrollment Agreement, the remaining 3 quarters of the student s tuition will be paid depending on the format: Life Time Academy (MN January 2012 v4) Page 6 of 28

Diploma Programs Professional Fitness Trainer Program Mission The mission of the Professional Fitness Trainer program is to help students gain the necessary classroom and real-world practical experience to succeed as one of the industry s top personal trainers. Program Overview Life Time Academy has created the new Professional Fitness Trainer program. This program provides a unique educational experience offering classroom and practical experience that prepares graduates to succeed as a Personal Training professional. The Professional Fitness Trainer diploma focuses not only on providing an integrated approach to health and fitness using sound knowledge of appropriate exercises, but also prepares graduates in the areas of leadership, business and sales. The Professional Fitness Trainer diploma can only be obtained through Life Time Academy as it is the result of a partnership between Life Time Academy and the National Academy of Sports Medicine (NASM). The Professional Fitness Trainer diploma equips students with the knowledge to function as a professional member of the fitness instruction industry and work at locations such as fitness studios, health clubs and private businesses. Program Description The Professional Fitness Trainer program offers a 40 week curriculum and externship, with the exception of any declared student holidays, totaling 690 contact hours. Fitness Specialist Program Mission The mission of the Fitness Specialist program is to help students with previous education obtained through an Associates or Baccalaureate Degree Program, training, work experience or College Level Examination Program, gain the necessary classroom and real-world practical experience to succeed as one of the industry s top personal trainers. Program Overview Life Time Academy has created the new Fitness Specialist program. This program provides a unique accelerated educational experience for students with previous education in the exercise sciences or work experience in the health Life Time Academy (MN January 2012 v4) Page 8 of 28

and fitness industry. The focus of this program is on applying the previously learned sciences to functional anatomy, assessments and program design in a practical hands-on training environment to prepare graduates to succeed as a Personal Training Professional. The Fitness Specialist diploma focuses not only on providing an integrated approach to health and fitness using sound knowledge of appropriate exercises, but also prepares graduates in the areas of leadership, business and sales. Acceptance into this program is on a case by case basis. A determination is made after reviewing the student s application, previous education (it is recommended for students who have completed college courses equivalent to Kinesiology, Anatomy & Physiology, Psychology & Coaching, Exercise Physiology, and Nutrition), work experience and/or health and fitness industry certification. All Fitness Specialist applicants will also be asked to complete the Professional Fitness Trainer Pre-test. Eligible Fitness Specialist applicants will need to complete the pre-test with an accuracy of 70% or higher. The Fitness Specialist diploma can only be obtained through Life Time Academy as it is the result of a partnership between Life Time Academy and the National Academy of Sports Medicine (NASM). Certificate Program LifePower Yoga Teacher Training Program Mission The mission of LifePower Yoga Teacher Training is to provide an exceptional, allinclusive yoga community and experience where we enlighten and inspire our students while also educating and creating the industry s best teachers. Program Overview LifePower Yoga Teacher Training is a Yoga Alliance Registered Yoga School that offers a 200 hour level program providing a quality educational experience that prepares graduates to succeed in the yoga instruction industry. LifePower Yoga Teacher Training equips students with the knowledge to function as a professional member of the yoga instruction industry and work at locations such as yoga studios, health clubs and private businesses. A graduate of the LifePower Yoga Teacher Training program will also have the credentials to register as a Registered Yoga Teacher (200 level) with the Yoga Alliance if desired. Program Description Topics covered in the LifePower Yoga Teacher Training 200 hour level program meet or exceed all the required standards set forth by Yoga Alliance. The LifePower Yoga Teacher Training program has 3 possible schedules depending on location either, a 10 week session (a total of 20 hours a week) or a 12 week Life Time Academy (MN January 2012 v4) Page 12 of 28

session (a total of 16.5 hour a week) and a 20 week sessions (a total of 10 hour a week.) Each schedule has the exception of any declared student holidays. Times do not include breaks. Students are also expected to attend at least three yoga classes weekly for practice, and observe a minimum of six yoga classes. General Course Requirements In addition to attendance and participation in classes, teacher trainees are expected to complete all homework assignments and to honor the following practices: Daily yoga practice Daily meditation (at least 20 minutes) Daily yoga meal (follow the guidelines) Daily gratitude journal A total of six yoga class observations Daily anonymous acts of kindness are to be performed. Formal observation of teachers is one of the most effective ways to learn to be a great teacher. The goal of any observation is not judgment but to earn from the teacher s experience. Observe a variety of classes and teachers, even ones that are outside of your experience and comfort zone. Observations can be of any class taught at the LifePower Yoga studios inside Life Time Fitness Centers. LifePower Yoga Teacher Training Program Tuition 200 Hour: The program cost includes 200 contact hours, application fee, LifePower Yoga Student Manual and, a Short Term Diamond level membership to Life Time Fitness for the duration of the program. Repeat Students The LifePower Yoga Teacher Training program may have limited spots available, at select locations, for students who have previously completed the 200-hour LifePower Yoga Teacher Training program to re-take the program. The tuition fee for a repeat student is $350.* If a repeat student refers a new student who enrolls into the LifePower Yoga Teacher Training program the repeat student will receive a $50 tuition discount. Those students who desire to re-take the program must contact Life Time Academy. *This offer is subject to classroom space availability. Current students or students having graduated within the past 60 days may request one free copy of their official transcript; any copies thereafter will be assessed the appropriate charges. A fee of $5.00 per transcript will be charged to all students. Requests to have a transcript faxed will be assessed an additional fee of $10.00. Transcript requests will not be processed for students with unmet financial obligations. Leave of Absence Leaves of absences, including military leaves, shall be reasonable in duration, not to exceed sixty (60) calendar days and shall be for specific and acceptable purposes. A Life Time Academy General Petition Form or written request for leave of absence properly dated and signed by the student and school official must be filed prior to the beginning of such a leave unless circumstances prevent completion of the request at that time. Student must return on or before the expiration of the leave of absence or face termination. Specialist (CES) certificates or College Level Examination Program ( CLEP ), but does not issue any credits for the previous education, training, work experience or CLEP. For all other students, Life Time Academy does not accept or issue credit for previous education, training, work experience or CLEP. Student Services A goal of Life Time Academy is to have each graduate obtain employment in a career field related to their field of study. Life Time Academy does not, however, promise or guarantee its students a job upon enrollment or graduation. A conscientious effort is made to match each student's capabilities and desires with the most appropriate jobs known to Life Time Academy. Educational Delivery Methods Life Time Academy will deliver instruction using lectures, labs, directed study, practicum and observation. Web-enhanced or online portions are a required part of all Fitness Trainer program classes. Facilities and Equipment Students will have access to Life Time Fitness - Crosstown, Life Time Fitness- Highland Park, Life Time Fitness- St. Louis Park and LifePower Yoga at Uptown for the LifePower Yoga Teacher Training Program, those facilities in which Life Time Academy is located, during the program. Whether the student is a Life Time Fitness member or non-member, a Short Term Diamond level Life Time Fitness membership will be issued only to the individual student from the session start date through the end of the student s enrollment in the program, regardless of how enrollment in the program may end. The membership fee is included in the cost of tuition. A Short Term Diamond level membership to Life Time Fitness provides access for the duration of the student s program to Life Time Fitness Diamond, Onyx, Platinum, Gold and Bronze centers and access to the tennis facilities located within Life Time Fitness' Diamond, Onyx, Platinum and Gold centers (court fees apply). A Short Term Diamond level membership does not receive any partner discounts, guest privileges, other additional advantages, amenities, benefits or services that may be available with standard Life Time Fitness membership types. Life Time Academy s programs will supply necessary equipment and texts required for the Fitness programs. However, students in the LifePower Yoga Teacher Training program are required to purchase their own text books through Life Time Academy or through an alternative source and bring their own yoga mat. Credit Hour Definition Life Time Academy defines its programs in quarter credit hours. Each quarter credit hour is 10 hours of lecture, 20 hours of classroom/lab instruction, or 30 hours of externship/independent study. practice, and supervised study) is 50 minutes of classroom contact time in a 60 minute period. Student Instructor Ratio and Typical Class Size An enthusiastic staff of experienced professional educators support Life Time Academy s objective to meet the needs of each individual student. The maximum number of students in a typical classroom or laboratory setting of instruction is 30 resulting in a student instructor ratio of 30 to 1. Grading and Satisfactory Academic Progress Grades are assigned at the end of all scheduled courses. Students will receive a Grade Point Average (GPA) for each course attempted, unless the class was taken on a pass/fail basis. The GPA is calculated by the Grade Points received multiplied by the credit hours attempted to derive the quality points earned. The Cumulative Grade Point Average (CGPA) is then calculated by using the quality points earned from all courses attempted. If you earn an F grade in your course, you have failed it and do not receive credit for it. In the case where a student has not completed all of the required work to complete their course, an I grade will be assigned as the final grade. Students receiving an I grade must turn in all outstanding work within 10 days. The I grade is not used when calculating a student s satisfactory academic progress. If the required work is not submitted in the timeframes listed above, the final grade for the affected course will then be averaged based on the work that was completed and submitted as the final grade. Failure to submit outstanding work can have a negative impact on the final grade and therefore will have a negative impact on satisfactory academic progress. Students must have received all P grades in each of their quizzes, exams and courses to be making satisfactory academic progress. In the event that a student does not achieve these criteria for determining satisfactory progress, the student will be allowed to present justification or evidence of extenuating circumstances as to why he or she should be allowed to continue training. The student will be considered making satisfactory progress during any probation period. If a student on probation does not receive all P grades for all subsequent quizzes, exams and classes after being placed on probation, he/she will terminated from Life Time Academy. If the student is terminated for unsatisfactory academic progress, he/she cannot be readmitted until a minimum of one grading period has passed. A student who is obligated for the full tuition may request an I grade if the student withdraws for an appropriate reason unrelated to the student's academic status. A student receiving an I grade may reenroll in the program during the 12-month period following the date the student withdraws and complete those incomplete subjects without payment of additional tuition. If a student must repeat a section of study, he/she will be charged a repeat fee equal to the prorated portion of tuition based on contact hours that the student must repeat. This repeat of the section of study may be due to academic failure or failure to meet attendance requirements. The repeat section must be completed within one year of the start date of the program. Attendance and Tardiness Students are expected to be on time and in regular attendance for all classes. Students are expected to contact their instructor and to indicate if they will be absent or tardy. It is the responsibility of the student to contact the instructor and to get the assignments and information missed. Students enrolled in on-line courses are expected to complete a minimum of one assignment per week to maintain the minimum attendance requirement. Practicums and Externships have attendance expectations that may differ. Failure to make arrangements may result in termination. Tardiness and leaving class early is subtracted from the total time of the program, and counts against the minimum required attendance. LifePower Yoga Teacher Training students are responsible for reporting their own class attendance, please be sure you remember to sign-in for class each day. In addition to making-up time and assignments for tardiness or missed classes, LifePower Yoga Teacher Training students missing more than three (3) classes will receive an incomplete grade and will not receive certification. If the student misses more than three classes, to fulfill requirements for certification the student must complete the specific days missed during the next LifePower Yoga Teacher Training session offered. Graduation Requirements In order to graduate, a student must: 1. Complete all requirements for their program of study within the maximum time Life Time Academy (MN January 2012 v4) Page 20 of 28

frame permitted while meeting at least the minimum satisfactory academic progress criteria. 2. Return all property belonging to Life Time Academy. 3. Fulfill all financial obligations to Life Time Academy. Student Conduct Students must adhere to the rules and regulations of Life Time Academy, which may be subject to change without notice at the sole discretion of Life Time Academy. Students whose conduct reflects as a discredit upon themselves or the academy may be subject to probation and/or termination as determined in the sole discretion of Life Time Academy. The student must adhere to conduct that will not interfere with the learning process of any other students, the classroom presentation by the instructor, presentations by visitors, or the progress of the class or Life Time Academy in general. Life Time Academy reserves the right of judgment, to place on probation and/or terminate a student on any of the following grounds: 1. Excessive absences or tardiness. 2. Nonconformity of rules and regulations of Life Time Academy. 3. Conduct that is unsatisfactory to Life Time Academy, its staff, faculty, and its students, which includes, but is not limited to using notes, textbooks or other materials when not allowed during an examination, giving test questions or answers to another student, and copying an assignment from or completing an assignment for another student. 4. Falsifying school records. 5. Breach of any representation made to Life Time Academy. 6. Acts of plagiarism. 7. Failure to pay fees when due and/or to make available required documents. 8. Entering school premises while under the influence of alcohol or drugs. 9. Carrying a concealed or potentially dangerous weapon. 10. Aiding, abetting, or inciting others to commit any act that would detract from the normal operation of Life Time Academy. 11. Theft. Involuntary Termination Procedure Students to be terminated from a program are notified in writing and may appeal to Life Time Academy s On-site Administrator within three (3) days of receiving a Notice of Termination. Reinstatement Students who have been forced to interrupt their education for any reason may request reinstatement by contacting the On-site Administrator of Life Time Academy. Students who were making satisfactory academic progress when they withdrew will be eligible for re-entry without condition. Students who were not making satisfactory progress may be placed on academic probation or have other special conditions placed on their re-entry. All students requesting reinstatement may be required to go through a portion of the admissions process again. Life Time Academy (MN January 2012 v4) Page 21 of 28

Student Grievance Procedure 1. Complaints directed at an individual instructor or staff member must be discussed directly with the individual involved. 2. If one-on-one discussion fails to result in satisfactory resolution, a written complaint must be submitted to the On-site Administrator of Life Time Academy. 3. If the On-site Administrator cannot resolve the complaint to the student s satisfaction in writing within 10 days, the student may file a complaint with the Minnesota Office of Higher Education located at 1450 Energy Park Drive #350, St. Paul, MN 55108-5227. Refund Policy/Buyer s Right to Cancel Each student will be notified of acceptance or rejection in writing. In the event a student is rejected, all tuition, fees and other charges will be refunded. Notwithstanding anything to the contrary, if a student gives written notice of cancellation within five business days of the execution of the contract or day on which the student is accepted, then a complete refund is given regardless of whether the program has started. If a student gives written notice of cancellation after five business days of the execution of the contract or day on which the student is accepted, but before the start of the program by the school, then all tuition, fees and other charges (except for the cost of books and supplies identified as non-refundable), less 15 percent of the total cost of the program (15 percent not to exceed $50.00) shall be refunded to the student. If a student gives written notice of cancellation after the start of the period of instruction for which the student has been charged, but before completion of 75 percent of the period of instruction, the student is assessed a pro rata portion of tuition, fees and all other charges, for those that are not otherwise identified as non-refundable, based on the number of days in the term plus 25 percent of the total program cost (25 percent not to exceed $100.00.) Any notice of cancellation shall be acknowledged by Life Time Academy in writing within 10 business days of receipt of such notice and all refunds shall be forwarded to the student within 30 business days of receipt of such notice. If mailing the letter of cancellation, the student must send it to Life Time Academy, 2902 Corporate Place, Chanhassen, Minnesota 55317, Attention: Student Services Administrator. This refund policy is not linked to any student conduct policy and any promissory instrument shall not be negotiated prior to the completion of 50 percent of the course. Written notice of cancellation shall take place on the date the letter of Life Time Academy (MN January 2012 v4) Page 22 of 28

cancellation is postmarked or, in the case where the notice is hand carried, it shall occur on the date the letter is hand delivered to Life Time Academy On-site Administrator, unless a later date is otherwise specified by the student in the letter. The date of execution of the enrollment agreement shall be presumed to be the date of delivery of the notice of acceptance: and if delivered by mail, the postmarked date of the letter of acceptance. Refund After the Commencement of Classes 1. Procedure for withdrawal/withdrawal date: a. A student choosing to withdraw from the school after the commencement of classes is to provide written notice to Life Time Academy s On-site Administrator. The notice is to indicate the expected last date of attendance and be signed and dated by the student. b. Students unable to complete the required clock hours have the option to complete these hours (with instructor approval) in future sessions without additional tuition. Certificate will not be issued until program requirements are met. c. For a student who is on authorized Leave of Absence, the withdraw date is the date the student was scheduled to return from the Leave and failed to do so. d. A student will be determined to be withdrawn from the institution if the student has not attended any class for 21 consecutive class days without contacting Life Time Academy to indicate an intent to continue in school or otherwise making arrangements concerning the absence. e. All refunds will be issued within 30 days of the determination of the withdrawal date. 2. Tuition charges: Tuition charges will be determined based upon the percentage of the clock hours attempted. The percentage of the clock hours attempted is determined by dividing the total number of clock hours elapsed from the student s start date to the student s last day of attendance, by the total number of clock hours in the program. a. If the student has completed less than 75 percent of the clock hours, the amount charged for tuition, fees and all other charges shall be prorated based on the percentage of clock hours attempted. An additional 25 percent of the total cost of clock hours attempted will be charged, but shall not exceed $100. b. If the student has completed 75 percent, or more of the total clock hours prior to the withdrawal date no refund will be given. 3. Tuition refunds will be issued within 30 days of the date of student notification, of date of school determination (withdrawn due to absences of other criteria as specified in the school catalog), or in the case of a student not returning from an authorized Leave of Absence (LOA), within 30 days of the date the student was scheduled to return from the LOA and did not return. 4. 5. If a student in the Professional Trainer programs has possession of books and/or supplies, after the five day cancellation period, the fees for the items in the student s possession are then considered non-refundable. 6. Workshop fees: Any workshops paid for and attended by the student are not refundable. 7. Special Cases: In case of prolonged illness or accident, death in the family, or other circumstances that make it impractical for the student to complete the program, the school may make a settlement that is reasonable and fair. Course Descriptions In order to take a course listing a prerequisite, the student must have received a passing grade in the prerequisite. PT1000 Leadership, Business, and Sales Students in this course will learn how to become a successful business owner. In addition, students will learn basic business principles and how to create a successful career as a personal fitness trainer. Students will also learn about office productivity software, marketing materials, resume development, business plan development, licensing, and continuing education in the fitness industry. PT1010 Kinesiology of the Human Movement System The study of anatomy, biomechanics and muscles including the principles of the human movement and the description of structure, function, and kinesiology of the extremities. Topics include joints, bones and muscles of the upper extremities and shoulders. Furthermore, students will learn the origin, insertion and action of the skeletal muscles. Students will consider various exercise techniques to understand the kinetics of human motion. PT1020 Anatomy and Physiology Students in this course will learn about the human anatomy including the nervous, endocrine, reproductive, cardiovascular, lymphatic, respiratory, digestive and urinary systems. In addition students will learn about the skeletal system including the bony landmarks and joints, the muscular system to include origin and insertion and function of the major muscles of the body, the integumentary system to include skin and its function and pathologies. PT1030 Exercise and Fitness Assessments Personal Trainers are required to perform subjective assessments, objective assessments, and movement assessments with their clients to provide quality personal training services. This course provides students with a practical opportunity to develop and hone those skills. Students are introduced to training techniques for use in program design courses.
