2 LONG TERM DISABILITY PLAN Long Term Disability Insurance Helps Protect You and Your Loved Ones No one is immune from suffering a disability. That s why it makes sense to consider Long Term Disability (LTD) Insurance, available to you through the State Bar of Texas Insurance Trust and issued by The Prudential Insurance Company of America (Prudential). The State Bar of Texas Insurance Trust (SBOTIT) LTD Plan is designed with professionals such as yourself in mind. Depending on your needs, you can select from a variety of plans, benefit amountsand waiting periods that allow you to create the combination that fits your lifestyle. Regular Occupation Protection This Plan pays total disability benefits if, due to injury or sickness, you are unable to perform the substantial and material duties of your occupation, you are not working at any job for wage or profit, and you are under the regular care of a doctor. Partial Disability Provision If you are less than age 70, you may be eligible to receive a portion of your monthly disability benefits if you become partially disabled, but continue to work a limited schedule. The percentage of disability coverage you receive is based on the reduction to your monthly earned income. Details are available in your certificate. Catastrophic Disability feature This provides an additional 20% if participants are unable to perform at least two of the Activities of Daily Living: bathing, dressing, transferring, toileting and feeding. Future Increase Option Members who apply for coverage prior to age 40 are offered an annual opportunity to obtain or increase their monthly coverage amount. Increments of $1,000, up to a maximum cumulative amount of $5,000, are available without having to provide evidence of good health. This offer is subject to amount limitations based on earnings and is subject to the overall Plan maximum. The offer expires at age 45. Survivor Benefit If you die while receiving benefits, your beneficiary will receive the lesser of three-times your monthly gross benefit or 50% of the gross monthly benefit times the number of months remaining in the unexpired portion of the maximum benefit duration at the time of death. Waiver of Premium Benefit for Total Disability No charges will be made for continued insurance coverage while you are receiving monthly benefits for total disability, starting with the first quarterly billing date after your benefits begin. Choose your Maximum Benefit from Plans I, II, or III Different Plans offer varying lengths of coverage. The maximum benefit period is the length of time you may receive benefits, depending on your age at the beginning of your disability. 3 Choose your A waiting period sometimes called an elimination period is the length of time that expires between your disability and when benefits start. With a shorter waiting period, benefits begin sooner; a longer waiting period has a lower cost of coverage. The SBOTIT LTD Plan offers a number of waiting periods, ranging from 30 to 365 days. You may choose the waiting period that offers the length of time that works best for you. Select a Benefit Amount Members under age 60 may request from $300 to $10,000, in increments of $100. Members ages may apply for up to $2,000 per month. You may also request up to $4,000 in total monthly coverage without having to provide proof of earnings. Monthly benefit amounts may not exceed 66.67% of your pre-disability earnings. Your pre-disability earnings will be calculated based on the gross income derived from your regular occupation, minus any expenses that relate specifically to your occupation. Complete the Earnings Calculation section on your Request Form to determine the amount of LTD coverage you re eligible for. Cost of Living Adjustment Provision An optional inflation protection feature for participants less than age 65 protects the buying power of your disability benefits. The feature provides an automatic annual increase of 6% in the monthly benefit amount if you are disabled for at least 12 consecutive months. You may receive up to eight annual cost of living adjustment increases. Cost of Living Adjustment Provision Quarterly Rate per $100 of coverage Age Plan I and II Plan III $ 0.36 N/A N/A N/A N/A N/A N/A N/A N/A N/A N/A Your rate will change as you enter a higher age category. Otherwise, rates may be changed on a classwide basis only. Your Quarterly Rate per $100 of Monthly Benefit Total Maximum Monthly Benefit Amount for ages 18-59: $10,000*; ages 60-69: $2,000 Plan I Plan II Plan III Ages 30* Day 90 Day 180 Day 365 Day 30* Day 90 Day 180 Day 365 Day 30* Day 90 Day 180 Day 365 Day $ 2.70 $ 2.30 $ 1.65 $ 1.60 $ 2.05 $ 1.85 $ 1.55 $ 1.45 $ 1.85 $ 1.65 $ 1.20 $ Cost will increase per the charts with your attained age. These age determinations are made on January 1 of each year. *Only a $2,000 monthly benefit amount can be allocated to the 30-day waiting period. Rates may change if Plan experience requires a change for all insureds.
4 ADDITIONAL INFORMATION Eligibility All association members under the age of 70 are eligible to apply for the State Bar of Texas Insurance Trust Long Term Disability Plan. All coverage is subject to Prudential s approval of satisfactory evidence of insurability, except for the coverage obtained through the Future Increase Option. Evidence of Insurability Many members will apply for coverage without the need for medical exams or tests. Depending on the amount of coverage applied for, and the medical history disclosed in the application, it may be necessary for Prudential to order a physical exam, a blood test, an ECG, or medical records from your physician. Termination of Coverage Your insurance will terminate if you are no longer practicing your occupation on a full-time basis, if you cease to be a member of the State Bar of Texas, if you enter military service, if you discontinue the required payments, or if the Group Insurance Contract is discontinued. Exclusions and Limitations for Total and Partial Benefits The insurance does not cover disability resulting from or contributed to by war or an act of war (including undeclared war and armed aggression); intentionally self-inflicted injury, or attempted suicide whether or not sane; or a pre-existing condition. If you receive medical treatment or advice for a sickness or injury within 12 months before your coverage starts, and you become totally disabled due to that condition within one year after being covered, benefits will not be paid for that disability or subsequent periods of disability due to the same condition. But this exclusion will not apply to a subsequent period of disability starting after you have been covered for one year. For disability due to mental illness, psychoneurotic, or personality disorders, benefits are payable for a maximum benefit period of 24 months while the insured is not hospital confined.
