AUTHORIZATION For purposes of processing my application, I (we) do hereby authorize Leelanau County, in the person of the Planning Director and/or Housing Manager and/or Building Inspector, to make an inspection of my (our) house, for the purpose of determining the housing repairs that may be necessary to bring the house up to Uniform Physical Condition Standards (UPCS) as defined by MSHDA and HUD. I (we) understand that the repairs necessary and the amount of money required for such repairs will be the basis for a loan application from Leelanau County. I (we) also understand that an inspection of the house is in no way a guarantee that a loan application will be approved. The undersigned certifies that all information in this application, and all information furnished in support of this application, is given for the purpose of obtaining a loan under the Leelanau County Housing Rehabilitation Program, and is true and complete to the best of the applicant's knowledge and belief. The undersigned also certifies that they have received a copy of the 2 page document titled What to Expect and What Not to Expect From The Program. The information is kept confidential and is only used to help determine eligibility for a loan. Verification of any information listed in my application is authorized including a credit report, if necessary. If an on-line Annual Credit Report is obtained from one of the three reporting companies, a copy will be given to the applicant upon request. Under Michigan's Freedom of Information Act (Act 442 of 1976, as amended), individuals or organizations have the right to receive the names, addresses, and amount of rehabilitation assistance provided to homeowners under this program. However, they are not entitled to see private information such as: income, sources of income, credit information, social security numbers, etc. This information is confidential. A copy of this form is sufficient to obtain the requested information. This Page Homeowner Copy WHAT TO EXPECT AND WHAT NOT TO EXPECT FROM THE PROGRAM Things That Homeowners Do in the Housing Rehabilitation Program The Leelanau County Housing Rehabilitation Program will help homeowners during the home improvement process. Homeowners will receive a free copy of the Housing Program Guidelines. Homeowners are responsible for the following: 1.) Homeowners provide the necessary information promptly. 2.) The Housing Review Committee will approve the loan and contractor. 3.) Homeowners sign home improvement contracts with the selected contractor. 4.) Homeowners sign payment requests from the contractor. 5.) Homeowners work with contractors to settle any disagreements during the job. 6.) Homeowners contact the contractor to correct any problems covered by the contract within 18 months of the final inspection. 7.) If the application for assistance is denied, the Homeowners have the right to appeal the denial. Things to Consider when Taking Out a Home Improvement Loan 1.) Not all the work that homeowners want done can always be done. 2.) Contractors must be licensed and insured. 3.) Don't expect the house to be completely new after the work is finished or all floors, walls, etc. to be level, and plumb. Workers can not make an old house like new. 4.) It can be stressful living in a house while contractors are working. 5.) Very few times in life is anyone completely satisfied with things they buy or have repaired. Buying a house or having a house repaired is no different. 6.) Houses always need improvements. It would be a good idea to save $25.00 or more each month to help cover the cost of future repairs and maintenance. 7.) The County housing staff is not the contractor and cannot guarantee the work. Homeowners may not be completely satisfied with all the work done, and should refer to warranties and the contract. 8.) This is a LOAN and requires regular monthly payments. 9.) Can we afford the monthly payments? Type of work that can be done with a housing rehabilitation loan: The home will be inspected to see what items fail the inspection for Uniform Physical Condition Standards (UPCS). Rehabilitation activities must be activities allowed by Michigan State Housing Development Authority (MSHDA). Examples of work that may be allowed: New roof, new windows or new doors Heating (new furnace) Plumbing and wiring improvements New baths or new kitchens if in poor condition New well or septic system New siding, porches, foundation work, insulation Handicap access, ramps, etc. Additions (only if there are crowded conditions) Insulation Leelanau County Housing Rehabilitation Program Summary The Housing Program provides low interest loans (zero to 3 percent) to low and moderate income homeowners in Leelanau County. Loans up to $40,000.00 are available for the rehabilitation of owneroccupied, single family residential housing units. The program is not intended to provide new housing only rehabilitation of existing housing. A1 Application 2015 version Page 7

This Page Homeowner Copy The program is not intended for rehabilitation of rental units or secondary homes. Applicants must meet HUD income guidelines and certain ownership and residency requirements. Summary of the application and rehabilitation process: Homeowners submit a completed application and related documents. The office processes the application and verifies information for eligibility. Inspection of the house is made to verify the work needed. Where applicable, Lead Based Paint (LBP) and asbestos inspections will also be conducted. Work specifications and cost estimate are developed, and reviewed with homeowner. The approved work specifications are sent to contractors for bids. Bids are reviewed and a contractor is recommended (generally the lowest bid). The HRC approves the loan and contractor(s) for the work. Meeting is held with the homeowner and contractor so both understand the work to be done and cost. Loan documents are signed. Contract between contractor and homeowner is signed. The three day rescission period is completed. Work begins on home with required permits and inspections. Final approval of the work, contractor payments, and full unconditional lien waivers are completed. Loan payments begin. The homeowners supporting documents such as income tax forms, monthly pay stubs, employment, property ownership, credit history, property taxes, and homeowners insurance are reviewed by staff and verified. This process can take several weeks unless it is an emergency project. Lead Based Paint testing is conducted on homes built before 1978 (depending on dollar amount and the work to be done). Also, a clearance from the State Historical Office for homes over 50 years old may be required. If rehab costs are over $2,500.00, a mortgage is placed on the property and a Promissory Note and Truth In lending statement are completed. 2015 Income limits for homeowner rehab loans and ADR home buyers (6/1/2015 to 6/30/2016): The table below indicates the Area Medium Income (AMI) limits for a family to be eligible for a homeowner rehab loan from the county. The household income must be at or below the amount in the table and is based on family size. These funds are based on an income range depending on project costs and other resources that may be available to the applicant.
