Be currently enrolled in the college or have an application submitted to the admission office. 2. Have official transcripts from all colleges sent to CSCC so transfer evaluations may be completed. 3. Complete all required developmental classes. 4. Complete chemistry with a grade of C or better. 5. Maintain a GPA of 2.75 or better on nursing academics (must have grades of C or better). 6. Pass HESI admission test: Reading Comprehension 75%, Cumulative/Overall 75% Once the above requirements have been met, submit this completed application along with: 1. Copy of HESI admission test score. 2. An unofficial CSCC transcript showing courses completed along with any transfer courses from other colleges. 3. An unofficial transcript showing a chemistry grade (high school or college). Only completed applications will be accepted. Incomplete applications will not be processed. Completed applications must be submitted by September 1, If you have ever had a nursing or health-related license suspended, revoked, denied, or subjected to disciplinary action or if you have ever been convicted of a criminal offense other than a minor traffic violation, The Tennessee State Board of Nursing has the authority to deny a license based on criminal convictions. 3 Cleveland State Community College Steps Toward Acceptance We are excited you are interested in the nursing program, and we hope this information is helpful. ** Complete an application and be accepted to Cleveland State Community College. Provide all official transcripts to the college. ** Establish eligibility for admission by completing the following prerequisites: 1. All developmental courses as required by the placement test. 2. A high school or college chemistry course with grade of "C" or better (it is your responsibility to provide a transcript to the Nursing Department showing a chemistry grade). 3. Students must take the HESI Test and score 75% on reading and 75% on the overall/composite. The HESI test will be available beginning Fall A student may be eligible to enter the program with a score below 75% on the HESI Test if they have achieved a 3.0 GPA in all three required science courses (BIOL2010/2011, BIOL 2020/2021, BIOL 2230/2231). A student may repeat the science courses only one time. ** Complete a Nursing Program application for Evening Part-time Option and return it to the Nursing Office after August 8, Make sure that all items are attached to the application or your application will not be accepted. Remember you must be eligible for admission by September 1, (You must establish eligibility by the end of Summer Semester 2011, since grades for courses taken Fall Semester 2011 will not be available until after the nursing class of Spring 2012 has been admitted.) Cleveland State students are given admission priority over transfer students. ** Begin required nursing academics. ** Establish and maintain a 2.75 or greater GPA in required nursing academics. (This is NOT a cumulative GPA.) NOTE: High school students attempting to enter the Nursing Program should consult the college catalog for additional information. Please make sure that your high school transcript shows your chemistry grade, cumulative GPA, and your ACT scores. REMEMBER: You must maintain a 2.75 or greater GPA in the required nursing academics to maintain your eligibility status. The developmental courses and the chemistry course are prerequisite; therefore, these courses DO NOT count in the required 2.75 GPA. An applicant scoring tool is utilized to admit eligible applicants when the number of eligible applicants exceeds the number of students that can be admitted.

4 Required Nursing Academics (According to the current catalog, the courses listed below are the courses that will be considered when calculating an applicant s GPA. To be eligible the GPA must be 2.75 or higher.) GENERAL PSYCHOLOGY (PSY 1010) HUMAN ANATOMY & PHYSIOLOGY I + Lab (BIOL 2010, 2011) HUMAN ANATOMY & PHYSIOLOGY II + Lab (BIOL 2020, 2021) HUMAN GROWTH & DEVELOPMENT (PSY 2110) COMPOSITION I (ENGL 1010) COMPOSITION II (ENGL 1020) OR PUBLIC SPEAKING (SPE 2310) HUMANITIES and/or FINE ARTS ELECTIVE (Listed in the online college catalog. Please go to Academics Information, then General Education Requirements, scroll down to Humanities/Fine Art, there are only 13 courses in this category.) MICROBIOLOGY + Lab (BIOL 2230, 2231) ABNORMAL PSYCHOLOGY (PSY 2310) Remember: To be eligible for admission to the Nursing Program, you must ** Complete the prerequisite chemistry, developmental courses, and the HESI Test. ** Have a "C" or better in the prerequisite chemistry, any required developmental courses and all nursing academics. ** Score 75% on the reading section and the overall/composite of the HESI Test. A student may eligible to enter the program with a score below 75% on the HESI Test if they have achieved a 3.0 GPA in all three required science courses (BIOL 2010/2011, BIOL 2020/2021, BIOL 2230/2231). ** Achieve and maintain a 2.75 or greater GPA in required nursing academics. ** If you have already completed required science courses (Human Anatomy & Physiology I and II, and Microbiology) these must be within 5 years of the date of admission. Note that D's and/or F's in required academics result in ineligibility. Student making multiple D's and/or F s should re-evaluate their professional goals.

5 Nursing Faculty Office Hours When you make application to the college with pre-nursing as your degree major, you will be assigned a nursing faculty advisor. The nursing faculty offices have name plates on the office doors and are located on the 2nd floor of the Career Education Building. Nursing faculty members have office hours posted on the doors to their offices during the academic school year (Mid-August through April). Appointments may be made on an individual basis. It is important to note the times your advisor will be available (days & times). The nursing faculty may be in class, skills labs, or making assignments at the clinical facilities. The nursing faculty are typically involved in clinical instruction two days during the week and, thus, they may not be on campus every day. (Example, the freshman faculty are in the clinical facilities on Thursdays and Fridays, while the sophomore faculty are in clinical on Tuesdays and Wednesdays). Remember the days and hours faculty are scheduled to be in their office will be posted on their office door during the academic school year. If a faculty member is not readily available, please call or to make an appointment. All faculty members have voice mail. Please leave a message so the faculty member can return your call. You may call the college s main number , then the faculty member s phone extension. Please remember that the nursing faculty members are typically in the clinical facilities two days a week and do not hold office hours on those days. Therefore, it may take some students more than five semesters to complete their program of study. In addition to traveling to class, students are required to attend clinicals. Distances to clinicals vary because they are held in different locations, and the students rotate every three to six weeks. 8 How to Calculate Nursing GPA You will not be eligible and your application for admission to the Nursing Program will not be considered unless all required items are attached to your nursing application. You must also maintain a 2.75 or greater GPA in the required nursing academics to maintain your eligibility status. To be eligible for admission to the Nursing Program, you must complete required science courses (Human Anatomy & Physiology I and II, and Microbiology) within 5 years of the date of admission (For example: Spring 2012, the date would be 2007). Results of HESI test Passing scores of 75% - reading comprehension, 75% - overall/cumulative 2. Unofficial transcript (high school or college) showing chemistry grade of a C or better. 3. Unofficial student transcript from Cleveland State 4. If you have transferred courses from another college, make sure that these are on the unofficial student transcript from Cleveland State Reminder: The deadline for nursing applications is September 1, 2011 for the Spring 2012 Evening Part-time Option. The Admissions Committee will begin reviewing completed applications after September 1, It is your responsibility to make sure that all the courses you wish the Admissions Committee to review are listed on your transcript.

11 Course Descriptions NUR 1200 Nursing Fundamentals 4 credits This course is the first in a series of six. Care of the adult with emphasis on basic concepts and skills. Emphasis will be on professional practice, patient centered care, nursing process, and physical assessment. Two hours lecture and two hours lab/clinical per week. (Prerequisite: Admission to Nursing Program - Evening Sequence.) S NUR 1210 Nursing Pharmacology 2 credits Emphasis will be on an introduction to pharmacology, medication calculation, medication administration, documentation and informatics. One hour lecture and one hour lab/clinical per week. (Corequisite: NUR 1200.) S NUR 1220 Medical Surgical Nursing I 6 credits This course is the second in a series of six. Emphasis will be on eye, ear, nose and throat alternations, respiratory, fluid and electrolyte, gastrointestinal, and genitourinary alternations. An introduction to perioperative nursing in also included. Two hours lecture and four hours lab/clinical per week. (Prerequisites: NUR 1200 and 1210.) M NUR 1230 Maternal Child Nursing 6 credits This course is the third in a series of six. This course will provide an emphasis on antepartal, intrapartum and postpartum care for the mother and neonate, and growth and development principles. Emphasis on pediatric patient will include EENT, respiratory, musculoskeletal, renal, neurological, cardiovascular, congenital and developmental disabilities. Two hours lecture and four hours lab/clinical per week. (Prerequisite: NUR 1220.) F NUR 2200 Medical Surgical Nursing II 6 credits This course is the fourth in a series of six. This course continues study begun in Medical Surgical Nursing I with an emphasis on cardiovascular alterations, neurological alterations, endocrine alterations, oncology and musculoskeletal alterations. Two hours lecture and four hours lab/clinical per week. (Prerequisite: NUR 1230.) S NUR 2210 Psychiatric/Geriatric Nursing 4 credits This course is the fifth in a series of six. This course will have emphasis in nursing care for patients experiencing alterations in mental health. Aspects of caring for geriatric patients will also be included. Two hours lecture and two hours lab/clinical per week. (Prerequisite: NUR 2200.) M NUR 2220 Nursing Management 8 credits This course is the last in a series of six. This course will place emphasis on managing client care, delegation, stress/time management, quality improvement, collaboration and legal/ethical principles.
