Application Requirements Sports Specialisation Scheme Professional & Vocational Qualification Scheme One (1) copy of the applicant s identity card One (1) copy of the applicants University/College Transcripts/MATSEC/SEC certificates (where applicable) One (1) copy of a signed nomination form by their respective National Sports Organisation or Club (where applicable) One (1) copy of a certificate as evidence that the applicant has participated in a Youth Development Scheme for a minimum period of three years (where applicable) Four (4) copies of a Sports Profile Report including a record of sports participation and achievements as per sample attached with the Application Form or on a DVD or both Four (4) copies of Sports Career Profile report showing evidence if any of work experience or voluntary service in Sport as per sample attached with the Application form. Any other documentation (four (4) copies) which may assist the SSSB to assess better the Applicant s Sport Merit. Signed Declaration by Parents/Guardians authorising Application to participate in the Scheme if the latter is under the age of 18 on Friday, 21 st June, 2014 Signed Declaration by Parents/Guardians confirming that Applicant will remain in compulsory education if the latter is under the age of 16 on Friday, 21 st June, 2014 If not submitted with this Application, the following shall be required by Friday, 25 th July One (1) copy of the Unconditional Letter of Acceptance from the Recognised Institution applied for; One (1) copy of MQRIC evaluation report (where applicable)

Please note that at least one (1) valid Unconditional Letter of Acceptance shall be submitted by the Friday, 25 th of July 2014 from a Recognised Institution applied for. No changes to this Application Form shall be allowed after the closing date for Registration. ONE application form shall be accepted for each Applicant. If more than one Application Form is submitted, the last Application Form submitted shall be considered by the SSB Board.
