Creating a robust network to connect physicians, hospitals and others involved in a patient s care with one another Managing that network, so physicians and hospitals, clinicians and labs can all talk to one another.

9 Why do we need an HIE? An HIE moves patient information electronically among physician offices, hospitals and other health professionals directly involved in a patient s care, such as pharmacies and labs. 9

13 What s CliniSync? CliniSync is a health information exchange in Ohio created by the Ohio Health Information Partnership. CliniSync makes it easy for healthcare providers to coordinate care for patients who have seen other clinicians in different settings. CliniSync synchronizes clinical records and lets the treating provider choose which data is shown and integrated. CliniSync is your platform for innovation!

18 Referral Management Referral management is a customizable feature allowing you to send and receive patient referrals electronically with confirmation and tracking in real time. Referrals move swiftly from one doc to another.
