Tiny imperfections and unique distortions make each person s vision one-of-a-kind. Konrad Filutowski, M.D. For years, these characteristics made it a challenge for ophthalmologists to accurately correct every patient s vision with laser surgery. The older technology corrected a patient s vision according to a glasses prescription only, said Dr. Konrad Filutowski, who performs High Definition LASIK laser vision correction surgery at Filutowski Cataract and LASIK Institute. 2004 Filutowski Cataract & LASIK Institute Wavefront technology offers greater accuracy Continued from page 1 In the past, ophthalmologists could not customize a patient s vision correction procedure. They simply relied upon the prescription for corrective lenses to determine how to proceed. Medical equipment manufacturers harnessed wavefront mapping technology and applied it to the diagnosis and treatment of visual imperfections, including myopia, hyperopia and astigmatism. Wavefront technology is much more accurate than older, conventional technology, Dr. Filutowski said. The older technology corrected 85 percent to 90 percent of patients to 20/20 vision with one surgery. But wavefront technology offers the best results, and Above, Dr. Filutowski points out areas of the eye needing correction utilizing the WaveScan map. This is safer and more accurate than anything else on the market, says Dr. Filutowski. The vast majority of patients I ve treated with this procedure see 20/20 or better. can correct 97 percent of patients to 20/20 vision with one surgery. With enhancement, more than 99 percent of patients can get 20/20 or better vision. We have proven clinical data that has given us these statistics. Wavefront technology was originally developed for use in high-powered telescopes. The technology reduced distortions in space. But in laser surgery, Wavefront technology sends a narrow ray of infrared light into the eye. The light is reflected off of the retina and passed back through the lens, where the machine senses the reflected light and analyzes it for optical distortions. Wavefront technology can improve patients vision, as measured by an eye chart. But it can also improve their ability to see fine details and contrast. When a patient is slated for laser vision correction, Dr. Filutowski uses wavefront technology to map the eye. He then transfers the map and other information to the laser being used for the procedure, where it serves as a guide during treatment. Dr. Filutowski uses the CustomVue System by VISX, an integral part of the Star S4 laser system. Wavefront technology produces information about the shape of the cornea and the rest of the visual system and introduces this into the laser, providing data specific to each patient s cornea, Dr. Filutowski said. With Traditional LASIK, surgeons use a bladed instrument called a microkeratome to manually cut a flap in the patient s cornea. An excimer laser gently reshapes the surface of the cornea with a cool laser beam, thus creating a new curvature for the surface of the eye. During High Definition LASIK procedures, the IntraLase FS femtosecond laser creates the corneal flap. Then the VISX S4 laser uses a small beam to precisely reshape the cornea, which corrects both simple and complex visual distortions. By using a precise, computer-controlled laser to create the corneal flap - rather than a blade clinical studies have shown that complications are virtually eliminated, Dr. Filutowski said. This is safer and more accurate than anything else on the market, he said. Since it treats all of the errors in a patient s optical system, it provides better results than traditional LASIK. The vast majority of patients I ve treated with this procedure see 20/20 or better. The procedure takes about 10 minutes. It can provide better night vision and is designed to reduce or eliminate halos, glare and light contrast disturbances, he added. Following High Definition LASIK, patients continue to tell me how excited and thrilled they are with their own results, says Dr. Filutowski. Dr. Filutowski sets his sights on fast-paced, continually evolving ophthalmology Dr. Konrad Filutowski s love of medicine and passion for advancing technologies combined to make him a pioneer in ophthalmology. Ivy League educated, Dr. Filutowski graduated from Columbia College in 1979. He attended Cornell University Medical College in New York, completing the program in 1983, before serving a three-year residency at Scheie Eye Institute at the University of Pennsylvania in Philadelphia, Pa. Vision is one of the most important senses we have, says Dr. Filutowski. What work could be more exciting and rewarding than having the ability of providing patients with better vision, thus enhancing a person s complete lifestyle. I wanted to work in an interesting specialty, Dr. Filutowski adds. Ophthalmology certainly qualified, since it is a fast-paced field that is continually evolving. Dr. Filutowski s residency prepared him to start his own practice. He chose central Florida (Daytona Beach and Orlando) as the sites for his clinics and surgery centers that bear his name, the Filutowski Cataract & LASIK Institute. From the day he opened for business in 1990, Dr. Filutowski was determined that his clinics would offer patients the highest-quality, most effective techniques for improving their vision. 2004 Filutowski Cataract & LASIK Institute Dr. Filutowski touts advantages of IntraLase Y ou ve heard about LASIK, the surgical procedure that reshapes the cornea and corrects a person s vision in the blink of an eye. Now, variations on the procedure take it to a new level. Dr. Konrad Filutowski touts the merits of IntraLase, which differs from what is known as Traditional LASIK in that the flap in the cornea is made with a laser instead of a blade. Having performed thousands of procedures with a blade and over 4,000 with the IntraLase equipment, Dr. Filutowski says that the IntraLase technique offers many advantages, including greater safety. The femtosecond (one quadrillionth of a second) solidstate laser allows the surgeon greater control and a better chance of success in creating the corneal flap than the bladed mechanical microkeratome. The IntraLase femtosecond laser uses a very short pulse that creates an air bubble 3 microns in diameter, allowing the laser to apply low levels of energy to the corneal tissue with great accuracy. The laser also has a long wavelength (1053 nm), allowing it to pass through corneal tissue with no effect until it reaches a preprogrammed depth. At that point, it causes tissue separation at a precise plane within 10 microns of the programmed depth. The air bubbles are placed close together so that a flap can be easily lifted with a specialized instrument. Dr. Filutowski says one of the greatest advantages of IntraLase over a mechanical keratome is that it can create a thinner flap with greater precision and consistent thickness. He routinely creates a precise 120 micron flap with IntraLase procedures, whereas mechanical keratomes can cut from 100 microns to as much as 250 microns. I have heard that LASIK can give you problems at night. I have heard that you can have one eye done for distance vision and one for close range vision, with LASIK. Is that true? That procedure is called monovision. Monovision is where one eye is corrected for near vision and the other eye is corrected for distance.
