Subtopic(s): Forms The purpose of this policy bulletin is to inform staff of revisions to the Legal Service Providers Eviction Prevention Program form (W- 146X). The form has been revised as follows: The locations, contact names, telephone numbers, and areas served were updated. Staff must ensure that the Agency s applicants and participants who are in need of legal services are afforded the most accurate and up to date information to assist them with their legal issues. Center Directors must ensure that staff utilize the updated version of the W-146X, and that, all previous versions are removed and recycled.
