Box Norfolk, Va The Board of Directors of Hampton Roads Pride is pleased to assist the LGBT and LGBT Friendly Community with the expenses of post-secondary education with our annual Scholarship program. Every year Community Service and General Scholarships are awarded to deserving college students throughout Virginia, and we are excited that you have decided to apply to be one of them. Scholarship Applications are due no later than June 1st, 2014 to be considered for an award. Simply complete the application using the instructions on pages 4 & 5 and return it with the required completed documentation. 4 Instructions General Instructions Complete the sections above using the instructions below as guidelines. Any section can be enlarged to fit the information provided by the applicant. Applications must be submitted by June 1 st, 2012 to be considered for an award. Application Submission Instructions: 1. Download this application to your computer. 2. Complete all fields. 3. Save a version of the application for your records. 4. Print and be sure to sign your application. 5. Mail your application to: Hampton Roads Pride P.O. Box Norfolk, VA All questions should be sent, via , to scholarship@hamptonroadspride.org Section1: Introduction Tell the Hampton Roads Pride Scholarship Review Committee about yourself. This paragraph should be less than 100 words long. This is your opportunity to give the Review Committee a chance to learn about you and why they should award you a scholarship. Section 2: Education Provide information for schools you have attended and enclose an OFFICIAL SEALED TRANSCRIPT from your most recent Institution and Semester. List any educational institutions that you attended after high school that you would like the review committee to consider. Provide high school information if you have only attended high school or are still in high school. For each institution, list the dates you started and last attended. You do not need to account for breaks in attendance. For each institution, estimate your average course load per semester in whichever units the institution used, whether that is classes, units, semester hours or something else. List your overall Grade Point Average for the entire time you attended the institution and the GPA for the last completed semester. Section 3: Community Service In this section you should list all activities that connect you to the community. Those include jobs, volunteering and participating in activities. You should explain how each of the entries ties you to the LGBT community and provide specific activities involving the LGBT community.

5 For jobs, provide your place of employment, its address, start and end dates, immediate supervisor, and describe your duties. For volunteer efforts, list the organization, number of hours you work, the duration of the work, and the type of work. You should also list fund raisers and other efforts in which you participated. You should list the organization, the type of event, the date of the event, and describe your role. List any service in the Armed Forces, including the National Guard. List the branch of the Military in which you served, your start and end dates and final status. Also list any awards, medals or honors you have received. Section 4: Educational Goals Tell the Scholarship Application Review Committee what you plan to do with your education and how that will help the LGBT community. You should list both your educational aspirations and career aspirations. Section 5: References Please attach two letters of reference from community leaders, teachers, supervisors, friends or coworkers. The author of the letter of reference should describe him or herself, and then briefly describe his or her relationship to the scholarship applicant, stating the length and nature of the relationship. The author should explain why they think the applicant deserves this award. Section 6: Agreement Carefully read this section before completing the form. The signature, date and contact information are required fields.
