2 About Ivy Homes INTRODUCTION Ivy Homes is a private care provider and owned by Mr. John, Mr. Paul and Mrs. Carol Taylor who have extensive knowledge and experience in care home provisions. We have a diverse and multi skilled management team who individually specialise in both care management and business management. The organisation consists of four individual care homes which are located in both South and West Yorkshire, all of which are registered with CQC as care homes for adults with learning disabilities. The organisation is overseen by the General Manager who has extensive experience in the field of learning disabilities and care home inspection. We have a Senior Manager who is responsible for overseeing all of the four services and the individual Service Managers who manage these services. Each service (home) has its own Service Manager and Deputy Manager, who ensure the service provisions are to the highest standards and the day to day running of the service is consistent with the requirements of our registering body. All of our services have individual Activity Co-ordinators who work closely with each individual service user and PCP Facilitators to devise plans to support the service user to achieve their own personal goals. Each service has a Senior support staff member who supports and guides the care staff teams. The delivery of support to service users is 24 hours a day, 365 days per year to support and assist service users with their individual needs and requirements. Ivy Dene Ivy Dene is located in a small village situated on the outskirts of Wakefield.

3 Aims & Objectives WE AIM TO: Act in recognition of each person s rights, needs and dignity and to respect individual decisions and autonomy. Support individuals well being, health and development to enable the highest possible quality and experience of life. Create a stimulating environment and atmosphere which promotes positive and stable personal relationships. Provide individualised care and opportunities using a systematic approach of assessment, planning, implementation, evaluation and personal appraisal. Create a safe and secure environment within which appropriate risk-taking is properly planned and managed. Utilise research findings and examples of leading edge practice elsewhere, relating theory to service provision in key areas such as integration, user empowerment, advocacy, and anti-discrimination practice. Critically appraise the service provided, evaluate success and failure and modify practice accordingly. Enable individuals to present a valued social image to the community and to exercise their rights and responsibilities in all interactions. Actively promote the philosophy of equal opportunities and rights for all people. By respecting individuals rights choices, independence, privacy and fulfilment through listening, observing and recognition. Maximise the control individual s have about the service they receive through involvement at all times about changes and options in their lives, including agreed person centered care packages. Have access to all areas of their home, except for other users bedrooms, unless specifically invited to do so. Have a key for their own bedroom. Providing advocacy services if needed and chosen. Providing the opportunity to decide on the choices of food they want to buy through discussion with their Key-Worker, house meetings and / or daily informal discussions. Have the right to follow their own cultural and religious beliefs by attending places of worship, facilitate in-house activities, listening and observation Have detailed Care Plans and Risk Assessments in accordance with the Human Rights Act Develop opportunities for employment, leisure and education through life- style planning, discussions, negotiations and other relevant professionals. Have the right to quality services provided by appropriately qualified and experienced staff. To have the informed choice of developing relationships and respecting sexuality.

4 Ivy Dene Doncaster Road Ferrybridge Knottingley West Yorkshire WF11 8NT Tele: Fax: Service Manager: Ms Tracy Wetherald Ivy Dene has been registered as a care home since November The home provides a high quality of care and support to 14 adults with learning disabilities and associated mental health issues. The main body of the home accommodates 10 service users with varied needs who require continuous ongoing support in managing most aspects of their daily lives including personal support. Adjoining the main house is the assisted living home which provides accommodation for 4 adults who continue to require support and assistance but have fewer personal needs e.g. personal care. The home delivers 24 hour support through a highly trained and motivated staff team. The team consists of a Service Manager, Deputy Manager, Activities Coordinator, senior staff members and support staff. Staff training and development is ongoing and includes; Person Centered Planning, Epilepsy and Diabetes Awareness, Vulnerable Adults, Values, Non-Violent Crisis Intervention, Mental Health, Medication Awareness, Visual Communication, Equality and Diversity, First Aid, Manual Handling, Food Hygiene and many others including Learning Disability Awards and NVQ qualifications at all levels. Each service user has their own individual assessment which identifies abilities, strengths and areas in which a higher level of support would be required. Each individual, with the support of the PCP Team and Service Manager devise their own Person Centered Plans and where appropriate PATHS to support them in achieving their own personal Goals.

5 Environment: Ivy Dene has 10 bedrooms in the main building. Each of these has a minimum of 11 square meters of living space. Service users are provided with a key which we encourage service users to hold themselves with appropriate support from staff if required. The home also provides 2 bedrooms downstairs for service users who require ground floor accommodation. If anyone needs to enter an individual s room for any reason, the person is consulted and has the right to refuse admission unless it is a health and safety issue. To promote a homely environment service users are encouraged to decorate and furnish their own rooms according to their own preferences and needs. A budget is allocated for each room to ensure service users are provided with a bedroom that has sufficient furniture and fittings suitable to meet the individual s needs and lifestyle. All service users will be supported to ensure their room is prepared prior to admission and will be planned within their move or introductory visits. Ivy Dene also has 4 bedrooms with washing facilities in an adjoining building. This provides a semi-independent environment which enables service users (if assessed appropriate) to develop their skills; more so that they may move to a more independent lifestyle in the future. It also provides an opportunity for service users to act and think without reference to other people, including a willingness to incur a degree of calculated risk. STATEMENT OF PURPOSE Ivy Dene aims to enable adults with learning disabilities and / or complex needs from our multicultural and socially diverse community to live as independently as possible by receiving the care and support consistent with their individual needs. The home provides accommodation and care for as long as appropriate to the individual who chooses to use our service. A learning through doing philosophy is used to encourage and develop an individual s independent life skills. Mission Statement To recognise that each person irrespective of their disability has equal value in society. We recognise that each person is unique and has the right to choose their own future and have an ordinary life equal to all in the community.

6 Management of the home The Registered Provider is: Mrs. Christine Gale, General Manager Ivy Mead, Littleworth Lane, Lundwood, Barnsley, South Yorkshire S71 5RG Mrs. Gale started her career in the care sector in 1979 training as a student nurse in Leeds, then Birmingham. The following years were spent in specialist care for those with learning disabilities and severe challenging needs. She spent many years developing her career and gaining a wealth of experience in both the private and NHS care sector. For several years Mrs. Gale inspected and registered residential and nursing homes within the West Yorkshire region. She then returned back into the private care setting and developed the company she now has overall responsibility for. The Service Manager is: Ms Tracy Wetherald Ms Wetherald started her learning disability nurse training at Stoke Park Hospital in Bristol in 1986 and gained the qualification in Ms Wetherald worked for 5 years in a hospital setting as an assistant manager in the assessment unit. During this time she gained experience in managing a team and supporting people with behavior that challenged the service. She also gained a qualification to become a workplace assessor. During the next 11 years Ms Wetherald managed several homes; increasing in size and a variety of service user groups. She gained experience in working with people who challenged the service, people with profound disabilities and in a supported living project. She undertook training in Person Centered Planning and involving Service Users in Recruitment. Ms Wetherald then moved to Yorkshire in October 2005 and took up the post of Service Manager at Ivy Cottage for a duration of 3 years before moving to the post of service manager of Ivy Dene in November 2008, Working with Ivy Homes has enabled me to put all my experience and training into practice in a way I wasn t able to achieve within the NHS.

The company is committed to maintaining a competent and skilled workforce ensuring staff individual training and development needs are identified and met through Continuous Professional Development.

8 Referrals: We welcome referrals for adults aged between 18 and 65 years with learning disabilities and complex needs. A brochure can be provided by contacting Mr Ross Evans, Business & Development Officer on or Referrals can be made by contacting Christine Gale, Claire Procter & Ross Evans or direct to the Service Manager of the home. An assessment will be completed by the management team to determine if the service is able to meet the individual s needs. Each referral is given careful consideration depending on current vacancies; however referrals can be placed on a waiting list or referred to another home owned by the company in an emergency. An on-call Manager is able to carry out an emergency assessment with support from the General / Senior Manager to agree a decision. A person centered approach is applied to devise an individual plan including any religious / cultural needs and preferences. Staff will provide service users with support to ensure they choose who they have contact with and maintain relationships / contact with relatives, friends and representatives. We encourage service users to play an active part in their community and employ an Activity Organiser to support individuals with a plan to engage in social activities, hobbies, interests, education and employment. We recognise that a person centered plan needs to be appropriate for the individual s level of need and understanding and are able to gain additional support from the company s PCP Team on appropriate plans to be used e.g. PATH, Essential Lifestyle Plan and MAP. Plans are reviewed on a regular basis and this again will be service user led including people that they want present, in a venue that they have chosen and at a time that is convenient to them. Service users are assured that they will be treated with respect and dignity according to their individual needs, wishes and preferences. The PCP Team consist of both staff and service users, ensuring service users have a say in the day to day running of the home, holding regular meetings and encouraging comments or suggestions from the people who use the service (in addition to a monthly service user meeting). Questionnaires are used to gain feedback from service users, family members and significant others about the service. Service users are invited to participate with the company s Regulation 26 inspections, creating their own report about the running of the home.

9 Therapeutic Techniques: The company uses a technique called Non-Violent Crisis Intervention. This is a non painful method accredited by BILD (British Institute of Learning Disabilities) and the BILD codes of practice to support service users in the event that they require support if they become a danger to themselves or others. The physical intervention will only be used as a last resort and all service users will have a plan based on their individual needs including: Assess before intervene; Appropriate verbal de-escalation techniques for the individual; and What approved physical intervention techniques to use and when. We have all relevant policies / procedures in place ensuring staff training is maintained and relevant safety checks carried out regularly including fire drills. 10 Complaints procedure: As a service user, advocate, relative, friend or visitor to Ivy Dene, you may have a complaint concerning some aspect of the home. If so then please ensure that you bring this to the attention of the Service Manager. This can be done by informing a service user s key worker, a member or staff or by contacting the Service Manager directly.
