1 ENROLMENT AT THE LYCEE FRANÇAIS DE BARCELONE FORM-FILLING GUIDE You can now make your enrolment application online. Please read the following instructions carefully before filling in the forms. Please print this document now. After logging in to the website (see page 6 of this guide), select the online enrolment on the left of the screen and click «Nouvelle inscription» ; «Année suivante ». Here you must read and accept the enrolment conditions. Then you will have access to the following forms: Representative 1 Representative 2 Your son or daughter (2 pages) His/her schooling (2 pages) At the end of each page, click Valider at the bottom right of the screen. Please complete all fields. Identity of legal representatives: (in most cases, these will be a mother and father living in the same home, or who are separated) Representative 1 is the legal representative with whom the child normally lives. There is no order of priority when the father and mother live in the same house, or when custody is shared. Representative 2 is the other parent, unless representative 1 is the head of a single parent family. Filling in the different fields of the pages representative and your child : Surnames must be entered in capital letters as on the National Identity Document, but without accents. Names must be entered in small letters as on the National Identity Document, but with accents. For people with dual or triple nationality, nationalities must be entered in the following order of priority: example: 1) French 2) Spanish 3) Other nationality. Phone numbers outside Spain only must be preceded by 00 followed by the country code. 1

2 Whenever the situation permits, you must automatically retrieve the information on representative 1 and transfer it to representative 2 or to your child by clicking «Récupérer les infos du responsable 1». For mastery of Spanish and Catalan, the levels mentioned refer to the following concepts: - Beginner: pupil who has never studied Spanish, or has studied very little Spanish. - Intermediate: student with an intermediate level of the language, following a number of years of education (1-3 years, depending on the case). - Bilingual: student capable of following (albeit with some difficulty) a language and literature course at mother tongue level. Please indicate if you would accept a place at that school if your application is rejected by the LFB. Whatever the case, the family must submit a single file to the centre chosen first. CLOSING THE FILE: The validation of the anti-spam screen allows you to obtain your file number and print it by clicking the printer icon. Please print it now. This printed document must be signed by both legal representatives and attached to the file with all the required documents (see p3 and p4 of this document) 2

3 1) THE SUMMARY OF THE DATA ENTERED MUST BE PRINTED, SIGNED BY THE LEGAL REPRESENTATIVES AND FORWARDED TO THE SCHOOL, ALONG WITH THE FOLLOWING DOCUMENTS: For kindergarten enrolments, submit the file in the Munner kindergarten principal s office or send it to the same office, carrer Munner, 5, BARCELONA. For primary and secondary school enrolments, mail it to the Enrolments Office of the Lycée Français de Barcelone, C/ Bosch i Gimpera 6-10, BARCELONA. 2) FOR ALL and PER CHILD document on personal data protection duly signed (only the document in Spanish of page 4 of this guide) 1 passport-sized photo of the pupil A photocopy of the Child s Family Booklet: pages about the father, mother and child (if a French national: French identity card) or birth certificate of the child photocopy of the identity cards of the father and mother, and of the child if any photocopy of the vaccination record of the child proof of residence in Barcelona (and surrounding area): a) employer certificate justifying the professional transfer of the father or mother Or b) census certificate provided by the Barcelona City Council in the case of residence in Barcelona or surrounding area Or c) affidavit of the main residence of the parents in Barcelona or surrounding area from September 2015 onwards. Original certificate of schooling of the current year (except for enrolment in kindergarten) Quarterly newsletters of the previous and current year (for secondary school only) SPECIAL CASES In case of separation or divorce: photocopy of an official document indicating parental authority and types of accommodation and schooling of the child document showing that one of the parents is a former student of the French educational system (other than a class photo) The committee will not take any incomplete file into consideration 3

Asimismo el alumno/padre/tutor presta su consentimiento expreso para que LICEO FRANCES DE BARCELONA pueda tratar posibles datos relativos a su salud, cuando resulte estricta y legalmente necesario para el adecuado desarrollo de los legítimos fines y funciones del centro y siempre en interés del afectado. Por otra parte le informamos que los datos personales de los alumnos no aceptados podrán permanecer en los ficheros de la escuela con la única finalidad de atender cualquier control o reclamación posterior. LICEO FRANCES DE BARCELONA informa al interesado de su derecho a acceder a los datos personales recabados por la escuela, del derecho a rectificarlos cuando éstos sean inexactos, de oponerse al tratamiento, y de solicitar su cancelación en los supuestos legalmente previstos. 6 In case of favorable decision of the committee, the definitive registration of your child will be submitted to the production of additional documents concerning its schooling and to the payment of first registration. The information concerning the pupils and families are integrated into the computer file of the pupils of the French High school of Barcelona. They are handled in the respect of the conventions, the directives and the regulations of the European Union and the organic law applicable 15/1999 regarding the treatment of personal data. Any request of access or rectification can be sent by mail to the French High school of Barcelona.
