1 HEALTHIER TOGETHER FREQUENTLY ASKED QUESTIONS (FAQs) Medtronic encourages all employees to live healthier and happier lives every day. One of the ways we encourage your overall wellness is through Healthier Together, our global well-being program, which supports your physical, emotional, social, and financial health. With the program, you ll find it even easier to make healthy decisions like moving more, eating well, getting enough sleep, and finding time for relaxation. All Medtronic employees, as well as spouses who are covered by the Medtronic Medical Plan in 2018, are eligible to participate in Healthier Together and earn points to avoid paying more for medical coverage in ENROLLING IN HEALTHIER TOGETHER 1. Who can participate? Healthier Together is available at no cost to all employees, as well as spouses who are covered by the Medtronic Medical Plan in 2018.Spouses not covered by the Medtronic Medical Plan are eligible to participate in the Wellness Screening and Onsite Flu Vaccination programs. 2. How do I enroll? Enroll in Healthier Together by visiting healthiertogether.medtronic.com. Log on to the Healthier Together site, and click the Employee Sign Up link (use your Medtronic username and password). You will complete a brief registration and then be directed to the Healthier Together platform homepage where you can access wellness activities and program details. 3. How does my covered spouse enroll? Your covered spouse can enroll in Healthier Together by visiting healthiertogether.medtronic.com, clicking Spouse Sign Up and following the instructions provided. 4. If I registered in Healthier Together in 2017, do I have to re-register for 2018? No, if you registered for Healthier Together in 2017, you do not need to register again in Do I have to participate? You do not have to participate in the Healthier Together program. However, keep in mind that you will pay more for medical coverage in 2019 if you (and/or your covered spouse) choose not to participate. You ll also miss out on the opportunity to earn Recognize! points.
2 6. My spouse is covered by the Medtronic Medical Plan but won t participate. Now what? We all play a role in the cost of healthcare coverage. At Medtronic, we ask that you take an active role in reducing our healthcare costs and investing in your well-being by participating in the Healthier Together program. And, because covered spouses contribute greatly to our healthcare costs, they are also encouraged to take steps towards a healthier lifestyle. Employees and spouses covered by the Medtronic Medical Plan need to complete Level 2 in the program by Dec. 23, 2018 to avoid paying $50 per month more for medical coverage in What if I am hired later in the calendar year and don t have time to earn the points needed to avoid paying more for my medical coverage for 2019? If you are hired on or after Sept. 1, 2018 you will not be required to reach Level 2 to avoid paying more for medical coverage in However, you are encouraged to register and start participating in the program you ll have the opportunity to earn Recognize! points in Why do I have to enter personal information (name, DOB, gender) during the registration process? The information on the registration form is needed to validate legitimate Medtronic employees during the registration process and to create a personalized experience within the portal for each member. The Medtronic and Virgin Pulse legal and security teams worked closely during our vendor assessment process. As part of that process, the security and privacy frameworks of the Virgin Pulse platform have been diligently reviewed and are in line with the standards required by Medtronic to ensure the privacy of Medtronic employees is appropriately protected. REWARDS PROGRAM 1. What are Recognize! points and how do I earn them? Recognize! is the Medtronic employee recognition program. You can earn points for completing Healthier Together wellness program activities. As you complete each level within Healthier Together, you will receive Recognize! points in your account. 2. How many Recognize! points can I earn in 2018? You can earn up to 50 Recognize! points, per year, and, when your covered spouse participates, they can help you earn up to an additional 25 Recognize! points per year. Points will be deposited into your (the employee s) Recognize! account. You can choose to use points to purchase a variety of retail items or gift cards.
3 3. What are the program levels and the point levels associated with each? To achieve each level within Healthier Together, you earn reward points by completing different activities such as tracking your steps, sleep or eating habits, receiving preventive screenings, joining company-sponsored challenges, or engaging in learning opportunities. You ll start earning points just by registering! Once you earn enough points to achieve a level, you ll also earn Recognize! points. The chart below shows the reward points needed to achieve each level, as well as the Recognize! points awarded at each level. REWARD LEVEL REWARD POINTS RECOGNIZE! POINTS Level I 3, Level 2 15, Level 2 Achievement = Avoid Paying More for 2019 Medical Coverage Level 3 35, Level 4 50, Covered spouses can help you earn an additional 25 Recognize! points per year (by completing Level 2) and must do so in order to avoid paying more for 2019 medical coverage. 4. How and when can I redeem Recognize! points? Once points have been deposited, you can visit recognize.medtronic.com to shop for merchandise. Points are deposited weekly. 5. What types of activities can I do to earn points and complete each level? There are many ways that you can earn points through Healthier Together, and it s up to you to decide what works best with your wellness goals. If you have a fitness tracker, many activities can automatically be recorded to your account, which makes it both easy and convenient to earn points. Below are a few sample activities that you can participate in to earn points and complete each level. ACTIVITY POINTS Take 1,000 steps in a day 10 Take 5,000 steps in a day 50 Take 7,000 steps in a day or more active minutes in a day 100 Take 7,000 steps 20 days in a month 400 Take 10,000 steps 20 days in a month 500
4 SLEEP Sleep greater than 7 hours 50 Track sleep 10 days in a month 100 Track sleep 20 days in a month 200 CHALLENGES Create a personal challenge 50 Join a personal challenge 100 Join a company challenge 1,000 WELLNESS Participate in a wellness screening 2,000 Have a preventive care visit 1,000 Speak with a financial advisor, Medtronic 300 EAP, or legal professional GENERAL ENGAGEMENT Register 1,000 Connect a personal activity device 150 Upload a profile picture What is the deadline for earning points and achieving Level 2 status to avoid paying more for medical coverage in 2019? To avoid paying $50 more per month for medical coverage in 2019, you and your covered spouse must each complete Level 2 by Dec. 23, PROGRAM INFORMATION 1. Who administers the Healthier Together website? Medtronic partners with Virgin Pulse, an external party who specializes in health and wellness programs, and delivers expert, confidential administration and support to participants. 2. Where do I download the mobile app? You can download the Virgin Pulse mobile app for ios or Android. You will be asked to enter a username and password for the app during registration. Once registered, you can stay connected on the go. Plus, the first time you log in, you ll earn bonus points. 3. How do I unsubscribe from social Virgin Pulse s such as Friend requests? To unsubscribe, follow these steps.
5 4. How do I track my activities? Visit the tracking and rewards tabs to begin tracking your activities and building healthy habits. Your tracking can be personalized to reach your wellness goals, and you can earn points by completing a wide variety of activities on the site or through the mobile app. You can also download the mobile app and sync your fitness tracking device to track activities and earn points on-the-go. You can track activities that support these areas of well-being: Getting Active, Eating Healthy, Sleeping Well, Reducing Stress, Being Productive, Learning New Things, Contributing to My Community, and Managing My Finances. Visit the rewards page to see point values and how they are tracked. Some activities involve self-reporting, which you can do by completing an online form, while others are automatically tracked. In addition, you may receive vouchers to redeem for points for some onsite wellness events. 5. How do I track for past activity? To track data for previous days, click on the arrows to toggle between the dates at the top of the Healthy Habits column of activities. You can retroactively track activity up to two weeks. 6. What if I don t have a fitness tracking device? You will be able to purchase discounted tracking devices through Healthier Together. Our partner, Virgin Pulse, has relationships with Fitbit and other vendors who provide discounts, as well as their own Max Buzz fitness tracking device at a low cost. You can also use your Recognize! points to purchase fitness tracking devices. Activity trackers are not required to participate in the Healthier Together program, but are one of the convenient tracking methods. 7. I do not have access to a computer. How can I participate? The mobile app provides the same experience as the website, allowing those on-the-go or with limited computer access a convenient way to engage and participate in the program.
6 Many points are also loaded automatically, such as preventive flu vaccinations, wellness screening participation, health club frequent fitness program, and syncing a fitness tracking device. An example of how to reach Level 2 (15,000) points with minimal computer access: Activity Max. To find out if your external gym is a participating club, contact your medical plan administrator. 9. Do I need to do physical activity to earn points? No, there are many ways to earn points, and complete the levels, without completing physical activity. Check out the reward page to learn about of the different ways you can earn points. There are multiple options such as eating healthy, volunteering in your community, getting more sleep, and taking time to relax. 10. Is my personal health data kept private? Will other people see my progress? Your health data and progress are kept entirely private. All data input into the platform is protected by law and 100% confidential. Medtronic only receives aggregate population health data, but none of this information is attributable to or can be used to identify any one individual.
7 CONTACT INFORMATION 1. Who should I contact if I have specific questions about the Healthier Together well-being program? If you have questions about the Healthier Together website, registration or log-in issues, or status changes, contact the Healthier Together participant support line at (844) or support@medtronichealthiertogether.com. You can also click the Support or Chat tabs on the website to view hundreds of FAQ articles or chat with live support. To review the wellness program in detail, register and begin participating, go to healthiertogether.medtronic.com.
