Endorsement Schedule Endorsements attaching to and forming part of Policy No 63-4002271-VFT. Except to the extent provision is already made, or more extensive cover is already provided by the printed policy wording, it is agreed that the following clauses are incorporated in the Policy. Should any ambiguities exist in the clauses they will be interpreted in favour of the insured. Headings have been included for ease of reference and it is agreed that the Terms, Conditions and Exclusions of this Policy are not to be construed and interpreted by reference to such headings. With effect from 30/06/2014 at 4:00pm this policy is endorsed as follows: Capped excess any one event You shall be liable for the excesses as detailed in the policy, which applies to each claim throughout the period of insurance. Should the total of all excesses paid by you arising out of any one event during the period of insurance reach $30,000, then the policy shall revert to a nil excess basis for any further claims arising out of that event. Claims Experience Discount Subject to our retaining a minimum premium of $15,000 and you renewing your policy for a further period of twelve (12) months with us, we agree to reduce your renewal premium for this policy by an amount calculated as follows: 70% of the adjusted premium $ less total incurred claims costs for the period of insurance $ sub-total of surplus $ 50% of surplus $ subject to a maximum rebate of 10% Adjusted premium means the premium paid for the period of insurance after application of the Declaration of vehicles clause contained in Term and Conditions applicable to All Sections.

Page 2 Endorsement Section Incurred claims means total claims paid by us, less any recoveries received by us, during the period of insurance plus an amount for outstanding claims, including an allowance for claims incurred (net of any excess) but not yet reported to us up to a maximum of 90 days after expiry of the policy. Councillors / Directors / Employees Vehicles Additional Covers - Section 3, 1.4 'Councillors / Directors / Employees vehicles' is deleted. Declaration of Vehicles Notwithstanding Terms and Conditions applicable to All Sections, 3.5 Declaration of vehicles, no premium adjustment shall occur unless either the number of vehicles or their value (as the case may be) has increased by 10% or more during the period of insurance. Other Owners Indemnification Clause Section 3 Additional Cover applicable all Sections 1.5 Cover for Other Owners is deleted and replaced by the following: This policy extends to provide cover to any owner, hirer or lessee of any vehicle, of which you are not the owner, hirer or lessee, but also for any other vehicles for which you are responsible or have assumed responsibility or in which you have an insurable interest, or which you have received instructions to insure. Section 3 Additional covers applicable to All Sections, 1.5. Cover for other owners is extended to include Councillors and/or Directors and/or employees and/or, Bush Fire Services and/or State Emergency Services and/or Civil Emergency Services and/or community groups and/or volunteer workers (other than those of the Volunteer Fire Brigade Member Vehicles) together with any additional Equipment (which includes but is not limited to movable plant, hand tools, rescue equipment, ropes ladders and the like) associated with the use of that vehicle but only whilst such persons are acting within the scope of their employment or authority and using their own or their spouse s vehicle within the scope of your authority.

Page 3 Endorsement Section For the purpose of this extension, nil excess applies and such cover is not subject to Declaration of vehicles. For the purpose of this extension, Section 2.5 Non Owned or Supplied Vehicles does not apply. Fire and Theft Cover Where your vehicle noted in the schedule is to be covered for fire and theft, then Section 2 Insurance Cover for Third Party Liability is deleted and the respective vehicle is only covered for damage caused directly by fire or theft. Hire of Vehicle following Theft Additional Covers - Section 1, 5.8 'Hire of vehicle following theft' is deleted and replaced with the following: 5.8 Hire of vehicle following theft In the event of your vehicle being stolen, we will, at our option, arrange hire of, pay the cost of, or reimburse you the reasonable cost of, the hire of a vehicle, to a maximum amount of $5,000 per event / per vehicle. On the basis that: 5.8.1 the payment / reimbursement is limited to the costs incurred by you after you have notified us of the loss; and 5.8.2 the substitute vehicle is of a similar like and kind to that stolen; and 5.8.3 the payment / reimbursement will cease on the day its been recovered and repaired, or the day we offer settlement to you for the stolen vehicle, whichever occurs first. Special Condition to Section 2 The Insurer will not provide any cover for death or bodily injury under Section 2 of this policy unless prior to the commencement of this policy, you have provided to the Insurer the following details of any other policy covering you for legal liability for death or bodily injury (other than CTP or workers compensation insurance) which is in force as

Third Party Liability Cover Only Where your vehicle noted in the schedule is to be covered for Third Party Liability, then Section 1 Cover for Loss or Damage to your Vehicle is deleted. Waiver of Subrogation/Release of Liability We agree to waive any rights, remedies or relief to which we may become entitled by subrogation against: (a) any company, corporation or organisation (including their directors, officers, employees, volunteers or servants): (i) associated and/or related and/or affiliated companies owned or controlled by you, or (ii) which is a subsidiary of yours, or (iii) in which you have a financial interest, or (iv) which comprise the insured under this policy; (b) any person and/or firm and/or company having an insurable interest herein;

Page 5 Endorsement Section (c) (d) any Railway Authority or Government, Semi-Government or Statutory or Municipal Authority, where required under the terms of any agreement or contract entered into by you; any directors, officers, employees, volunteers, community group or servants, partners or individual connected herewith, at the option of you. This insurance shall not be prejudiced by you agreeing to such provision and that the indemnity and/or release given by you shall be equally binding upon us, whether such releases or agreements are given in the past, present of hereafter. Increased vehicle testing limit The limit of cover stated in Section 3 Additional cover applicable to All Sections Section 3, 1.15. Vehicle testing is increased to $600,000. Police, Fire Brigade and other Authorities The limit of cover stated in Section 3 Additional cover applicable to All Sections Section 3, 1.12. Police, Fire Brigade and other Authorities is increased to $100,000.
