2 The Mental Health Of Our Community Choice. A simple word, but one that holds particularly significant meaning when it comes to people s health care, including mental health care. We, at Transitions, are very proud of the fact that since 1980, we have offered the good people of the Illinois Quad Cities a choice of where and how they want to address their mental health challenges. There has been much in the news lately about the exciting things going on nation-wide and locally that will change how mental health care is delivered. It s important, however, to keep in mind that the population of persons with severe mental disorders is a population of people with many varied diagnoses, symptoms, individual goals, experiences, challenges, levels of personal support, feelings, fears, priorities, medical conditions, ages, genders, economic levels, races, cultures, levels of education, and preferences of services, to name a few. We, at Transitions, as providers and as advocates, never forget that client choice of services, methods of treatment, providers and venues is critical. By limiting or eliminating choice, we limit client participation and investment in their own health. To assume that all persons with mental illnesses should, and will, want the same services from the same provider, delivered in the same manner dehumanizes them, and in fact sends them the message that they are nothing more than their diagnoses. Moreover, eliminating choice results in too many not seeking out services and if left untreated, over time, become sicker and sicker, ultimately causing them and their families great distress, and costing the community far more when, due to their untreated illnesses, they reach emergency or disastrous status. Services to persons with mental illnesses must be sensitive, personalized, carefully matched, and carefully administered. It s important that this is not lost to a philosophy of closed or single systems of care. At Transitions, this very critical reality has not been lost. Gary Weinstein, CEO
3 Who We Are We are TRANSITIONS MENTAL HEALTH SERVICES. A highly responsive, and sensitive organization of professional staff members who come into work every day expressly to lessen the pain, discomfort, frustration, isolation and hopelessness of the people we serve while striving to help them identify their goals, provide them the guidance and equip them with the skills and insights necessary to reach these goals. A satisfying, safe, productive life for everyone we serve is always what we strive for. What We Do We provide a carefully selected array of practical services, all of which are intended to improve the quality of life for individuals with mental health issues and their families, and for all those in the community who are impacted by mental health issues. Our Services The services we provide were chosen specifically because they create a framework within which we guide our clientele through the mental health system, through their personal issues and obstacles, and help them in reaching their goals. Therapy Services In-depth explorations of long-standing, deep-seated issues that have caused major disruptions in their ability to process, cope, overcome challenges, and improve one s life. Consultation Services Assistance in understanding, responding to, and working with, individuals with mental health issues. Employers, co-workers, teachers, clergy, family members, law enforcement, health and human service providers are examples of those who can benefit from this service. Rehabilitation Counseling Individual and group opportunities for working through current issues and obstacles that negatively impact an individual s ability to live a productive, safe and satisfying life. Residential Services Respite, short-term and longer-term supported and supervised living facilities for persons needing assistance with developing and improving the daily living and coping skills they need to successfully and safely live independently in the community. Employment Services Assistance with attaining and maintaining community employment, including job searching, job development, assistance in identifying employment opportunities, job skills development, on-site job coaching, application completion, resume development, job interviewing practice, conflict resolution, and training and employment in our own three agency-owned and operated community businesses; Property Professionals our community-based building repair and renovation company since 1983 Transitions Commercial Cleaning our community-based janitorial company since The Printer s Mark our full-service community-based printing company since Psychiatric Services Assistance in attaining medications to alleviate the symptoms of mental illnesses, and medications monitoring to ensure continued effectiveness.
4 Who We Serve We serve individuals, five years of age and older who are struggling with mental health issues including mental illnesses, personality disorders and behavior disorders. We serve those who have struggled to the point where holding a job, maintaining relationships, living independently and/or finishing school have not been possible. We also work with those who have families and jobs and responsibilities but who are currently having difficulties staying focused and productive due to their symptoms. We help make that possible. We also serve families who are struggling as they attempt to care for their loved one, and we work with employers who may have someone working for them who is dealing with a mental disorder. Finally, we work with clergy, educators, friends, health and human services providers, and others in the community who want learn more about mental disorders and/or assist the people who have them. Number Served Total clients served 455 [Adults 361 Youth 94] Individuals and groups served through community presentations, trainings and provision of educational materials 352 Customers 223 Local Vendors 70 Total Community Contacts 1,100 Our Outcomes Transitions is dedicated to promoting, enhancing and improving the health and well-being of individuals, families and the community impacted by mental health issues. (excerpt from Mission Statement) Are we accomplishing our mission? Are we impacting the lives of our consumers? Tracking outcomes is one way to answer these questions. We have four basic issues which impact the well-being the individuals, families and community served as well as the Agency. These are: accessibility, efficiency, effectiveness and satisfaction. Are services readily available to consumers? (Accessibility) The issues surrounding mental illness many times demand a prompt response. Such a response begins with an assessment and a recovery plan. Working on recovery begins with the client meeting his/her rehab consultant. This year began with a waiting time for this meeting of 25.2 days. The amount of waiting time was reduced to 14 days by the end of the year. This increase in accessibility makes the journey to recovery a shorter process.
5 How efficient is the Agency in providing services? (Efficiency) As in any good business the greater the efficiency the more actual services can be provided. With the onset of the Affordable Care Act many more people with mental illness have the means to address the problems they face. The meeting of requirement for good clinical practices and an increase in overall service hours primarily helps the consumers, but also brings more revenue to the Agency. Efficiency is measured by comparing the number of service hours to the total hours worked. In FY14 the Agency s efficiency was an average of 66.75%. This was an increase over the last fiscal year of 22%. How effective are the services Transitions provides? Do the clients served move forward in recovery? (Effectiveness) Recovery is a complex and individual journey. We have considered life safety, community integration and productivity as markers on the road to recovery for clients over 18 years of age. Transitions shows 80% of these adult clients have improved or maintained a high level of life safety % of those served have improved or maintained an adequate degree of community integration, which is a particularly challenging element for those living with mental illness. These percentages do not reflect the progress made by clients who terminate services in a short time period. Change takes time. Of all clients wishing to seek a productive life through employment, continuation of education, volunteering or other ways of contributing to their growth and that of society, 62% were successful in reaching their goal. Are clients of Transitions satisfied with their services? (Satisfaction) Consumers responded positively on a satisfaction survey. 50% were very satisfied and 47% were satisfied with the services they received. Together these total a satisfaction rate of 97%. Are all clients of Transitions adults? No. Transitions has a Youth program, serving youth from ages 6 to 18. This constitutes 17.6% of the total population served by Transitions in FY14. The Youth program measures progress with two assessments the Columbia and Ohio instruments in conjunction with the State of Illinois. Out of 25 youth completing the Columbia assessment at least two times in 6 months 76% of the youth were rated as having made progress and/or a positive change in dealing with mental health symptoms, problems or issues. In the parent version of this measure 56% of the parents reported seeing a positive change in their child. 76% of the youth participants had a positive rating in terms of functioning, indicating they were able to function better at the second evaluation than when they began. The same instrument evaluated progress in coping skills and showed that 72% of the youth had improved in this area. Some youth clients do not remain in the program for 180 days. Therefore, their growth/progress is not able to be reflected in the above percentages.

It will consist of comprehensive rehabilitation programs and individual business/ training enterprises serving as the primary environments for learning and growth. Each business, program and service will be community-recognized and preferred as a responsive, reliable, innovative, friendly provider of quality services and products. Each business, program, and service will exist to provide persons with severe mental health issues the counseling, therapy, consultation, support, training, psychiatric assistance, and employment experience needed to reach their goals, and develop self-respect, skills, self-confidence and life satisfaction.
