8 HEALTH INSURANCE 101 How to understand our plan options to decide what s right for you. Plan types we offer What s the difference between HMO, POS, PPO? MyPriority HMO is our most popular health plan type. Members choose a primary doctor (family doctor) that coordinates their care. They can go to any specialist in our network without a referral from their primary doctor. MyPriority POS follows the HMO plan type with members choosing a primary doctor. They can seek care outside the network without a referral but will pay more for covered out-of-pocket expenses. MyPriority PPO does not require a primary doctor and members can choose in- or out-of-network care, but will still pay more out-of-pocket for non-network doctors and hospitals. Plan designs we offer RxPlus immediate access to office visits, including specialists before you meet your. Generic prescriptions before applies. Plus all the benefits of RxPlus, except generic prescriptions are covered after is met. Standard office visits to primary doctor and urgent care before. HSA all office visits and services apply to. When you enroll in a HSA plan you can open a savings account that is specifically used to pay health care costs. These accounts are completely tax-free, but there are contribution limits: Individual limit: $3,350 annually Family limit: $6,650 annually Tips for choosing a health insurance plan: 1 Evaluate your needs based on how often you use health services or what you anticipate you might use. Decide what you would want covered before you start paying against your : Doctors visits? Specialists visits? Prescriptions? 8
