New medical ID cards for High Option Supplement and Medicare PPO plans As part of a system upgrade on September 1, 2015, Blue Shield of California will issue new medical plan ID cards that include a new member number and a new group number. These new ID card will have a new look and feel, and each enrolled family member will receive their own ID card. Important note: Members who are enrolled in the High Option Supplement to Medicare Plan or Medicare PPO Plan with Rx plan have two ID cards one for medical benefits and one for pharmacy benefits. This new ID card is only for your medical benefits (not pharmacy). You received a new Blue Shield of California Medicare Rx Plan (PDP) ID card in January 2015. What this change means for you Expect to receive new ID cards for you and/or your family members by mail from Blue Shield during the first week of September. (If you are covering 10 or more dependents, you can expect to receive more than one envelope because each envelope can only contain up to 10 ID cards.) Continue using your current medical ID card to obtain medical services until your new card arrives. Keep and continue using your pharmacy ID card to obtain or refill your prescriptions. Present your new medical ID card on your next visit to your physician or other healthcare provider. You can print a temporary ID card or request additional copies of your card online after September 1 by going to blueshieldca.com/uc, selecting Login to blueshieldca.com, and then clicking on View/Print ID Card (on the top right). You can also request additional ID cards by calling Shield Concierge at the numbers listed below. If you already have a blueshieldca.com online account, continue to use your current user ID and password. If you do not have an 1
online account, please use your new member ID if you are registering for the first time on or after September 1. If you use the pharmacy mail order service and are registered on primemail.com, you will need to change your member ID on that site to your new number. If you do not have a primemail.com account and would like to use the pharmacy mail order service, please use your new member ID if you are registering for the first time on or after September 1. If you are registered on HealthEquity.com, continue to use your existing login and password to access your Health Savings Account, Post-Deductible Health Reimbursement Account, or Stand-Alone Health Reimbursement Account (PDHRA/SAHRA). Enrollment in Blue Shield of California depends on contract renewal.
