2 Salon Gold Individual Personal Accident and/or Sickness Insurance Policy Summary Policy Summary This is a summary of the cover provided by your policy and does not contain the full terms and conditions of the cover, which can be found in the policy document. It is important that you read the policy document when you receive it. Insurance Provider This insurance is provided by Salon Gold (Henry Seymour & Co) and the covers are underwritten by certain underwriters at Lloyd s. Key Covers, Features and Exceptions Your policy includes the following key covers, features and exceptions, which are detailed in your policy documentation. Type of Insurance The Policy is a personal accident and sickness insurance contract that provides cover for persons under the age of 70 years. The sections that apply to your insurance will be stated in the Schedule. Policy Term The standard duration of the contract is 12 months from the date on which cover incepts. Any variations to this duration will be shown in the schedule. If your period of insurance is more than one year you may need to review and update this cover periodically to ensure it remains adequate.

3 Individual Personal Accident and/or Sickness Insurance Policy Personal Accident Section page 15 Cover, Features and Benefits Cover includes Accidental death Loss of limbs and sight Permanent Total Disablement (PTD) Temporary Total Disablement (TTD) or Temporary Partial Disablement (TPD) Medical Expenses lump sum compensation lump sum compensation following injury lump sum compensation following an injury as defined in the schedule weekly income as a result of an injury which temporarily prevents an Insured Person from carrying out the whole of their occupational duties (TTD) or part of their occupational duties (TPD). paid in additional to the above benefits Significant Exceptions and Limitations - Please refer to your policy document Permanent Total Disablement (PTD) Temporary Total Disablement (TTD) or Temporary Partial Disablement (TPD) Medical Expenses Where you are not in full time employment, PTD is defined as any and every occupation The benefit period and deferment period is stated in the schedule Maximum of 20% of the TTD sum insured Sickness Section Page 15 Loss of Sight Permanent Total Disablement by Paralysis Temporary Total Disablement (TTD) or Temporary Partial Disablement (TPD) Medical Expenses Lump sum compensation following an illness Lump sum compensation following an illness. Weekly income as a result of an illness which temporarily prevents an Insured Person from carrying out the whole of their occupational duties (TTD) or part of their occupational duties (TPD). Paid in additional to the above benefits. Significant Exceptions and Limitations - Please refer to your policy document Permanent Total Disablement by Paralysis Temporary Total Disablement (TTD) or Temporary Partial Disablement (TPD) Medical Expenses Excluding alcohol abuse or addiction, HIV or AIDS or ARC and preexisting medical conditions 24 months prior to the effective date until 24 months has elapsed with no medical treatment for that condition The benefit period and deferment period is stated in the schedule. Maximum of 20% of the TTD sum insured. Permanent Disability (Continental Scale) Page 16 Lump sum based on set percentages dependent on the degree of disablement. Rehabilitation Expenses benefit Page 16

4 For a (PTD) loss an additional 500 per month is payable for tuition, advice or treatment from a licensed vocational Payable for a maximum of 6 months Hospital and Convalescence Benefit Page 17 If hospitalised from an injury a daily allowance is payable Hospitalisation is payable for 180 days. Convalescence following 4 nights in hospital up to 4 weeks. Fractures benefit Page 16 Lump sum compensation following an injury Restricted to fractures of the skull (excluding nose and teeth), shoulder, arm or leg. Osteoporosis is excluded Burns Benefit Page 18 Up to a maximum of 5,000 The lump sum follows a benefit scale shown in the policy Facial Scarring Benefit Page 18 Up to a maximum of 1,000 The lump sum follows a benefit scale shown in the policy. Optical Injury Benefit Page 18 Cost of repairing or replacing spectacles or contact lenses or prescribed following eye damage, up to 150 and 25 for an eye test The Insured Person must be admitted as an in-patient to hospital following the optical injury Clothing and Personal Effects Benefit Page 18 Clothing and/or Personal Effects lost, stolen, damaged or destroyed A single item limit of 250 applies; Excluding sports equipment in use, valuables stolen when attended unless locked and out of sight in a motor vehicle or securely locked in a building Care first Counselling and Information Service Page 20 Helping managers and staff resolve situations that may impact on their work performance. Provides managers with consultancy that enables them to deal with a wide range of people problems including bullying, harassment, absenteeism, grievance and cultural. Assist proactive management of stress at work by telephone counselling. Counselling critical and traumatic incidents. Telephone: +44 (0)

5 General Exclusions that apply to all Sections Page 13 War and Terrorism Flying other than as a passenger Pregnancy within 2 months of the estimated date of delivery, or childbirth Nuclear or radioactive contamination Weapons of mass destruction Professional sports Armed Forces operational duties Illness where you have received or required medical or psychiatric treatment or counselling in 24 months prior to the effective date until 24 months have elapsed where you have not received nor required any medical or psychiatric treatment or counselling for the said illness HIV and AIDS or ARC Age limit 70 years Conditions With regard to the information that You provide to us when applying for Your policy, You are confirming that You have answered the questions to the best of Your knowledge and belief. It is very important that You check that it is complete and accurate. If it isn t, please contact Us immediately and We will arrange for correction and reissue of the documentation. If any information on the document is not complete and accurate: we may cancel Your policy and refuse to pay any claim, or we may not pay any claim in full, or we may revise the premium and/or change any excess, or the extent of the cover may be affected

6 The parties are free to choose the law applicable to the insurance contract. Unless specifically agreed to the contrary this shall be subject to English Law. Cancellation page 11 Only you and us may cancel the policy or any cover. We will give 30 days written notice and you will give 30 days written notice. Any unearned portion of the premium paid will be returned, subject to a minimum retention of one half of the annual premium or 150, whichever is the lesser.
