2 ADN Program Application Requirements The following information summarizes the requirements that must be completed before an applicant will be considered for entry into the Associate Degree Nursing Program. The ADN Program Application and the ADN Health Requirements Form, which are required for application to the program, are available on the GCC website at web.gcc.mass.edu/admission/health-careers/. Requirements: 1. Complete a general GCC Application for Admission. Submit an application electronically from the GCC website at web.gcc.mass.edu/admission/. Call the Office of Admission at (413) if you do not have internet access and require a paper application. 2. Submit official high school transcript with graduation date. 3. Submit official transcripts from previously attended colleges by the transcript deadline, if applicable. You will receive a transcript evaluation notification sent to your GCC address, which will indicate whether or not placement testing is required. Pay attention to the transcript deadline for the year in which you are applying. Be sure to check your MyGCC account to ensure all required courses are transferred. 4. Complete the math requirement by receiving placement test scores of 67 in arithmetic and 91 in algebra within five years prior to entering the ADN program, OR by completing Intermediate Algebra (MAT 096) OR a higher level math course at GCC or another college with a C+ or better within five years prior to entering the program. 5. Complete the English requirement by completing English Composition I (ENG 101, 103 or 105) at GCC, or equivalent course at another college, with a grade of C or better. Note that eligibility to take English Composition I is determined by placement testing and additional coursework may be required depending on test results. 6. Complete ten credits from the list of GCC courses below, including one of the four-credit BIO* courses listed below: BIO Anatomy and Physiology I 4 cr. C+ required within 5 years** BIO Anatomy and Physiology II 4 cr. C+ required within 5 years** BIO 205 Microbiology 4 cr. C+ required within 5 years** ENG 112, English Comp II 3 cr. C required 114, or 116 SOC 101 Principles of Sociology 3 cr. C required PSY 101 Principles of Psychology 3 cr. C required PSY 217 Human Growth and Development 3 cr. C required within 7 years** Humanities elective 3 cr. C required Humanities elective is any course coded: AHS, ART, ASL, DAN, ENG (excluding ENG 090, ENG 094, and English Comp I and II), FLK, FRE, GGY, HIS, HUM, LAT, MUS, PHI, PCS, SPA, or THE. + Formerly BIO 195 and BIO 196. *Biology I (BIO 126) is a pre-requisite for Human Anatomy and Physiology I and II (BIO 215 and BIO 216), but it is NOT required to apply to the ADN program. Students who feel that they have a strong science background from high school or college may choose to take a challenge exam for Biology I to meet the BIO 215/216 pre-requisite. **Currency Exams are available for these courses if they were successfully completed more than five years prior to the beginning of the program. 7. Submit the ADN Health Requirements Form and documentation of all health records as described on the form. There are specific timelines for completing the physical exam and immunizations. After reviewing the form call the Health Records Office at (413) if you have any questions about submission of health requirements. 2
3 8. Submit your signed ADN Program Application to the Office of Admission, attn: Laura Earl, between the submission dates stated on the ADN Program Application for that year. Note: Applications submitted prior to the completion of the college application, transcripts or placement testing, health records or required coursework will be considered incomplete and returned to the applicant. Grade Point Average: GCC students must maintain a cumulative grade point average (GPA) of at least 2.0 to be eligible for the ADN Program. High School Students: High school students who wish to enter the ADN Program in September following high school graduation, and have a B average, and rank in the upper third of their graduating class, are eligible to waive requirement #5. Call Laura Earl at (413) for more information and to determine your eligibility. Letters of acceptance into the ADN Program will be sent to students in April of each year. A waiting list of qualified applicants will be established in the event that selected students are not able to accept their place in the program. The waiting list will only be valid for the same year of entry. Applicants who are not selected for their desired year must reapply to be considered for a subsequent year. Additional Requirements for Enrollment for Accepted Students Enrollment in the ADN Program is conditional upon completion of the following items prior to the start of classes in September. Detailed information about completing these requirements will be sent to students in their acceptance letter to the program. The information below will help you to plan ahead for the timely completion of requirements in the event you are accepted. The deadlines posted in your acceptance packet are strictly enforced. Students who do not have these items completed by the posted dates WILL BE REPLACED by students on the waiting list! Criminal Offender Record Information (CORI) check. An authorization form will be sent to you in your acceptance packet. Additional CORI/SORI checks may be required by affiliating agencies prior to pediatric clinical placements. Proof of CPR certification American Heart Association Healthcare Providers (infant, child and adult) CPR certificate is required before entering the ADN program, and currency must be maintained while in the program. No other CPR certifications will be accepted. Mandatory attendance at Orientation. One full day in the summer. Grade point average (GPA) of 2.0 for all students who have attended GCC. Completion of PPD (Tuberculosis) test between June 1 and July 15. See the Health Requirements Form, Section VI. Yearly PPDs are required to continue in the program. Completion of all immunization requirements per state and federal regulations governing individuals working in clinical settings, including yearly flu vaccinations or declination forms. Students requiring additional immunizations will be notified by the GCC Health Records Office. Failure to complete immunizations on schedule will jeopardize eligibility to begin the ADN Program. Mandatory drug screening required prior to beginning of fall semester. Mandatory finger printing required before pediatric clinical placement. 3
4 CORI (Criminal Offenders Record Information) All students entering the ADN Program will be required to have a CORI (Criminal Offenders Record Information) check. A student with a charge in the Lifetime Presumptive Disqualification Category is ineligible for placement in the GCC ADN Program. Any charges in the Discretionary Disqualification Category will be reviewed on a case by case basis. Some clinical facilities will run their own CORIs on students. Applicants need to know that individuals with a record of conviction by a Court of Law will be required to provide a complete explanation to the Board of Registration in Nursing in order to be permitted to take the licensing examination (NCLEX) after graduation. Important Health Information for ADN Program Students Please read carefully! Completed health records must be submitted to the GCC Health Records Office to fulfill the application requirements. The following is a summary of health requirements needed for the Associate Degree Nursing Program: Physical exam within a year of starting the program Tdap must have one Tdap and Td if Tdap is more than 10 years old. MMR 2 doses at least one month apart, or positive titers to each Measles, Mumps, and Rubella disease. Hepatitis B - 2 doses one month apart, third dose 6 months after the first, or positive titer (at least 2 doses of Hepatitis B are required by the application deadline, with the third dose due prior to entering the program.) Varicella positive titer or 2 doses of vaccine at least one month apart Tuberculosis screening PPD (Mantoux) test. First may be submitted with application. Second due in summer. If positive, chest x-ray is required, see the health form for specific date requirements. Yearly Flu vaccination or completion of declination form. Drug screen with clean/negative results prior to clinical For detailed information about health requirements and required documentation refer to the ADN Health Requirements Form or call the Health Records Office at (413) Mobility to Bachelor of Science Degree in Nursing Completion of the BSN level of nursing education provides the graduate with a much greater variety of career opportunities and eligibility for graduate education. GCC has articulation agreements for our ADN graduates with the University of Massachusetts/Amherst, AIC, Elms College, Fitchburg State University, Framingham State University and Vermont College. Most, or in some cases all, of the required course work for these BSN programs can be taken at GCC. College advisors will be happy to assist you in planning your course of study. A currency exam is available for all BIO courses if taken more than five years prior to entering the program and PSY 217 if taken more than seven years prior to entering the program. Students are required to pass every course in the curriculum with a grade of C (74); science and nursing courses require a C+ (77). Courses must be completed in sequence; no student may progress to the next semester until all courses required in the previous semester are successfully completed. 5
6 Advanced Placement for LPNs Greenfield Community College offers two options for advanced placement of Licensed Practical Nurses into the Nursing Program, depending on space availability. Download the appropriate Advanced Placement ADN Program Application at web.gcc.mass.edu/admission/health-careers/. The first step of applying to these programs is to meet with Laura Earl (413/ to schedule). 1. For eligibility to enter the program in January of the second semester an LPN must: complete an approved LPN Program complete all general education requirements taken by first semester students pass the in-house Fundamentals of Nursing exam successfully complete the NUR 111 bridge course during January intersession 2. For eligibility to enter the program in September of the second year an LPN must: complete an approved LPN Program complete all general education requirements taken by first year students pass the in-house Fundamentals of Nursing exam pass the Maternal and Child Nursing (Baccalaureate Level) Excelsior College exam successfully complete the NUR 112 bridge course during summer session II LPNs that graduated from GCC with a "B" or higher grade average in their nursing courses, AND have graduated within five years, are eligible to waive the Fundamentals Exam and Maternal and Child Exam requirement. LPN applicants should be advised that Maternity and Pediatric contact hours in some PN programs may not meet eligibility for RN licensure outside of MA. Transfer students may be accepted on a space available basis to the second semester only, but priority will be given to LPN applicants.
