1 Course Prospectus Elevate your practice to the next level of patient care.
2 Description Transform your ability to manage your patients medication therapy with ADAPT, an innovative, application-based, skills development program. ADAPT delivers evidence-based content from leading experts in an interactive online learning environment. Participants will practice new skills in real-world settings, supported by continuous assessment, feedback, and mentoring by peers and trained moderators. ADAPT will increase your confidence and experience in integrating new patient care skills and systematic processes and tools into your everyday practice, including: Conducting thorough medication assessments. Collaborating successfully with other health care providers. Interviewing and assessing your patients (incorporating physical assessment and laboratory data). Making evidence-based clinical decisions. Using validated documentation practices to support patient care. Developing and implementing care plans. ADAPT changes how you practice, while you practice. Learning Schedule and Time Commitment ADAPT includes 7 online modules, each lasting 2 to 4 weeks. The entire program takes place over 19 to 21 weeks (including 1 or more weeks of study break, depending on the time of year). Participants can expect to spend 5 to 6 hours per week on ADAPT activities. A learning week will open on Wednesday and close on the following Tuesday. Moderators ADAPT participants are guided through the program by online moderators. ADAPT moderators help participants translate their theoretical learning into real practice. All ADAPT moderators are specially trained in the art of online facilitation, and undergo continuous coaching and performance assessment to ensure the highest quality experience for ADAPT participants. Requirements Pharmacists must have completed the online prerequisite self-assessment An Introduction to Primary Health Care Is ADAPT Right for Me? before registering for the ADAPT online course. This self-assessment helps pharmacists determine if they have the recommended factors in place to be successful in ADAPT. APhAADAPT.com 3
3 Pharmacists must have their license to practice pharmacy in the United States before registering for ADAPT, and should be practicing in the United States while participating in ADAPT. Pharmacists should be practicing in a direct patient care setting while participating in ADAPT. Patient care settings can include a community pharmacy, a hospital, a patient-centered medical home, an ambulatory clinic, or other clinical setting. Pharmacists who designate space and time for counseling patients and have an established relationship with a prescriber will benefit the most from ADAPT. Accreditation The American Pharmacists Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. ADAPT is approved for 100 hours (10.0 CEUs) of continuing pharmacy education credit. Participants must complete a minimum of 70% of the learning activities for each module in order to obtain their CEUs for each module. Learning activities include assignments, reflections, self-assessments, and regular contributions to discussion boards. Participants can view their transcript once credit is claimed at In addition to earning CEUs, participants have the option to challenge for a Certificate in Patient Care Skills. Modules and Learning Objectives MODULE 1: Orientation to ADAPT Navigate the online learning environment using basic online features to complete the activities and communicate with other students and course moderators. Explain the underlying philosophy and learning approach of ADAPT. Review the course structure and identify how each module contributes to the overall course goals. Describe the role and expectations of the learner. Discuss how patient-centered care is acknowledged in daily practice and its influence on medication management. MODULE 2: Medication assessment List the essential information to elicit for a medication assessment. Detect drug therapy problems by applying the appropriate problem-solving framework and tools. Identify the components of a care plan. Generate the components of a medication assessment: a medication history, a drugtherapy problem list, and a follow-up/monitoring plan. Identify steps for further learning to complete medication assessments efficiently and effectively and develop a plan to address those learning needs. MODULE 3: Collaborating with physicians and other primary health care providers Develop strategies to enhance collaboration and teamwork and manage interpersonal conflict. Apply the elements of successful collaboration when working with other health care providers. Identify the roles, responsibilities, scope of practice, and expertise of other primary health care providers to integrate collaborative care for patients. Evaluate the use of communication skills to facilitate collaborative care and teamwork. Identify prescribers and other health care providers to apply new approaches for collaborative strategies. Propose steps to integrate the pharmacist with other primary health care providers on the health care team. MODULE 4: Patient interviewing and assessment (4 weeks) Explain the goals of diverse patient interview encounters within the health care setting. Use required communication skills to conduct focused and comprehensive patient interviews. Perform physical assessments and interpret relevant findings. Evaluate situations to determine when to use comprehensive or focused interviews. Determine appropriate physical assessments to obtain necessary patient data. Integrate laboratory data as part of the assessment. Apply communication and interviewing approaches to challenging patient situations. 4 MODULE 5: Making decisions about medications using evidencebased practice and understanding the challenges in clinical decision making (4 weeks) Generate focused clinical questions for therapeutic dilemmas. Answer clinical questions using current guidelines and summaries in the evidence-based medical literature. Evaluate the methodological quality of a study, describe and interpret study results, and apply results to an individual patient. Justify medication-related decisions using medical literature, patient and clinician information regarding values and preferences, and situational context and constraints. MODULE 6: Documenting medication-related care (3 weeks) Establish effective documentation practices to support patient care. Assess documentation notes for accuracy, completeness, and readability. Develop and use a standard format and strategy for effective documentation. Identify best practices and challenges to documentation and develop a plan to incorporate these best practices or address challenges in practice. MODULE 7: Putting it all together Produce a comprehensive medication assessment including a medication history, identification of drug therapy problems, and a care plan. Practice primary health care skills in patient care. Develop a learning portfolio to showcase skills and development. Create a comprehensive ADAPT Action Plan for addressing personal learning needs.
