1 Adult/Gerontology Primary Care Nure Practitioner Program at UCLA School of Nuring Overview: The Adult/Gerontology Primary Care Nure Practitioner (AGNP) i a Regitered Nure educated at the Mater level a a nure practitioner. The focu for the AGNP i to provide primary care to individual acro the adult age pectrum from late adolecence through older adulthood. In addition, AGNP are prepared to provide comprehenive end of life care to adult. Becaue the health care need of adult range from wellne to complex illne care, the etting in which the AGNP deliver care may include community health center and clinic, private medical practice, pecialty clinic, health maintenance organization, Veteran Adminitration facilitie, Geriatric Evaluation and Management (GEM) unit, adult day health center, aited living facilitie, killed nuring facilitie and other long-term care etting, acute and chronic rehabilitation center, urgent care and emergency department. In many cae, AGNP follow their patient acro care etting to maintain quality and afety during care tranition. Education: Credential: Mater of Science in Nuring Eligible for the American Nure Credentialing Center Adult and Gerontological Nure Practitioner certification exam Eligible for the American Academy of Nure Practitioner Gerontological Nure Practitioner certification exam Eligible for precriptive authority in all 50 tate with retriction a pecified by each tate Type of Clinical Care provided by the Primary Care Adult/Gerontology Nure Practitioner: o Health promotion and dieae prevention activitie including creening exam, couneling, providing adult immunization and ordering/evaluating reult of appropriate diagnotic tet. o Comprehenive phyical, pychoocial, developmental, enory, functional and cognitive aement o Epiodic management of acute condition, including minor acute injurie and illnee o Evaluation and management of chronic condition common among adult uch a HTN, diabete, table chronic cardiovacular and neurologic dieae, athma/copd, arthriti, oteoporoi, depreion o Evaluation, prevention, and management of geriatric yndrome uch a delirium, urinary incontinence, and fall o Advance Care Planning and comprehenive end of life care Advanced Practice Nure alo perform thee general function: Obtain health hitorie and perform comprehenive phyical examination, including pychoocial, functional, and developmental aement Order and interpret lab reult and other diagnotic tudie Develop differential diagnoe Develop/order therapeutic plan of care including precription medication Maintain patient record Evaluate patient repone to plan of care and modify a needed Provide patient/family couneling and education Arrange for patient referral/conultation Participate in reearch tudie Collaborate with interdiciplinary health team member
