I want to personally thank you for integrating Paragon CRT in your practice. It s an exciting time to get involved in one of the fastest growing modalities in the global contact lens market. Corneal reshaping is quickly entering the mainstream of contact lens fitting, and thousands of practices around the world have begun capturing new market share, while enjoying the incremental revenue that CRT can provide. With mainstream practices focusing on increased profit-per-patient, CRT provides a seamless way to achieve higher profit margins while minimizing chair time. There are two key areas where CRT is making a strong impact: First, tweens and teens, referred to as the 8-to-18 market, need the freedom to be children. During school and daytime activities, contact lenses and glasses can get in a child s way of learning and playing. With CRT, mom and dad have comfort knowing their children are seeing well at school, versus wondering if they are wearing or breaking their glasses, or losing contact lenses. Parents also have the convenience to help manage the care and handling of CRT lenses at home, just before bedtime and upon awakening. Second, soft contact lens wearers that are experiencing discomfort, manifested as feelings of dryness or scratchiness. This group is struggling with daytime discomfort, but continue making every attempt to not return to wearing glasses. By providing total freedom from contacts during waking hours, CRT is simply removing the problem; lens discomfort. Having no lenses on eyes during waking hours allows patients a healthy 16 hours of unobstructed access to oxygen. Paragon recently conducted a survey regarding chair time and the use of staff members when fitting CRT. Of the surveyed CRT Certified Practitioners: - 70% require 30-60 minutes for an initial CRT fitting visit. - 93% take between 15 to 30 minutes for each CRT follow-up appointment. - 78% report using a technician to assist on CRT fitting. - 50% use a technician for lens insertion. Having a successful Paragon CRT wearer on staff can be an excellent reference to potential CRT patients. The CRT SureFIT system provides three lenses per eye to fit and dispense on the initial day of fitting. Once your patient is successfully fit using one of the three lenses, you have 90 days to return the two non-dispensed lenses back to Paragon. In your initial CRT SureFIT order, a pre-addressed, postage paid return envelope will be included. If you fail to return the nondispensed lenses within 90 days, or choose to keep them, your lab of choice will issue an invoice to you charging the list price for the non-dispensed lenses. 6

90-Day Practice Integration Plan CRT Traditional Warranty Program If you chose to fit CRT using the Traditional Warranty Program, this enables you to order lenses from K readings and spectacle refraction. (North America Only) Designate a half-day to fit new CRT patients. Because patients will require a check-up the morning following their dispensing visit, Friday fittings are appropriate if there are Saturday morning office hours; or instruct the patient not to wear the lenses until Sunday evening for a Monday morning appointment. Allow 15-30 minutes for each follow-up exam. Time allotted for CRT fitting is, as always, dependent on the level of delegation of routine tasks to your staff, i.e. visual acuity testing, corneal topography, over-refraction, history, etc. We recommend 45 minutes per initial fitting. 70% of surveyed CRT practices require 30-60 minutes for the initial CRT fitting visit 93% take between 15-30 minutes for each CRT follow up appointment 78% report using a technician to assist on CRT fitting 50% use a technician to apply the initial CRT lenses Paragon CRT Dual Axis can be used to successfully fit a patient when lens centration is an issue when using CRT or DUAL AXIS CRT SureFIT. CRT Dual Axis allows for modulation of the lens periphery in two meridians when corneal elevation or curvature differences limit the success of a fit. It features auto-orientation, with no prism or slab off, while maintaining flexure resistance and an overall thin lens design. The enhanced design maintains an optimum treatment zone and provides a lens perimeter that remains round and planar. The product s ability to permit independent manipulation of a second RZD and LZA, that doesn t alter the dimension found optimum in a first meridian, expands CRT s technological superiority. Establishing Service Fees Corneal reshaping is quickly entering the mainstream of contact lens fitting and practices around the world have begun capturing new market share, while enjoying the incremental revenue that CRT can provide. With mainstream practices focusing on increased profit-per-patient, CRT provides a seamless way to achieve higher profit margins while minimizing chair time. Elements to consider when establishing your fees: If you are using a corneal topographer, it should be slightly reflected in your fees. You are providing a therapy, not just a contact lens. CRT provides visual freedom, without the hassles of traditional daytime vision correction. Visual freedom is invaluable and priceless. Do you want more volume at a lower fee, or less volume at a higher fee? What will be your ongoing renewal fee based on your COG s? To avoid surprises, it is recommended that you advise all patients on what to expect in annual fees. You should establish a cancellation policy in the event a patient decides that Corneal Refractive Therapy is not for them. Many patients think that initial yearly fees are annual costs. 8

Initial CRT Patient Selection Paragon CRT is FDA approved up to -6.00D, with or without 1.75D astigmatism, and there are no age restrictions. A good CRT candidate: Is myopic and falls within the FDA guidelines Is between the ages of 8 to 18 (Tween and Teen) Has an active or athletic lifestyle and currently wears spectacles or daytime soft contact lenses that interfere with their daily activities Adults who suffer from uncomfortable daytime soft contact lens wear that may cause dryness symptoms Adults who are apprehensive about having eye surgery or afraid of a bad surgical result, the reversibility of CRT is attractive Is not a candidate for laser surgery, due to age (too young) or contraindicated Has discretionary income and prefers quality and convenience To gain fitting experience we suggest your first 5 to 10 CRT patients: Are existing soft contact lens or spectacle wearers and fall between the ages of 8 to 18. Many practitioners assume the best candidates for CRT are current GP wearers; however, it has been shown that the best candidates are those who wear soft lenses or spectacles, have moderate myopia with a Manifest Refraction Sphere under -3.00D with or without up to 1.00D of refractive (corneal) astigmatism. Is not a current GP (including Ortho-K) wearer, as these patients probably have cornea molding that could make the transition to CRT more challenging. Initially avoid current GP wearers as fitting CRT can present challenges that can best be overcome after gaining some routine fitting experience. Are within a Flat K range of 41.00 to 45.00D Patients that exhibit any of the following should not wear CRT: Inflammation, infection, red or irritated eyes or suffer severe dryness Any eye disease, injury, or abnormality that affects the cornea or surrounding tissue Any systemic disease that may affect the eye or be worsened by wearing contact lenses Allergic reactions of the eye, which may be caused or exaggerated by wearing contact lenses or use of contact lens solutions Lack maturity and understanding of the risks involved with contact lens wear. Initial CRT Patient Selection The Role of Corneal Topography Paragon strongly recommends the use of corneal topography in the management of Corneal Refractive Therapy. This instrument is acknowledged as an important tool in pre-fit evaluation, baseline documentation and ongoing management of CRT patients. Corneal topography also provides accurate treatment assessment during follow-up and improves chair time efficiency and practice profitability. All contact lens wear, even soft spherical contact lens wear, has an effect on the epithelial layer and its topography. If the pre-crt treatment topography shows a generally symmetrical presentation with a centrally located corneal apex, practitioners should proceed with the fitting and dispensing of CRT lenses. If topography is unavailable, or if topography indicates contact lens wear has substantially redistributed the epithelium from pre-contact lens baseline, then it may be prudent to discontinue contact lens wear 1-2 weeks before CRT fitting and allow the cornea to return to baseline curvatures. In very rare instances, infections of the eye, corneal ulcer, iritis, neovascularization, corneal scarring or permanent decreased vision may occur. The occurrence of these side effects should be minimized or completely eliminated if proper lens care is exercised. Lens Storage and Handling CRT lenses are plasma treated, shipped wet (North America only) and can be stored up to a maximum of 30 days in the unopened vial until ready to dispense. After 30 days the vial should be opened and the solution replenished. Prior to use, the lens should be cleaned and disinfected, following the lens care solution manufacturer s instructions. Paragon recommends the use of Progent, from Menicon, as an effective and convenient way to clean and disinfect CRT lenses. This solution loosens and removes lens protein deposits that can irritate eyes and compromise treatment. Marketing CRT In Your Practice Implementing a CRT marketing campaign doesn t have to cost a fortune, involve an outside marketing firm or consume your valuable time. Paragon strongly recommends taking action to alert your existing patients about the new technology offered in your practice. Here are some quick, easy and inexpensive ways to begin your CRT Marketing campaign. CRT Quick Start Program Place CRT consumer brochures at your check-in counter, in your reception area and exam rooms. Consider a direct mailing to your patient base using the CRT patientspecific recall cards. Place a link to the CRT Consumer website on your practice website: www.crtvision.com. CRT Patient Testimonials - Patient testimonials are a powerful marketing tool that can help other patients become comfortable with the treatment option by learning about the experience and success of others. We recommend you receive permission from patients to use their testimonials. We have included a sample form (Page 16) to help you gather this information. The following are some suggestions on how to implement patient testimonials: CRT Patient Testimonial Book - Fill the pages of a binder with CRT patient stories, thank you cards and other practice memorabilia and place in your reception area to generate patient interest. Practice website - Place CRT testimonials on your website so consumers have the opportunity to read what satisfied patients have to say about CRT. CRT Testimonial Fliers - Hand patients a copy of your favorite CRT testimonial(s) to generate discussion. CRT Wall of Fame - Display CRT patient testimonials and/or photos on the walls of your office. CRT Patient Advocate Program - Many CRT patients enjoy telling their personal success stories to others. Capitalize on these patients by asking them to participate in a CRT Patient Advocate Program. These patients can be used as a resource for patient testimonials at educational and product information seminars and answer questions via email or telephone or write articles for your patient newsletter. Adolescent patient advocates and their parents can be especially helpful to practices by addressing the concerns of other children and their parents. 12

Marketing CRT In Your Practice CRT Patient Referral Program - Studies indicate that one satisfied CRT patient will refer an average of four new patients. To support this opportunity we suggest developing a CRT Patient Referral Program. CRT Informed Consent Document Example of CRT informed consent You are being fitted with gas permeable (GP) contact lenses, also known as Corneal Refractive Therapy (CRT). Corneal Refractive Therapy refers to the use of specially designed GP contact lenses to temporarily reshape the cornea (the clear layer on the front of the eye), allowing you to see clearly without the use of glasses or contact lenses during waking hours. The Corneal Refractive Therapy contact lenses must be worn on a regular basis during sleep in order to reduce the need for glasses or contact lenses during the day. Complications and Side Effects Corneal Refractive Therapy carries the same risks as other types of contact lenses, such as swelling of the cornea, scratching of the eye, irritation, infection, unusual eye discharge, excessive tearing, dry eyes, sensitivity to light, pain, redness, and distorted vision. These risks are usually temporary if the contact lenses are removed promptly and if appropriate professional care is received. In some instances permanent corneal scarring, infection, or blood vessel growth on the cornea may occur, which can lead to reduced sight in rare cases. Although uncommon, infection of the cornea can develop rapidly and lead to loss of vision. The risk of infection of the cornea has been shown to be greater among patients who wear their lenses overnight than among those who do not sleep in their lenses. Corneal Refractive Therapy also has risks that are not typically associated with other types of contact lenses, such as blurry or variable vision, especially late in the day. The blurry vision and how long it lasts each day should decrease with time. You may also experience distortions or ghost images, particularly outside at night which may affect night driving. For the first few weeks, the risk may be increased in patients with a high degree of correction or large pupils. All risks are minimized if you follow the correct contact lens wearing schedules and care procedures, remove your contact lenses if problems occur, and report to your primary eye care practitioner as soon as possible. With any procedure, there may be unforeseeable risks. If you experience any of the symptoms listed above, remove your lenses immediately. If the condition continues after lens removal, you should immediately call for an appointment or consultation with your eye care practitioner who will provide the necessary treatment. In order to keep your DDS working at its fullest potential, it s important to develop a lens ordering system. 28 Once a lens is dispensed from the DDS to a patient, most practices wait about one week before ordering a replacement lens to fill the hole(s) in the DDS. One easy way to manage the lenses that need to be ordered is to flip over the vials of the dispensed lens(es) and put a round sticker on the bottom with the patient s information. Another way is to designate a tray for the vials of dispensed lenses. When you are ready to order your replacement CRT lenses, contact your Authorized Paragon CRT laboratory. Gathering the story 3. Receiving patient permission on story use Approach patients who are enthusiastic about the change CRT has made in their lives at their two week or one month appointment, or when their daytime vision has stabilized. CRT Patient Referrals Generating, tracking and acknowledging the referrals of friends and family from happy CRT patients should be part of your marketing strategy. Word-of-mouth referrals are the strongest practice builder and the most cost effective form of marketing. Most computer systems are set up to track new patients on the scheduling, but if this isn t an option for you, manually track how new patients found out about your practice on their new patient intake form. One way to generate referrals is to order a specially designed business card with a message that focuses solely on the referral of new patients. This tactic is an inexpensive way to spread your marketing message. 29

If you stop wearing the lenses regularly while you sleep, your vision will return to its original state in as little as 72 hours. Can everyone wear CRT? CRT treats nearsightedness (myopia up to 6.00 diopters) with or without minor astigmatism (up to 1.75 diopters). And because there are no age restrictions with CRT, it may be the perfect option for patients that experience discomfort while wearing daytime contact lenses. At this time, Paragon CRT is not approved for treatment of farsightedness. How long does it take to reach good vision? Most patients have rapid improvement in the first few days of treatment and achieve their optimum vision in 10 to 14 days. What should a patient expect during the 10 to 14-day treatment process? It s important to understand that for a short time after a patient has begun treatment, but before sufficient treatment is realized, old glasses or contacts will no longer be the appropriate prescription. It may involve re-insertion of CRT lenses for part of the day or some use of temporary soft lenses in different prescriptions. The doctor will discuss the options with each patient for visual correction during that temporary period of time. Are there any risks involved with wearing CRT? There is a small risk involved when any contact lens is worn. It is not expected that the CRT lenses for contact lens Corneal Refractive Therapy will provide a risk that is greater than other contact lenses that are worn during sleep. There were no serious adverse events reported in the Paragon CRT FDA clinical study. (See the package insert online at www.crtvision.com/faqs) Can I see with my CRT lenses in? Yes. One of the great features of the CRT lens is that if you get up in the middle of the night, you will be able to see perfectly. You will enjoy great vision with your CRT lenses 24 hours a day, on or off! How often will I have to replace my CRT lenses? Generally speaking the Paragon CRT lenses will have to be replaced annually. However, depending on factors such as protein buildup, how well the lenses are taken care of, etc., the lenses may have to be replaced more or less frequently. How do I care for my CRT lenses? Your lenses should be chemically disinfected after every use (not heat). Your doctor will instruct you about which care system is best for you. 30

CRT Staff Training / Frequently Asked Questions How much does CRT cost? Our initial cost for Corneal Refractive Therapy with Paragon CRT is $. This includes: (list products and services included in the fee). We also offer optional payment plans, which we will be happy to review with you. Payments can be as low as $ per month. Also, for subsequent years, the cost of CRT is about the same as regular contact lenses or glasses. Is Corneal Refractive Therapy painful? No. The therapy is not painful. Initially, you may have a slight awareness of the therapeutic lens. You will not feel the lenses when you sleep and there is no sense of physical corneal change just visual improvement when the lenses are removed. Is Corneal Refractive Therapy FDA-approved? Yes. CRT was the first therapeutic lens design approved by the FDA for overnight contact lens Corneal Refractive Therapy for the temporary reduction of myopia. Paragon Vision Sciences, the manufacturer of CRT, received the final written approval on June 13, 2002. The issuance of this approval is the culmination of the most extensive clinical study to date to establish the safety and efficacy of contact lens corneal reshaping in overnight use. What is the difference between Corneal Refractive Therapy and CRT? Corneal Refractive Therapy is the process of using a therapeutic contact lens to reduce myopia by reshaping the corneal surface while you sleep. CRT is the product used for this therapy. Are there age restrictions for Corneal Refractive Therapy with CRT? No. The FDA placed no age restrictions on candidates for Corneal Refractive Therapy with CRT. CRT is being successfully performed on children 8 years old and younger. It can be a great option for children and teens that are active in sports activities or any other extra curricular activities. Those who are older and presbyopic (requiring reading glasses or bifocals) may still be candidates for CRT. They may correct their distance vision with CRT and wear near vision glasses for close work. At the doctor s discretion you may be fitted in a monovision mode (one eye corrected for near vision and one corrected for distance vision). Visual requirements due to age or activities can only be made after a thorough eye exam and recommendations of a CRT certified eye care professional. Are CRT lenses difficult to apply or remove? No. These lenses are generally no more difficult to insert or remove than any other similar contact lens modality. You may want to instill comfort or rewetting drops prior to insertion of the lenses and immediately upon waking. The lenses must move freely prior to removal. If you have difficulty removing the CRT lens, there are lens removal aids available to assist this process. 31

CRT Staff Training / Frequently Asked Questions If I discontinue CRT, will my vision return to normal? Yes. There are no recorded permanent corneal changes post-treatment. Generally, the more myopia being reduced, the longer it will take for the corneal curvature to return to normal. For instance, if your pretreated correction was -4.50D, it would take a few days longer to return to normal than if the pretreated correction was -1.50D. My driver s license says I need corrective lenses to operate a motor vehicle. What do I do? You will be supplied with a document that states you are participating in a visual therapy program that eliminates the need for corrective lenses while operating a motor vehicle. The document should have an expiration date for this treatment and you should have your driver s license updated with a new vision screening through your local DMV office to reflect this change before the expiration date. What happens if I lose or damage a lens? Corneal Refractive Therapy is different than wearing regular contact lenses. If you discontinue wear for one night, your vision may be impaired the next day. Previously worn glasses or contact lenses may not help. Immediate replacement is necessary! Having a spare pair of lenses is strongly recommended. Will my insurance plan cover Corneal Refractive Therapy? This is an elective procedure and, to the best of our knowledge, is not usually covered by benefit programs. If regular contact lenses are covered under your vision care plan, Corneal Refractive Therapy may be partially covered. You should check with your benefits plan administrator. I suffer from dry eyes. Am I a candidate for Corneal Refractive Therapy? Only your eye care professional can determine if you are a CRT candidate. Many patients with slightly dry eyes do well with CRT. Since you are wearing the lenses only while sleeping, the closed eye minimizes evaporative loss of tears and may give dry-eye patients suitable vision without compromising the ocular surface. I used to wear contact lenses, but stopped due to discomfort. Am I a candidate for Corneal Refractive Therapy? CRT lenses are worn at night while you are sleeping. This greatly reduces any discomfort normally experienced while wearing contact lenses during the day. I have astigmatism. Am I a candidate for Corneal Refractive Therapy? CRT is designed to correct myopia with up to 1.75 diopters of astigmatism. Each individual should be evaluated by a certified eye care professional to determine if CRT is right for your visual requirements. How can I tell if I have the original Paragon CRT lens? Each CRT lens is laser-marked with a series of very small numbers and/or letters. If your lens does not have a laser mark, you can rest assured you are not wearing an authentic CRT lens and you should consult your eye care practitioner. 32
