3 Admission Procedures for the Vocational Nursing Program Students with applications currently on file are required to contact the Allied Health Office if they have a change in address or phone number. Applications will stay on file for the next program unless the student retracts his/her candidacy. I. Length of Program and Cost: Three semesters (one and one-half years) excluding summers. The cost for the program is included. Non-residents and residents of California will pay tuition set by the College. II. Filing Application: Admission application forms are obtained from the Merced College Bookstore or the internet ( Only complete packets will be accepted, incomplete packets will be returned to the student. If it is determined that all program prerequisites / application requirements have been met you will be placed on the waiting list. The first 40 applicants on the waiting list will be admitted into the program. The next 10 will be alternates. III. Transcripts: 1. You must submit with your application unofficial transcripts from Merced College and/or official transcripts from other colleges/universities attended that reflect program prerequisite coursework completed. 2. Submit the enclosed Transcript Request Form. IV. Minimum Requirements for Admission to the LVN Program: Students must meet the following requirements BEFORE submitting application packet: 1. High School graduation or the equivalent as measured by General Education Development (GED) test or California State High School Proficiency Examination and the completion of the following prerequisite courses. 2. *Have completed within 5 years of date of application the following courses or their equivalents with a grade of "C" or better: 1. ALLH 67 - Medical Terminology (No recency required) 2. *BIOL 50 - Survey of Anatomy & Physiology or BIOL 16 - General Human Anatomy 3. *NUTR 10 - Nutrition 4. *VOCN 46A - Applied Mathematics for Pharmacology 3. Must be in good health. 3

4 V. Selections: A minimum of 40 applicants on the waiting list will be notified by mail around 3-4 months prior to the beginning of a new class. If any person admitted into the program declines, an alternate will be called according to their order on the waiting list until a full class has been designated. VI. Physical Examination: Upon acceptance into the program, the student will receive a physical examination form which must be completed by a physician and returned to the Allied Health Secretary prior to the start of school. VII. Immunizations: Students will need to submit proof of immunity to rubella and rubeola (antibody titer or immunization record), proof of polio immunization and proof of a negative TB skin test or chest x-ray within the last year and DT within last 10 years. Proof of Hepatitis B immunization or release form is required. VIII. Registration: Once selected for the program, the student must attend a pre-registration orientation meeting at which time information will be given on the classes to register for, and when to register. A list of required supplies and where to obtain a white uniform will be provided. New Merced College students will be required to enroll in the College and are expected to obtain a student identification card. IX. History of Convictions: Applicants should be aware of the provisions of the California Vocational Nurse Practice Act which requires the Board of Vocational Nurse & Psychiatric Technicians to review the history of previous convictions and in those cases where offenses bear a substantial relationship to the nursing profession, the Board may withhold the license. Should a student decide to decline again, he/she will have to reapply. 4

6 Step by Step Procedure for Application 1. Read entire Vocational Nursing Application Handbook, CAREFULLY. 2. Send for two official copies of all your transcripts (except Merced College) to be sent to you directly, including: 1. Official college transcripts from other colleges/universities attended. To be official, transcripts must remain sealed in the original envelope. It is the applicants responsibility to obtain a copy of past college catalogs or course descriptions, to assist in the establishment of equivalencies. 2. Foreign-born students with transcripts outside the U.S. will need to have them evaluated for U.S. equivalency. Obtain your High school Diploma, or High school transcript or GED, or equivalent. 4. Make an appointment to see an Allied Health Counselor (209) and bring with you: 1. One set of sealed, official transcripts 2. Other college catalogs, course descriptions 3. LVN Application Handbook 6. When meeting with Counselor, review: 1. Basic information - any questions you may have. 2. Transcripts (credits for Program) 3. Military Credits 4. Foreign transcripts - Formal Evaluation 5. Minimum Requirements for the LVN Program 6. Other program requirements 7. Competency requirements 8. To ensure that courses are equivalent to those offered by Merced College, a counselor must complete Course Equivalency form (See Application Packet at end of Handbook) 7. 7 8. When Minimum Requirements have been met, prepare a COMPLETED application packet containing: 1. Check-Off Sheet 2. Admission Application 3. Merced College Student I.D. Number 4. High school diploma or High school transcript or GED or equivalent 5. Unofficial transcript from Merced College - Transcript must be printed from the Admissions & Records Office 6. Official transcript(s) from other colleges/universities (if applicable) 7. Transcript Request Form 8. Prerequisite Course Equivalency Form (if applicable) 9. Program Waiting List/Status Report 10. Self addressed envelope 9. Submit COMPLETED application packet to the Allied Health office. If it is determined minimum requirements have not been met or an incomplete application packet is submitted, applicant will not receive official application date and the packet will be returned to the applicant. Completed Applications Will Be Accepted Throughout the Year. 7

12 Merced College Vocational Nursing Program Waiting List/Status Report Name: As of this date: Your number on the Waiting List is: Your official application date is: Your application and transcripts have been reviewed to verify that all program prerequisites have been completed and you are therefore qualified to be on the Waiting List for the Vocational Nursing Program.!!ATTENTION!! Your status on the Waiting List may move up or down at anytime. You may periodically check your location on the Waiting List three (3) months after the date of application by calling or or rd Keep in mind that all students on the list are contacted every 3 semester to advise them whether or not they have been accepted to the program beginning the following semester. If a student for what ever reason declines acceptance into the program, the first alternate will take st nd their place. If the 1 alternate declines, or another student declines, the 2 alternate will be notified. This process continues until the class has a minimum of 30 students. Each student on the list may decline acceptance into the program ONE TIME ONLY. If a student wishes to decline a second time and wishes to remain on the Waiting List, he/she will be placed on the bottom of the list. If a student fails out of the program but wishes to repeat they must meet with the program director prior to resubmitting their application. Notify this office as soon as possible if you have changed your plans and no longer wish to remain eligible for entry into the program. If you have any questions contact the Allied Health or or Reminder: It is very important to contact the Allied Health Office IMMEDIATELY if you have a change of Name/Address/Phone(s) number(s). If we are unable to contact you to confirm acceptance and/or to maintain your status on the Waiting List, you are at risk to lose your seat in the program and to be removed from the Waiting List.
