1 Fisher Family Practice Teaching Plan 1. Scope of Clinical Practice: a. A descriptive profile of patients attending the practice Fisher Family Practice is in the suburb of Fisher on the Southside of Canberra and is a popular Practice with a diverse patient base. The suburb of Fisher is classified as Outer Metropolitan, and has an Area of Need AON) and District of Workforce Shortage (DWS) rating. The Practice has equal representation of all age groups, ensuring an exposure to children, their parents, and a significant aged population. We have a small group of indigenous patients who attend for the majority of their health care. The practice has a policy of mixed billing, with a bulk bill rate of approximately 15-20%. The practice is currently open from 8am to 6 pm from Monday to Friday as well as Saturday mornings, and we offer home visits to those patients who have great difficulty in coming to the surgery. We also service approximately 70 Nursing Home patients, mostly by visiting them in the Nursing Home. b. Special interests and skills of individual Supervisors and others involved in teaching: Fisher Family Practice has currently 3 GP Supervisors: Dr Anne Marie Svoboda has an interest in: General Family Medicine Sexual Health Forensic Medicine Palliative Care Shared Antenatal Care Medical Education-GP Registrars and Medical Students Dr Kalum Ratnaike has an interest in: Family Medicine Women s Health Shared Antenatal Care General medicine in particular Endocrinology
2 Dr Martin Liedvogel has an interest in: Internal Medicine Minor Procedures Men s Health Practice Ownership Fisher Family Practice is committed to medical education and research. We teach GP Registrars and Medical Students, and participate in medical research projects (e.g. ASPREE trial). We also have 3 very experienced Practice Nurses who are willing to share their wealth of knowledge about: Chronic Disease Management such as diabetes Pap smears Immunisations Wound Care Preventive Aged Care c. Services Provided outside of the Practice: Fisher Family Practice provides the following services offsite: Residential Aged Care Facility visits Home Visits Supported Group Home Visits Palliative Care visits Practice Website Social Activities d. Other Teaching Opportunities accessible from the practice: National Prescribing Service education sessions Local Physiotherapist Medical Imaging Companies Palliative Care teaching ACT Medicare Local education sessions GP Grand Rounds at the Canberra Hospital Staff Meetings Doctor Meetings Specialist teaching opportunities Drug Company Representative visits
3 2. How Fisher Family Practice organises Registrar teaching a. Who teaches and when, including expectations of the Registrar. Teaching will be delivered by the GP Supervisors at Fisher Family Practice, as well as allied health and clerical staff. The times for teaching are either from 8.30am to 9.30 am or from 1.30pm to 2.30pm on either Tue, Wed, Thu, or Friday. These times are flexible, and may be rescheduled to fit in with the requirements of the Registrar and/or Supervisor. Registrars are expected to be available for teaching at the allocated time. b. Modes of teaching available. Registrar teaching will involve the following techniques: Initial orientation to the practice, local hospital, nursing home and community Direct observation of Registrar by Supervisor or vice-versa Demonstration and teaching of practical procedures Development and review of Registrar s learning plan Case discussions Video debriefing of consultations taped on an earlier occasion In depth discussion of topics raised by the Registrar/identified during case review Joint nursing home visits or house calls. These are especially useful during the first few weeks of the term Review of referrals Audit of pathology/radiology, cost effectiveness and relevance to General Practice Structured feedback using the Formative Feedback form Specialist letters Review of Critical Incidents Going through exam scenarios Teaching of Medical Students Clinical Audits Spending teaching time with local health professionals Role play of difficult, less common consultations All methods will be used for Registrar teaching. The preferred mode of teaching will depend on the progress of the Registrar. For example, Registrars that are comfortable with the consultation process will spend less time being observed, and spend more times exploring other teaching techniques. c. The role of direct observation and videotaping at Fisher Family Practice: Fisher Family Practice uses the teaching techniques of direct observation. This involves the Registrar observing the Supervisor, and the Supervisor observing the Registrar. Videotaping the consults of Registrars will also be used with the consent of the patient.
4 3. How supervision is organised and scheduled particularly where multiple supervisors are used: Fisher Family Practice allocates a primary supervisor for every Registrar. This Supervisor is responsible for the overall supervision of the Registrar. Teaching time is protected time during the session, ensuring that teaching requirements are met. The Registrar is asked to contact their primary GP Supervisor for advice whenever possible, to ensure that learning needs for the Registrar can be identified early and accurately. From time to time Registrar teaching will be allocated to another supervisor while the primary Supervisor is unavailable. The practice will endeavour to always have a supervisor available for advice on location or by phone as appropriate to the level of training of the registrar. 4. How assessment is organised and undertaken at Fisher Family Practice: Registrar assessment is an ongoing process at Fisher Family Practice, and occurs each time the Supervisor seeks and gives constructive feedback after every teaching session. Assessment also involves monitoring of how the Registrar will complete the goals identified in their learning plan, and by monitoring entries into the learning and procedure log. Your GP Supervisor will also take into account patient and staff feedback when assessing your progress. ECT visits are an important milestone in monitoring Registrar progress, and the medical educator will provide constructive feedback to the Registrar and their Supervisor. Finally, the primary Supervisor will do a formative assessment towards the middle of the term. This assessment will provide an opportunity to adjust the learning plan and teaching sessions to allow the Registrar to develop to their full potential. 5. Information resources available in the practice for Registrars: Fisher Family Practice has a wide variety of medical information that Registrars can access online, electronically on the network, and in hardcopy. Online and network access is available in each consultation room. a. Within the medical software package Fisher Family Practice uses Medical Director and Pracsoft Software to manage medical records and patient billing. The software has a comprehensive database of local specialists and allied health professional that can be searched for either alphabetically or by specialty of practice. Medical Director also has an inbuilt database that can be searched by using keywords. This database will link to multiple online resources, including Murtagh.
5 b. Online ACT HealthNet. An online gateway to therapeutic guidelines, textbooks, Cochrane library, Medical Journals, PubMed, RCPA manual, MIMS, Australian Medicines Handbook, Australian Resuscitation Council, Health Insite, and up to date ACT Health outpatient services information. Coast City Country GP Training GP Start. This section of the Regional Training Provider Coast City Country provides online modules covering key areas of training for GP Registrars. Each module provides the Registrar with relevant reading and case studies to enhance the learning experience. c. Electronically via computer network Hundreds of current Clinical Management guidelines covering a wide range of clinical areas are accessible on the practice computer network. Examples include: Fit to Drive, NHF Hypertension Guidelines, RACGP Diabetes Management Guidelines, NHF heart failure guidlines, and Chronic Kidney Disease guidelines. d. Hardcopy Registrars have access to the library of each supervisor. Books include fracture management, dermatology, Paediatrics, internal medicine, and more. 6. Other CPD and educational opportunities in the area: ACT Medicare Local provides regular education evenings that are generally eligible for CPD points. These sessions include practical skills such as Implanon insertion. Registrars have internet access to allow them to participate in online educational activities such as GPLearning and the telecast of the GP Grand Rounds. Australian National University and University of Canberra offer a variety of post graduate training courses. Registrars are asked to complete one NPS audit per term. 7. Opportunities for clinical and community leadership participation: Registrars have the opportunity to participate in leadership roles if they wish. Opportunities include: Registrar Liaison Officer with Coast City Country GP Training GP Registrar Australia Board Membership RACGP Board Membership RACGP standing Committee Membership AMA Doctors in Training committee membership Intern, RMO, and medical student teaching Canberra Medical Association participation
