The result is a program designed to accommodate busy professionals who are trying to balance a full-time work schedule with an online program. The format of the program provides the flexibility you need to log into course portals, access course materials, and work on assignments around the clock from any location. By participating in this program, you ll also build lasting networks and share a common understanding and commitment to lifelong learning. 4 Program Targets the Unique Roles and Training Needs of Public Health Workers The online chronic disease program is designed to enhance the contributions of public health workers as agents of change who help governmental public health agencies, partner organizations, and the Centers for Disease Control and Prevention improve the health of the public. The program aims to support the unique educational needs of the public health workforce, taking into account their expertise, experience, and working environments. The goals of the program are to help participants Understand the biologic basis of chronic disease and the roles of clinical medicine in diagnosis, treatment and public health programs for the prevention and control of these diseases. Apply fundamental quantitative concepts, methods and tools to identify health problems and target populations. Identify and prioritize evidenced-based program and policy options. Develop a plan and implement interventions; and ensure strategy effectiveness. Participate in a national network of public health practitioners dedicated to lifelong learning. The program is a 1-year course of study. The curriculum consists of 5 mutually reinforcing courses, each of which is 8 weeks in duration. Each participant will take 2 courses each semester in a sequential order. Course Descriptions Introduction to Biostatistics: This course covers topics of interest for public health fields, including descriptive statistics, proportions, relative risks, probability, estimation and hypothesis testing applications, regression, and categorical data analysis. Applications use the statistical software, SAS. Introduction to Epidemiology with an Emphasis on Chronic Disease: This course introduces concepts of epidemiologic methods and their practical applications to understanding the determinants and distributions of disease. The course covers the basic principles of epidemiology, including chronic disease control and analysis of risk factors. The course relies on readings in the field of chronic disease prevention and health promotion to illustrate major issues, such as types of epidemiologic studies, key analytic methods, and causal inference. Problem sets will provide experience in chronic disease epidemiologic methods and inferences. Health Program Planning: This course focuses on the key steps shown to be effective in the planning, implementation, and evaluation of community health promotion programs. The course emphasizes the practical application of the core principles and rationale of epidemiology, social and behavioral sciences, and evaluation, and acknowledges that effective chronic disease prevention programs must thoughtfully address the cultural, political, environmental, and economic realities that define a community.

5 Practical Aspect of the Social Determinants of Health: This course explores social and economic characteristics of individuals, communities, and societies that are the root determinants of health and illness and the principal causes of health disparities. The course examines the implications for acquiring health related resources and exposure to environments which influence health, their role in forming a context in which health behaviors develop and their function in triggering biological and physiological processes which underlie disease. The course also considers the evidence base on potential solutions, particularly from the perspective of the scale of intervention and of the point of intervention along the pathway connecting social and economic characteristics to health. Principles of Tobacco Control: This course will provide an overview of the history and evolution of tobacco in society, with particular emphasis on variety of public health disciplines used to reduce tobacco use, including behavior change, communications, law, regulation, public policy and community action. Tobacco Control Progress in reducing tobacco use is one of the ten greatest achievements of the 20th Century; however, tobacco use still remains as the leading preventable cause of death in America. The control of tobacco use draws upon many if not all of the academic disciplines that compose public health.
