3 PREFACE Fifth Edition This Index includes a compilation of references to journal articles and book information providing descriptions, samples of and entire copies of measurement and assessment tools/instruments and questionnaires (physical and psychosocial content) useful for nurses and allied health workers. The purpose is to provide ready access to many tools that are scattered throughout the literature and are difficult to locate. All entries have complete bibliographic information and are available at the CSUS Library. Many instruments appear in materials that the CSUS Library does not own, these are not included. References are to works that include sample portions of instruments or the entire instrument whenever possible and information for obtaining the instrument. Instruments from the sociological and psychological literature are included if deemed useful for the health care worker. The fifth edition includes 1,367 entries, an additional 453 since the fourth edition. 4 ACKNOWLEDGEMENTS (First Edition) Inger M. Lindholm, a nurse friend, was the inspiration for this Index. Approximately ten years ago she began persuading me to search for and create an index listing assessment tools useful for nurses. During the intervening years a heightened interest in the creation and application of assessment instruments appropriate for workers in the health care field has furthered the need and interest in the production of this index. It has taken many years to comprehend the scope of this request, follow through, gather, and compile these instruments into an index. 5 INTRODUCTION Fifth Edition The Index is divided into two sections: Reference List and Keyword & Instrument Name Index. The Reference List is arranged alphabetically by authors names and each entry is preceded by a number corresponding to the numbers used in the Keyword & Instrument Name Index (numbers are not page numbers). The Keyword & Instrument Name Index is arranged in alphabetical order. Included are the names, abbreviations or acronyms of the instruments, subject descriptors and keywords. The subject descriptors are derived from controlled vocabulary used in the CINAHL 2003 Subject Heading List (Cumulative Index to Nursing and Allied Health Literature) and older subject heading lists. Keywords were assigned if the controlled vocabulary did not suffice. Items in the Keyword section included under the "General" designation are references to books that describe many instruments in broad subject areas. Instruments referred to under General may include samples, the entire test or provide only a description of the applicability and validity for the instrument and a source for obtaining the instrument. All other entries include at least some sample questions used in the instrument or the entire instrument. Information for the Index was gathered by searching the print version of the Cumulative Index to Nursing Literature (CINAHL) from and the electronic version of CINAHL from 1982 to the present. The EUREKA Online Catalog was searched to locate monographic information. Browsing book and journal literature and pursuing bibliographic leads also provided sources. Determination of reliability and validity of the instruments referenced in the Index is the responsibility of the user. No Grant funding was provided for the preparation of the second, third, fourth or fifth editions of this index.
