(It is important to answer each and every appropriate question and sign and date where indicated) Complete the Multihousing Credit Control (MCC) General Consent Release and Application for Apartment Occupancy forms for each household 18 years of age and older if you need to copy additional forms for more than two adults please do so. Include a $35 application fee for each household member 18 years of age and older. Money Order payable to Sunwood Village. Complete, date and sign the Applicant Acknowledgement. I understand that: SUNWOOD VILLAGE is a tax credit development, not a HUD Section 8 property, but the property does accept Section 8 Housing Choice Vouchers. The rents are not based on income and rents do not change as income goes up or down. All questions on the Initial Housing Application must be answered and only complete, signed and dated applications can be accepted. I will need to have third-party verified household annual income of at least $18,198 for a 1 bedroom, $21,141 for a 2 bedroom and for 3 bedrooms in order to be eligible for housing at this development. Applications that do not meet the minimum income requirements will not be accepted. For each adult (18+) member of the household: I need to complete and submit both pages of the Multihousing Credit Control (MCC) background screening and the General Consent Release form. All applicants will be screened by a third-party company according to the property s Tenant Selection Plan (TSP). Two sets of the Forms have been provided, please make additional copies as needed. An Application Fee made payable to Sunwood Village in the form of a Money Order is required for my application to be processed.
