1 CLASSIFICATION DESCRIPTION CARVER COUNTY SECTION I: GENERAL INFORMATION Position# Position Title: Social Worker - Masters Division / Department: Community Social Services Immediate Supervisor s Position Title: Child & Family Supervisor FLSA Status: Eempt Union Affiliation: AFSCME Job Summary: Under the direction of the Child & Family Supervisor, the Social Worker - Master provides mental health, child welfare and child protection case management services. Duties include the responsibility for implementing and providing community based interventions and services to enable children to safely obtain treatment services and support to reduce the risks of out-of-home placement; providing consultation and support to various partner agencies and schools in the early identification and intervention for children and families eperiencing emotional, behavioral, cognitive and social problems; providing assistance to partner agencies in the assessment of needs, the design and implementation of programs and interventions. The classification of Social Worker Master differs from that of Social Worker II, in that, the nature of the work requires a higher level of compleity, training, skill and knowledge level in order to appropriately carry out the requirements of the work. SECTION II: ESSENTIAL DUTIES AND RESPONSIBILITIES Duty/Responsibility No: 1 Statement of duty/responsibility: Provides case management services to children with severe and persistent emotional illnesses under the clinical direction and guidance Percent of Time: 35 of a Mental Health Professional. Develops and establishes a mutual service plan with the client family consisting of measurable goals. Provides information concerning the availability of relevant community resources. Maintains contacts with service providers. Reviews service plan progress and makes revisions to the plan as changes or circumstances occur. Follows all relevant local, state and federal requirements pertaining to the delivery of services and the documentation of case management activities. Page 1

2 Duty/Responsibility No: 2 Percent of Time: 12 Statement of duty/responsibility: Serves as a consultant advising and assisting various school(s) special education programs, teams and partners in the identification, assessment, and referral of special needs or at risk children. Consults with and advises special education team members in the identification of children and families eperiencing cognitive, emotional, behavioral or family problems. Assists school district personnel in the diagnosis and assessment of children and families and with the development and implementation of appropriate treatment plans and services to meet their needs. Provides short-term case management counseling services to families. Assists IEP team determine student s social and emotional needs and assists in the design of educational programs to address those needs. Helps develop and foster an interdisciplinary approach and intervention to meet the needs of children. Duty/Responsibility No: 3 Statement of duty/responsibility: Serves on the IEP team for assigned schools so as to coordinate and integrate and coordinate the district s IEP with the social service plan and to manage the ongoing caseload. Assists in the assessment of client needs to help determine mental health needs, parenting issues, and social skills. Provides case management activities such as securing proper authorizations for programming; initiates family based services (i.e. counseling, use of case aides, family therapy, purchased home services, placement of children in foster care or residential services). Evaluates and assesses the need for out-of-home placement; funding or need for purchased services; present placement plans to committee; develop long range plans and objectives; monitor, review and document placements; close or terminate cases. Follows all relevant local, state and federal requirements pertaining to the delivery of services and the documentation of case management activities. Duty/Responsibility No: 4 Statement of duty/responsibility: Evaluates, supervises and implements regulations, polices and procedures in all out-of-home placement situations. Evaluates the need for out-of-home placement and identifies alternatives to placement. Eplores possible placement resources. Secures approval for placement from the placement committee. Supervises the placement and support the placement facility in meeting the needs of the client. Page 2

3 Duty/Responsibility No: 5 Statement of duty/responsibility: Initiates legal processes to ensure the safety of children and provide needed services consistent with the needs and interests of the client. Informs client families of the possibility that CHIPS petition may need to be filed if children remain at risk or if plans are not followed. Consults with County Attorney regarding need for CHIPS petition. Provides case documentation to legal staff. Submits proposed plan to court to resolve risk issues. Provides written reports and plans to the court. Duty/Responsibility No: 6 Statement of duty/responsibility: Monitors, reviews, and assesses program functions, operations and activities to assure compliance with any laws, rules, guidelines or requirements pertaining to assigned program areas. Duty/Responsibility No: 7 Statement of duty/responsibility: Performs other duties of a comparable level of type. Attends seminars, training sessions, and conferences. Engages in literature reviews to keep current and abreast of changing trends, methodologies, laws or issues relevant to the work. Attends department meetings and staff meetings. Collaborates and assists other professionals in the community dealing with related issues and concerns. Duty/Responsibility No: 8 Statement of duty/responsibility: Collaborates with other social workers in the administration of the SELF program. Identifies eligible children, assists assigned staff in Percent of Time: 3 the development of independent living plan and assists children in achieving their goals. Page 3

4 SECTION III: WORK REQUIREMENTS AND CHARACTERISTICS EDUCATION/KNOWLEDGE REQUIREMENT: Minimum education required to perform adequately in position could reasonably be attained only by completing the following: REQUIRED EDUCATION/TRAINING (choose one) less than high school diploma High school diploma or GED. 1 year college 2 years college DEGREE INFORMATION: Type of degree: (B.S., M.A., etc.) Master s degree Major field of study or degree emphasis: Psychology, Counseling, Social Work, Psychiatric Nursing or related area X 3 years college 4 years college 1st year graduate level 2nd year graduate level Doctorate level Essential knowledge and specialized subject knowledge required to perform the essential functions of the job: Knowledge of the principles, practices, techniques and trends of counseling, assessment and evaluation. Knowledge f social service agency goals, objectives and organizational structure of the County Considerable knowledge of social service systems, family system dynamics, child development, attachment and separation issues, child and adolescent psychology, chemical dependency, family violence, child placement. Knowledge of relevant laws, rules, guidelines and requirements pertaining to assigned programs and services. Knowledge of school district policies and procedures, due process laws and rules, and IEP procedures and functions within districts. Knowledge of agency policies, rules, and procedures as it pertains to assigned programs areas and activities. Knowledge of community resources, providers and other resources within the community in areas of program responsibility. Required Work Eperience in Addition to Formal Education/Training: Requires one or more years of eperience working with children and families. Required Supervisory Eperience: None LICENSE/ CERTIFICATION Identify licenses/certification required: Valid driver s license or evidence of equivalent mobility. Currently licensed or license eligible as a GSW, LISW or LICSW and as a School Social Worker is highly desirable. Page 4

Consulting, advising and collaborating with families, educational personnel, legal personnel, providers in the development of plans, resources and goals for clients. 6 HAZARDOUS WORKING CONDITIONS Unusual or hazardous working conditions related to performance of duties: Employees are frequently on the road performing case management responsibilities within the County involving travel to client locations, agencies or provider locations. Employees may be eposed to poor housing/housekeeping conditions or unpleasant human interactions such as eposure to clients that are hostile, violent, angry, upset, uncooperative, or clients with chemical/emotional illnesses.
