2 ~ Introduction ~ The athletic program at Horizon Christian Middle School is designed to provide athletic opportunities for students in a Christ-centered arena. We consider it our responsibility and privilege to assist the development of each child s growth, as well as help them discover and strengthen their individual gifts and talents. Athletics is another element toward that goal in the education of the student. The interscholastic program is completely voluntary and any eligible student is invited to participate. Involvement does, however, require extra effort and extra time in addition to the regular school day. Those choosing to participate in the program represent Jesus Christ, their families, their church, and their school in all activities. Therefore, high standards of citizenship, sportsmanship, conduct, and attitude must be maintained. The standards of HCMS are also reflected by those participating on and with the athletic teams, including athletes, parents, and coaches. Not all teams in the interscholastic league are Christ-based schools. This provides an opportunity for the athletes from HCMS to portray Christian character to others. The interscholastic contests are competitive. The level of competition increases with the grade levels. In addition to teaching skills, teamwork, and sportsmanship, the goal of each game, match, or meet is more than simply winning on the scoreboard. At Horizon winning can look different than what the world may tell you is considered winning. For us, a win might be achieving a personal goal, improving on a specific skill, playing better as a team than the previous contest, etc. The athletic program is committed to training athletes to compete at the highest level of their ability. While we do not operate on a, win at all costs, mentality, there is also not a guarantee that all athletes will have equal playing time. The selection of team members may be necessary. Every effort is made at this level to allow all interested students the opportunity to participate on our school teams. Tryouts and cuts are rare and are used only if there are more players than there are spots available on the team. Should students need to be cut from a team, parents will be notified of this process ahead of time; preference for available spots on the team will be given to 7 th and 8 th graders. Please note that the selection of team members is not based solely upon athletic performance. Christian character, attitude, cooperation, loyalty and the ability to keep up with academics are also extremely important factors weighed in to qualifying a student for an athletic team SW Sagert St. * Tualatin, OR * (503)

3 ~ Rules of Eligibility ~ A. Attendance Requirements 1. Students must be enrolled and attending classes at Horizon Christian Middle School. 2. Student athletes will be allowed to participate in a practice or game if they attend at least a half-day of school on the day of the function. 3. When a family makes the decision to allow the student to participate on a team, the student is expected to attend each practice session and all games. 4. If the student is absent because of family events, the coach must be notified in advance. 5. Unexcused absences from practice are handled at the coach s discretion, but may include ineligibility to start or to participate in games. B. Scholastic Requirements 1. Participation in athletics is considered to be extra-curricular. Therefore, eligibility is partially contingent on the student first meeting the curricular standards required. Failure to maintain these standards may result in athletic probation or loss of eligibility. 2. Athletes will be required to fill out a weekly progress report sheet that will hold them accountable not only to their grades and completion of their work but also to a standard of behavior. 3. Any students with missing work in their classes will be required to attend two morning study sessions in order to remain eligible for that week. If there are special circumstances that should be considered, please notify the AD prior to filling out the progress report in order to help avoid confusion on the part of the teachers and/or coaches. 4. Student-athletes must not have any failing grades (F) or more than two D s or they will be ineligible until they raise that grade and it is reflected on the next progress report. Ineligible athletes are expected to fully participate in all scheduled practices as well as support the team from the bench during games during their period of ineligibility. C. Behavior Requirements 1. Student athletes are, by default, representatives of our school and also of Jesus Christ. Students must be able to model appropriate behavior in the classroom in order to be entrusted with the responsibility of representing our school and our Savior. 2. On the weekly eligibility sheet there will be a place for teachers to address misbehavior that has taken place that week. While we understand that we all make mistakes, Horizon Christian athletes will be held to a high standard of behavior and if more than one teacher has had trouble in a week, that athlete will lose their eligibility to participate in games until the following week; provided their next progress report reflects an improvement in behavior SW Sagert St. * Tualatin, OR * (503)

4 D. Transportation A scheduled number of athletic events are away from the HCMS campus. The school, coaches, or volunteers will arrange transportation to the events. Transportation may include school vehicles, but also the personal vehicles of coaches, parents, and/or volunteers. When personal vehicles are used, the vehicle s insurance will provide primary coverage. Student safety is a primary concern for us. In order to make sure all student athletes are accounted for, each athlete leaving an away facility will be required to be signed out with the coach by their parent or guardian. Parents wishing to have their student ride with another athlete s family should provide written permission to the coach. A note can be provided at any time, authorizing designated adults to routinely provide transportation for your athlete for the given season. E. Fees The fees collected cover the purchase and replacement of equipment, officials, custodial time for set up and tear down, use of facilities, and any special events. The participation fee is $125 per child, per sport. F. Injuries There are inherent dangers in any athletic activity. When a person is participating in athletic activities, an injury can occur. When an injury does occur during a practice or game, the injury needs to be attended to. 1. The coach must be notified and an accident report completed. 2. The information needs to be relayed to the parents. 3. If it is suspected that the injury will require medical attention, the coach or school official may recommend to the parents that professional medical attention is required. 4. If an injury does occur that requires medical attention, the parent s insurance will provide the primary coverage. 5. Following an injury serious enough to require medical care, or after an extended illness, an athlete may be required to present a physician s written release to resume participation. A physician s written release may also be required at any time if a school official deems it necessary. G. Participation: Before the first practice: 1. Student Contract signed and returned 2. Code of Conduct and Student Contract standards maintained ~ Code of Conduct ~ A. The Athlete is: 1. To always remember that he/she represents the Lord, self, family, team, and school 2. To respect the officials and their decisions. 3. To act in accordance with all the rules of eligibility and standards in this handbook. B. The Coach is: 1. Responsible for a growing relationship with Jesus Christ. 2. To live on and off the court with godly character as a role model for the athletes. 3. To inspire in the athletes a love for Jesus Christ, a love for the game, and a desire to do his/her best. 4. To teach the team Christian characteristics such as fair play, honesty, respect, integrity, good sportsmanship, and loyalty. C. The Parents are: 1. To encourage their child to try his/her best. 2. To help their child to stay on top of assignments and to get help if/when needed. 3. To set examples of sportsmanship, conduct, respect, godly character. 4. To accept the decisions of coaches and officials. 5. To support the school and its standards. ~ Volunteer Hours ~ Each family in the school is asked to volunteer a pre-determined number of hours during the academic year. Working with the athletic program is one way to fulfill those requirements. Should you wish to volunteer, several options are available, including: 1. Coaching or assisting the coach 2. Team parent to organize other volunteers 3. Transportation 4. 6 ~ Team Snap ~ TeamSnap is a web-based application that allows coaches, players, administration, and parents to all see the same information in the same place. A few of the key features we will be using are explained below. A. Roster Tab 1. The roster shows the members of the team and any contact information that parents have added and made public. B. Schedule Tab 1. View: you can view the schedule as a list or in calendar view. 2. Location: clicking on this link will get you the address of each contest. 3. Notes: this area will have any specific instructions for a particular event including release times and departure times for away contests. C. Media Tab 1. Files: this is where you find all the necessary forms and paperwork that is needed to participate in a sport at Horizon Christian Middle School 2. Photos: Parents can upload photos taken during the season to allow other parents to see. These photos are only viewable by members listed on the roster and are not available to the public. D. Messages Tab 1. This is a great resource if you need to contact anyone from the team. You can send one message and then select the recipients of the message (to some or to all). If you use this feature, please be sure to put a clear subject on the message. There is also an SMS text option, but not all roster members will have elected to use this option. 2. Coach Communication: Messages via TeamSnap will be the primary form of communication between coaches, AD, and families. Any last-minute changes or updates will be sent from here so please be sure to heed messages from TeamSnap. E. Preferences Tab 1. Personal Game and Event Notifications: You can elect to have reminders sent before each practice, event, both, or none. Adjusting this setting will help keep your in-box from overflowing with notifications from TeamSnap. 2. Medical Release: I recognize that as a result of participation in the Activity described above that emergency medical treatment may be necessary for the Participant and that Horizon personnel may be unable to contact me prior for my consent to emergency care. I therefore give consent in advance for such emergency care, including first aid treatment, transportation to a medical facility, and medical/hospital care as deemed necessary. I authorize and consent to all medical, surgical, diagnostic, and hospital procedures as may be performed or prescribed by a physician. I waive my right to information prior to such treatment. I authorize and request personnel from Horizon Community Church/Christian School/Learning Center to administer or supervise such treatment and to do any procedure that they deem necessary until such time as the Participant can be safely transported to a doctor or hospital. Transportation Release: I give my permission for my Participant to travel to games and practices in vehicles driven by Horizon approved adult drivers. Media Release: I also irrevocably and for perpetuity authorize and consent to the use of any and all photographs, video, and audio recordings of myself and my Participant for use in promotional and educational projects by Horizon Community Church and Schools. I understand that this is consent given without any expectation of compensation to me or the Participant, and that all photographs, videos, and recordings shall become the property of Horizon Community Church and Schools. Liability: I acknowledge that participation in the Activity described above involves risk to the Participant (and to Participant s parents or guardians, if Participant is a minor), and may result in various types of injury including, but not limited to, the following: sickness; personal injury; death; emotional injury; property damage; and/or other financial damage. In consideration for the opportunity to participate in the Activity described above, the Participant (or parent/guardian if Participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the Activity. The Participant (or parent/guardian) accepts personal financial responsibility for any injury or other loss sustained during the Activity or during transportation to and from the Activity, as well as for any medical treatment rendered to the Participant that is authorized by Horizon personnel or its agents, employees, volunteers, or any other representatives. Should it become necessary for the Participant to receive medical treatment for any reason, it is understood that the Participant s insurance is responsible for the primary costs of all care provided and that any insurance provided by Horizon is secondary. The Participant (or parent/guardian) accepts full responsibility for the excess costs of medical treatment for any injury which is over and above that which is covered by insurance. Further, the Participant (or parent/guardian), understanding the inherent risks of the Activity, releases and promises to indemnify, defend, and hold harmless the Horizon Community Church/Christian School/Learning Center, and its agents, employees, volunteers, or any other representatives, for any injury or loss arising directly or indirectly out of the described Activity or transportation to and from the Activity, whether such injury arises out of the negligence of the Activity Sponsor, the Participant, or otherwise, except in the case of gross negligence as may be determined. Arbitration: If a dispute over this agreement or any claim for damages arises, the Participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the Participant (or parent/guardian) and the Activity Sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution pursuant to the rules of the American Arbitration Association. I will strive to always exhibit good sportsmanship toward HCMS team members and all other players. 2. I will strive to follow coach s and/or other adult helper s instructions without hesitation or complaint. 3. I will keep all my schoolwork up to date and will maintain acceptable work standards on all classroom requirements. 4. I will be respectful and obedient to any and all school authority at all time. 5. I will make every effort to attend all practices, games, and team functions. 6. I will abide by school policy and rules for all practices, games, and team functions whether on or off HCMS campus. I have read and accept the standards in this contract and the HCMS Athletic Handbook.
