Consultation fees at appointed service providers. 2. Diagnostic laboratory and radiology services 3. Prescribed Physiotheraphy 4. Prescribed drugs and dressing 5. Outpatient cover for chronic conditions and HIV/ AIDS subject to two years waiting period. 6. Outpatient prescription subject to a sublimit of Ksh. 50,000 per person per annum. 7. All outpatient claims are subject to Ksh. 1,000 copayment. Outpatient Rates 150, , , , , , , , , , , ,000 General Conditions 1. All illness claim have a 28 days waiting period. 2. Non accidental surgical claims have a 60 days waiting period. 3. Admission by doctors not in the panel is subject to 20% co-payment. 4. Joining age 65 to 80 years. 5. Cover on per person basis. 6. Every member must undergo a medical Checkup and submit report to UAP. 7. Cover subject to written acceptance by UAP Insurance after review of medical report. 8. If dependants are below 65 years they will be covered under the AfyaImara, AfyaImara County or AfyaImara Junior. 9. Referrals shall be to India and South Africa Only. 10. Road evacuation shall be provided for under the inpatient cover. 11. Air evacuation shall be provided for persons whose cover limit is 1 million and above. 12. Medical bill incurred at a non-appointed facility. 19. Alcoholism, drug abuse and related ailments. 20. Treatment outside the approved and or recommended service provider. 21. Dialysis for management of a long term ailment. How do i sign up? 1. Obtain the specified medical examination report form an approved provider. 2. Submit the medical report above together with the proposal form and premium cheque to UAP. 3. UAP shall advise you whether your cover has been accepted in writing. 4.
