1 Driver Safety Purpose To provide guidance for Catholic Community Services of Southern Arizona Inc. (CCS) employees and volunteers who operate CCS vehicles and those employees and volunteers who drive personal vehicles on CCS business. Scope All CCS Employees and Volunteers who are authorized to drive on agency business Policy Job descriptions and career announcements will list requirements for jobs that require a valid driver's license for state of residence, good driving record and personal vehicle insurance. Procedure The following provides procedural guidelines for both the drivers of CCS vehicles and those employees who drive personal vehicles on CCS business. The Supervisor will ensure: Employee or volunteer possesses a valid driver's license for his/her state of residence. The employees and or volunteers driver's state motor vehicle records for the last three years are checked for accidents and traffic violations. A Staff Driver form is completed and signed by employee or volunteer and Supervisor. Employee or volunteer has been properly trained. The Employee and Volunteer: Employees and volunteers who will drive personal vehicle on CCS business and receive mileage reimbursement will provide CCS with confirmation of personal auto insurance. The employee will maintain this insurance during the period the employee drives on CCS business. Employees and volunteers must notify supervisors of changes in coverage or insurance company. Human Resources will review each employee s driving record annually. Vehicle Operations / Agency Vehicles The Employee or Volunteer: 1

2 An employee or volunteer involved in an accident while driving an agency-owned or agencyrented motor vehicle must report the accident immediately to his/her supervisor. The employee s or volunteer s (driver s) supervisor will verbally report the accident to the CCS Operations Director as soon as possible. The employee or volunteer will call law enforcement to file a corroborating report when the accident involves personal injury or property damage. Employees or Volunteers will provide the number of the report to the CCS Operations Director as soon as possible. The Supervisor: Supervisors will submit the driver s accident report to the CCS Operations Director within one working day by fax (520) or teresac@ccs-soaz.org. Supervisors will provide employees and volunteers the CCS Driver Safety Rules when filling out the CCS Staff Driver's form and tell them that they are required to adhere to such rules, including completion of the Driver's Safety Training Program. Personal Vehicles Employees and volunteers involved in an accident while driving personal vehicle on CCS business must immediately report the accident to their supervisor. Supervisors will submit the driver s accident report to the CCS Operations Director within one working day by fax (520) or teresac@ccs-soaz.org. Property Damage Employees and volunteers involved in a work-related accident will immediately report the incident to the supervisor. Supervisors will report the accident to the CCS Operations Director. Employees and volunteers who witness a CCS-related accident or incident that results in an injury to a non-ccs employee (i.e. volunteer, client, etc.) will immediately report the incident to supervisors. Supervisor will immediately report the incident to the CCS Operations Director, and subsequently submit a written incident report. Driver Safety Program CCS has a Driver's Safety Program in order to assure the safety of employees and volunteers, clients, and the community at large. The program consists of the promotion of safe driving, the monitoring of employee driving records, and the disciplining of unsafe drivers. This program will: Maintain information pertinent to the driving record of each employee or volunteer. 2

3 Review accidents, infractions, and critical incidents with the Director and appropriate representatives of CCS Administration. Assigned Agency Personnel will be responsible for: Collecting and submitting employees MVD driver's information form and insurance status to the Human Resources. Reviewing accidents, infractions and critical incidents. Disciplining unsafe drivers. Qualification of Drivers Drivers of vehicles that are owned, rented, or leased by CCS or personal vehicles used for CCS business must follow defensive driving techniques and practices. The following requirements will be met: Drivers must possess a valid driver's license for their state of residence in order to operate any CCS vehicle or a personal vehicle on CCS business. Job descriptions will state the requirement for a driver's license. Employees or volunteers with a temporary restricted license that results from a motor vehicle violation citation (whether or not adjudicated) conviction or guilty plea will not be permitted to drive CCS vehicles or transport clients or employees in personal vehicles. Employees and volunteers shall report to their supervisors any restrictions imposed on their license to drive. Employees or volunteers driving on CCS business must report a violation or accident to supervisors. The following records will be maintained to ensure that an employee is qualified to drive: Verification that employees and volunteers have a valid driver s license from their state of residence Review of the driver's state MVD record for the most recent three-year period Review of the accident report history showing the dates and types of accidents regardless who was at fault Review of the traffic violations for the last three years Confirmation of personal insurance for those driving personal vehicles on CCS business Physical examinations when required by the state or by the funding/licensing source Drivers Responsibilities Drivers on agency business are required to use seat belts and instruct passengers to do likewise. Persons operating a motor vehicle when transporting a child who is under eight years of age and not more than 4 9 must have the child properly secured in a child restraint system, such as a child safety seat or a booster seat. Drivers transporting more than one child must secure as many children in child restraint systems as is reasonable given the size of the passenger area and the number of passengers being transported. 3

4 Drivers who use a personal vehicle on agency business must furnish proof of insurance. Drivers must report a moving violation to supervisors within one working day by submitting a motor vehicle violation/accident report. Point System The point system applied by CCS follows the Arizona Motor Vehicle Department and it will be utilized to determine eligibility for employees who drive for CCS. Violation Points DUI/Extreme DUI 8 Reckless driving 8 Aggressive driving 8 Leaving the scene of accident 6 Failure to stop for a traffic signal, stop sign or to yield the right-of-way, causing death Failure to stop for a traffic signal, stop sign or to yield the right-of-way, causing serious injury Speeding 3 Driving over or parking in a gore area 3 All other moving violations 2 Not reporting any of the above Employees or volunteers who have attended Traffic Defense School for a violation may submit a copy of certificate or receipt and relevant points may be removed from their record. Employees / Drivers will pay fines received while driving CCS vehicles. Disciplinary Action Employees who have accumulated six points or more in the past 12 months may be disciplined. Employees who have had their license suspended or revoked may be terminated. Volunteers who have had their license suspended or revoked will not be eligible to volunteer for CCS as a driver. An employee who accumulates eight points, is convicted of a criminal activity while driving, and/or has his or her license revoked may be discharged. A volunteers who accumulates eight points, is convicted of a criminal activity while driving, and/or has his or her license revoked will not be eligible to drive as a volunteer. Employees who fail to pay fines for violations received while driving a CCS vehicle may be disciplined up to and including discharged from CCS. 4

5 Employees and volunteers will indemnify and reimburse CCS for any fines or assessments CCS is required to pay as a result of any moving or parking violation committed by the employee or volunteer 5
