2 September 2012, Dear Friends and Colleagues, We are happy to present the results of Caring for Colorado s 2012 grantmaking in this report. This fiscal year we awarded over $5.8 million in grants to 124 non-profit and governmental entities across Colorado. All of these awards had the same theme: to promote the health of Coloradans. Since the foundation was formed in 1999, Caring for Colorado has provided over $73 million dollars to Colorado communities to further our vision of making Colorado the healthiest state in the nation. Highlights from the past year include: The conclusion of Caring for Colorado s 10-year, $10 million Oral Health Improvement Project that worked to: strengthen Colorado s oral health safety net, increase and improve prevention programs for children and support policies that improve access to care for underserved populations. An evaluation report on this special initiative will be available on the Caring for Colorado website at the end of this year. The conclusion of Advancing Colorado s Mental Health Care, a 5-year, $4.5 million joint effort among Caring for Colorado, The Colorado Health Foundation, The Colorado Trust and The Denver Foundation to support mental health system improvements in communities across Colorado. The results of this project, as well as the publication, The Status of Behavioral Health Care in Colorado, can be found at: The launch of a new Caring for Colorado Safety Net Initiative designed to strengthen the capacity of Colorado s safety net clinics, and thereby communities, to provide high quality primary care to underserved populations. The primary objectives of this project are to support safety net clinics in building strategic, financial and administrative resources; explore and implement innovative plans to support continued access to care and build expertise through technical assistance. A funding opportunity was released in summer 2012 and grantee partners will be selected in fall The participation in the Partners Investing in Nursing Project, working to strengthen the home healthcare workforce by developing innovative, replicable tools and strategies to increase the number and competencies of nurses providing home healthcare. This project is a partnership among

3 Caring for Colorado, The Colorado Health Foundation, The Colorado Trust, The Robert Wood Johnson Foundation and the Colorado Center for Nursing Excellence. The incubation, within Caring for Colorado, of a new program: The Colorado Partnership for Children s Oral Health (recently renamed Cavity Free at Three). This project, funded by Caring for Colorado, The Colorado Health Foundation, The Colorado Trust, Delta Dental Foundation of Colorado, Kaiser Permanente and Rose Community Foundation, spent a year conducting research and planning for a statewide effort to increase private dentists participation in the Medicaid program and to increase the number of children in our state who have a dental home by age one. The project was given to the Colorado Area Health Education Center Program to implement in June of It will be a three year effort to improve children s oral health outcomes in Colorado. This new system will simplify the grant application process for potential grantees as well as improve operational efficiencies for the Foundation. The launch of our new website that is designed to provide better information about the Foundation s grantmaking priorities, results, and instructions for how to seek funding. We said thank you and farewell to two Board members: Sue Birch from Steamboat Springs and Cindy Stevenson from Boulder. 4 Caring for Colorado s four main funding priority areas are: Build health care capacity with the goal of improving access to care for underserved populations and people living in rural areas of the state. Strengthen systems of care so that people can receive comprehensive, high quality health care. Link people to care in order to overcome the barriers presented by geography, culture, income, language and system fragmentation.
