General Rules and Conditions, SwissAssist Plus SA. With the purchase of the Swiss Assist Card, (relating to the company SwissAssist Plus) the card holder has the right to use the services of the company Swiss Assist Plus SA according to the General Rules and Conditions, (GRC) until the expiration of the card. Scope and description of the different types of cards available. European Cover: The European area of application includes all the nations on the European continent plus the Mediterranean and Canary Islands, Madeira and the nations that border the Mediterranean but are outside Europe. Please check in the list of countries. Fortnightly Card: (two weeks) This includes free repatriation in the event of illness or accident requiring more than three days in hospital and who are at least 400 km from home. This service is available for the cardholder and one child below the age of ten at the time of purchase. maximum of two members of the cardholder s family may travel free on the air ambulance if they have valid travel documents (passport, etc.) In the purchase form selected, the starting date of coverage is valid for 14 days. Monthly Card: this includes free repatriation in the event of illness or accident requiring more than three days in hospital and at least 400 km from home. This service is available for the cardholder and one child below the age of ten at the time of purchase. member of the cardholder s family may travel free on the air ambulance if they have valid travel documents (passport, etc.). In the purchase form selected, the starting date of coverage is valid for 31 days. Annual Card: this includes free repatriation in the event of illness or accident requiring more than three days in hospital at least 400 km from home. This service is available for the cardholder and one child below the age of ten at the time of purchase. member of the cardholder s family may travel free on the air ambulance if they have valid travel documents (passport, etc.). In the purchase form selected, the starting date of service coverage is valid for 365 days. Family Card: this includes free repatriation in the event of illness or accident requiring more than three days in hospital and who are at least 400 km from home. This service is available for all the member of the family (Parents and children under 18 years old at the time of purchase) maximum of two members of the cardholder s family may travel free on the air ambulance if they have valid travel documents (passport, etc.). 1

Please read carefully the list of countries served. Any questions concerning the validity of the country must be made in writing to admin@swissassist.ch before departure. Services provided by Swiss Assist Plus SA. The cardholder has the right to contact the Swiss AssistPlus call centre in the case of illness or injury. The client needs to be at least 400 Km from his home, and within the area covered by the service, and at least a three (3) days prognosis foreseen in hospital. The area covered is detailed in the countries list, which is attached to this GRC. The list takes part of the general terms and conditions. In the case of areas considered as war zones The Company SwissAssist Plus requires written notification prior to departure these areas. These areas will change and any intend to travel to possible conflict areas without prior notification will render the repatriation invalid. The card holder has the right to be brought to a hospital or specialise centre near where he is domiciled, which is specified when he bought the card, without any costs, aboard an air ambulance unless contraindicated for medical reasons. SwissAssist plus will not pay for search and rescue costs in the case of skiing or other activities. If the cardholder changes their address, this has to be indicated to the offices of SwissAssist Plus, especially if the cardholder moves out of the repatriation zone. SwissAssist Plus has the right to refuse cover if the member changes his domicile. The transport from the hospital to the airport and transport to the hospital after the flight is always included in the service. The cardholder can also include one child for free, under the age of ten years at the time of purchase, but this name and age must be registered at the time of purchase. General conditions for using the services of SwissAssist Plus. The cardholder must be at least 400Km from their domicile and within the covered area. At the time of purchase, the cardholder must NOT suffer from an acute illness or be in an unstable phase of an illness at the time of travel. In the case of a previous accident, we request that the client is no longer in an acute phase of recovery. In all cases, people who left their domicile in an acute or unstable phase of illness or accident that justifies a hospitalization, they may not be accepted for repatriation. This fact will be checked by a SwissAssist Plus medical doctor who has the right to not accept a transport if the problems are considered potentially unstable or acute before leaving the domicile. The service is available for people, even with pre- existing non- acute medical conditions, providing they are stable and deemed fit to travel by their doctor and also for women who are not more than 24 weeks pregnant. A fit to travel person should be able travel unaided, without the aid of oxygen and to be self sufficient in walking, stairs, etc. 2

We do not accept person who is resident in a hospice. People wishing to travel within two months of receiving either chemotherapy or radiotherapy unfortunately are unable to participate this repatriation cover. After this period, providing the person is fit to travel, there will be no problem. We ask you to contact SwissAssist Plus by E- mail if you have any questions of doubts as to the validity of your purchase. We welcome any questions in writing and are happy to help if you think you have a medical condition that will maybe invalidate your cover. Failure to disclose a serious medical condition might render your repatriation cover invalid. Please, if you are in any doubt, ask us in writing. In order to benefit from a repatriation by ambulance jet requires that, at the moment a Swiss Assist Card Plus card holder is being hospitalized with a forecasted stay in hospital for at least 3 days, Swiss Assist Plus has to be informed as soon as possible via its call centre either by the card holder, an accompanying person or a relative to the card holder or a person he/she knows. Missing compliance with this obligation to inform SwissAssist Plus may result in a rejection of the repatriation service. Repatriation by ambulance jet is only granted to SwissAssist Plus Card holders who are hospitalized at the time of intervention. SwissAssist Plus Card holders who have been released from hospital before repatriation mission starts are not entitled to any repatriation by ambulance jet regarding that specific case. In the case of illness or injury, the cardholder must allow the treating medical doctor to give access to his or her clinical and medical information to the SwissAssist Plus medical staff. The client's medical condition is a medical secret and will not be disclosed to a third party and used solely for the purpose of a safe air transport and medication. SwissAssist Plus will choose the closest hospital to the cardholder's domicile. The choice will be made according to the pathology and the insurance cover of the cardholder; under the condition that Swiss Assist Plus has been be informed of the possible level of insurance cover prior to admission. If the client refuses the choice of hospital, and another solution cannot be found, SwissAssist Plus is not obliged to transport the cardholder back. SwissAssist Plus has the right to refuse transport for medical reasons, legal, political, social or events beyond our control. This includes misuse of drugs, alcohol, suicide or attempted suicide, participation in actions involving risks, where the individual is Knowly exposing himself/herself to danger. This includes commitment to crime or offence or the intention to do so. During the flight, the patient will always be accompanied by a specialist medical team trained in critical care air transport, depending on the pathology of the cardholder. The treatment of the patient during air transport is at the discretion of the treating doctor. The patient may be accompanied with a family member at the discretion of the medical crew. The repatriation of all the passengers (patient and family members) must be within the norms for immigration and custom regulations, including visa regulations. The cardholder has the right of a maximum of one flight per year. After a refused repatriation by Swiss Assist plus the cardholder will no more be able to benefit from a repatriation flight free of charge for the actual membership period. 3

For deceased persons outside their home country: In the case of a deceased person outside their country of residence, SwissAssist Plus will pay for the transport and return journey of the deceased SwissAssist cardholder back to their domicile. This will include: The preparation of the deceased for transport according to religious beliefs and legal requirements of transport. Transport of the deceased to the port or airport whilst abroad, and the transport back to the home country SwissAssist Plus will not be liable for the purchase of a coffin, flowers, and other associated items, nor for the transport of the family members. Swiss Assist Plus will help and pay a reasonable sum to arrange the documentation of transport of the deceased, but not of other family members. Swiss Assist plus has the right to refuse transport a deceased person who has been involved in political, social events. This includes misuse of drugs, alcohol, suicide, and participation in actions involving risks, where the individual has knowing exposing himself/herself to danger. This includes commitment to crime. SwissAssist Plus will need official confirmation of the country of residence in writing from the office of the registering of inhabitant s before proceeding with the arrangements of transport. The responsibility of SwissAssist Plus SA The company SwissAssist Plus, within the limitations of the regulations, will organize the complete transport of a cardholder from hospital to the airport, the flight in an ambulance plane, and the transport to an incoming hospital at the patient's domicile. SwissAssist Plus will assume responsibility for the complete escort, care, and medication of the patient during flight. SwissAssist Plus will not be able to help or be responsible for the choice or organization of any hospitalization, due to illness or accident of the cardholder while away from their domicile. The cardholder or family must choose where they intend to be hospitalized. SwissAssist Plus will decide if organize the repatriation of a cardholder, and this will be based on the diagnosis of a local qualified treating doctor. SwissAssist Plus does not take any responsible of a misdiagnosis of the local treating doctor of the cardholder prior to transport. 4

SwissAssist Plus does not take any responsibility of mistreatments performed prior to the repatriation. SwissAssist Plus is not responsible for any inconvenience that might result in a road ambulances used for the repatriation. SwissAssist Plus is not responsible for any inconvenience for unforeseeable circumstance due to events out of our control. Important notes for the cardholder. The SwissAssist Plus card can only be bought by a persons that, at the moment of purchase is NOT suffering from an acute stage of illness or has had recently an accident from which they are still in an acute stage, but the service is available for people, even with pre- existing conditions, providing they are stable and deemed fit to travel by their doctor. The service is available for women who are not more than 24 weeks pregnant. In these cases, if these conditioned are not respected, a Swiss Assist card will not be valid. SwissAssist Plus has the right to verify the reliability of the diagnosis of 3 days of hospitalization according to the Swiss medical standards. You will not be covered if you are travelling specifically for the purpose of obtaining and/or receiving any elective surgery, procedure or hospital treatment The SwissAssist plus card is personal, and is only valid to the named cardholder. It is not transferable. This fact is true for the cardholder and the one named child is registered at the time of purchase. We reserve the right for the verification of the identity of the child s cardholder. After payment of the SwissAssist plus card, there is no refund or reimbursement the sum of initial payment. Renewal of the SwissAssist plus card is NOT automatic. The Client must note when his or her card is no longer valid. At the time of purchase of the SwissAssist plus card, or renewal of the SwissAssist Plus card, the prevailing terms and conditions of the card are valid. In the case SwissAssist Plus changes the rules and regulations, these will be published in the Swiss Assist plus Internet site, without informing the client. We ask you to regularly check our Internet site (www.swissassist.ch) for updates or news. If the card holder will be not agree with the changes on the Swiss Assist s Plus GRC, he will be able to get the card s refund, if he has not used the service yet. All the terms and conditions written are in accordance to Swiss Law. The cardholder has to confirm that they have read and understood the terms and conditions. These conditions have been translated into various languages. In the case of dispute, the Italian version is the true version. 5

Important information of personnel data Your personnel and electronic data will be treated according to Swiss law (LPD of 19.6.1992) Swiss Assist Plus will nominate a person who will be responsible for guaranteeing the effective control of your personnel data according to Swiss legislation 11 LPD Our transaction is secured by Data trans, The Swiss E- payment standard in online security and PCI compliance. Your personnel data are necessary for the purchase of the SwissAssist plus card, in order to perform all the services offered by Swiss Assist Plus. The data will not given to a third party, but Swiss Assist Plus employers and collaborators in their function as case managers can use them, in order to process your personnel information. Swiss Assist Plus will be responsible to protect your personnel data.
