D. ~ Superintendent of Schools March 2015 We are excited to introduce online self enrollment through FFenroll for supplemental benefit options for the 2015 2016 plan year. In order to continue, add, or cancel supplemental benefit coverage for the 2015 2016 plan year, all employees MUST access their supplemental benefit options electronically through the FFenroll site during the open enrollment period scheduled from April 1, 2015 April 26, 2015. Coverage will begin September 1, 2015. Please study this booklet carefully before completing the online self enrollment process in order to make a well informed decision about supplemental benefits for yourself and your family. Overview Plainview Independent School District and First Financial Group of America would like to take this opportunity to present to you the information for the upcoming plan year. This information has been created to bring forth a brief overview of your choices as well as offer you a reference guide when questions may arise regarding your insurance plans. Please take the time to look over the information contained in this booklet to familiarize yourself with the benefits that are provided to you as an employee with PISD. Open Enrollment will be April 1 April 26. All employees must review plan options and make any necessary changes to your supplementary elections under the Cafeteria Plan. This is the only time you can make changes to your supplemental insurance, unless there is a qualified family status change during the year. The Plan Year for Plainview ISD is September 1 through August 31. Payroll deductions for your benefits will begin in September. Open Enrollment for TRS Medical Plans will occur at a later date. You will be notified of new plan rates and information when this information is available. No changes will be made to Medical Coverage at this time. This enrollment is for Supplemental Insurance only. This guide contains a summary of the benefits offered by Plainview ISD. If there is a conflict between the terms of this outline of benefits and the actual contracts, the terms of the contracts will prevail. View/Self-Enrollment Instructions How do I view/enroll my benefits prior to open enrollment? Conveniently, you can view your benefits, enroll or make any necessary changes for the upcoming plan year at work or at home using our secure, online website. Where do I go to view/enroll in my benefits? Go to https://ffga.benselect.com/enroll. What is my login and PIN? Your login is your social security number (123456789). Your pin is the last four digits of your social security number and the last two numbers of your birth year (678977). You will arrive at the Welcome screen. You may use the Navigation buttons (the Next and Back buttons) to move through the website. Verify your personal information Verify all dependent information (ssn/date of birth) **Very Important** View employment information Please note that you will need to either enroll or decline all voluntary benefits before the system will allow you to sign and complete your elections. By seeing Congratulations, this means your elections are complete. Should you find any discrepancies, you can contact First Financial at 800-883-0007 or email them at plainview@ffga.com. Useful Information to know Contact First Financial @ 855-523-8422 with any technical questions. No changes will be allowed until the annual open enrollment period (unless you have an IRS S125 approved event).

Section 125 Cafeteria Plan First Financial Administrators, Inc. As a district employee, you are eligible to participate in a Section 125 Flexible Plan. Enrollment opportunities are limited to the plan year dates for your district. A Section 125 Flexible Plan allows you, the employee, to select from a list of available benefits that will meet your family s healthcare needs. Certain premiums are deducted from your gross earnings before federal withholding taxes are figured. The amount you elect to have deducted pre-tax actually lowers your taxable income. By implementing this plan, your employer is helping you reduce your taxes and increase your take home pay. You cannot change your elections during the plan year except for certain specified changes in family status. Those changes include: Marriage Divorce Death of a spouse/child Birth or adoption of a child Termination of spouse s employment You must notify your employer within 31 days of the qualifying event to make changes. Section 125 Plan Sample Paycheck The example below shows how a married employee claiming 1 exemption can reduce their taxable income

District Paid Teladoc Plainview ISD is committed to keeping you and your family happy and healthy. Don t let a trip to the Urgent Care facility keep you off the job. 2/3 trips to the Urgent Care could have been handled via Telemedicine. You and your family received free coverage with Teladoc as an employee of PISD. Be sure to add your dependents during enrollment so they will be covered. District Paid Group Life Insurance Plainview ISD pays for $10,000 of life insurance as long as you are employed by the district. This coverage is one of many benefits afforded to those working for the district. See Brochures PDF for details. Voluntary Group Life Insurance Employees have the opportunity to purchase additional Group Life Insurance. Group Life Insurance is a great way to cover short-term life insurance needs. The rates are among the most affordable and no medical exam is required for new employees. You must sign up for coverage within 30 days of employment to be eligible for this policy. Rates increase every 5 years and coverage does not continue upon retirement. See Brochures PDF more details. Portable Permanent Life Insurance For your long term insurance needs, Plainview ISD offers portable permanent Texas Life Insurance. Applying for coverage is easy; just answer 3 work and health related questions with express issue underwriting. Coverage may be kept after retirement. See Brochures PDF for details. Disability Insurance This insurance is designed to protect your ability to earn an income. This plan will provide you with income should you become disabled as a result of a covered accident or illness. Most people have inadequate resources to keep their bills paid, should they become unable to work for an extended period of time. Disability Insurance helps them know that should something happen that keeps them from being able to come in, they may not have to worry about the loss of a monthly paycheck. See Brochures PDF for details. Dental Insurance Plainview ISD offers a 2 Tier Dental plan. Both plans pay the standard 100% for preventive, 80% for minor work, and 50% for major work. The High Plan pays up to $1,500 a year and allows you to see ANY DENTIST. The Low Plan pays $750 and only allows you to see a Network Provider. DO NOT ENROLL in the Low Plan without verifying that your provider of choice is in the network. Visit www.ameritas.com to search for providers. See Brochures PDF for details.

Vision Insurance Eyetopia Vision provides a rich vision plan. This is a 2 Tier Plan with comparable benefits to most vision plans on the Basic Plan and enhanced benefits on the Gold Plan. Vision Insurance includes a benefit for an annual eye exam and the option to get 1) Glasses (Frames and Lenses) 2) Contacts OR 3) LASIK. Cancer Insurance 25% of people reported that they used up all or most of their savings dealing with cancer. * Most of the costs related to treating cancer are not directly for medical bills. Even with health coverage, many people want help for the rising costs of deductibles and copays. Cancer insurance is a great way to protect you and your family from the financial difficulties associated with cancer treatment. Coverage is issued with NO MEDICAL QUESTIONS during the first 30 days of employment. Accident Insurance Accident Insurance helps to cover the expenses for emergency room costs, follow-up treatments, medical imaging, hospital confinement, and many other expenses associated with accidental injuries. This plan can help with medical expenses and living costs when you get hurt unexpectedly. It pays cash to you, the employee, which you can use to help with those unforeseen costs. See Brochures PDF for details. Critical Illness Insurance Critical illness can come at a high price. Critical illness insurance offers you financial protection when you need it the most with cash benefits paid directly to you. Supplementing your major medical with critical illness insurance helps you pay for your care so you can focus on getting well. Critical illness insurance may cover: Severe burns, Coma, Paralysis, Heart attack, Permanent damage from a stroke, Bypass surgery, Kidney failure, and organ transplant. Flexible Spending Accounts (FSAs) Flexible Spending Accounts offer a money saving way to pay for eligible medical and dependent daycare expenses with pre-tax money from your paycheck. FSA s are a type of employee benefit that comes from Section 125 of the Internal Revenue Code. Take advantage of this benefit during enrollment. Simply choose the deduction amount and funds are available for eligible expenses throughout the year. The IRS allows employees to contribute up to $2,550 annually to a Medical Reimbursement FSA. Benefits Flex Card The Benefits Flex Card is available to all employees that participate in Medical Reimbursement FSA. The Benefits Flex Card gives you immediate access to your money at the point of purchase. Cards are available for participating employees, their spouse and eligible dependents that are at least 18 years old. The IRS requires validation of most transactions. You must submit receipts for validation of expenses when requested. If you fail to substantiate by providing a receipt to First Financial within 60 days of the purchase or date of service your card will be suspended until the necessary receipt or explanation of benefits from your insurance provider is received. FF Flex Mobile App The FF Flex Mobile App is available for Apple or Android TM devices on the App Store SM or the Google Play Store TM. With the FF Flex Mobile App you can: Submit Claims View Account Balance & History See Claim Status View Alerts Upload Receipts and Documentation Dependent Care Reimbursement A Dependent Care FSA allows you to pay for daycare expenses for your qualified dependent/child with pre-tax dollars while you (and your spouse) are working, seeking employment, or attending school as a full- time student for at least 4 months during the year. Eligible dependents must be claimed as an exemption on your tax return. These dependents can include step-children, grandchildren, adopted children or foster children. Under IRS regulations, eligible dependents are further defined as: under age 13 and/or physically or men- tally unable to care for themselves, such as a disabled spouse, disabled child, or elderly parents that live with you. The IRS allows employees to contribute up to $5,000 annually to a Dependent Care FSA.
