2 Epidemiological situation update Kabale There was no new admission in the isolation facility at Rushoroza HC III on 8th November. The two Marburg confirmed cases (the mother and her baby) are still admitted in the isolation ward at Rushoroza HC III. The child has started showing slight improvement though she still remains weak, while the mother is doing well. The Honorable Minister of State for Health (Primary Health Care) is slated to visit Kabale district on 9th November to assess the current situation and monitor the response to the Marburg outbreak in Kabale district. Ibanda & Mbarara There were no new suspect cases admitted in Ibanda or Mbarara. The confirmed case in Ibanda who has been critically ill died and a supervised burial was conducted. Kampala 6 of the Kampala contacts have completed their days of follow up. The rest are still being followed up and they are well. The Minister of State for Health in charge of Primary Health Care, with a delegation consisting of Dr J. Amandua (Commissioner Clinical Services), Dr J. Amone (ACHS Curative Servises), Dr H. Luzze (SMO) and Dr R Musoke (Epidemiologist) is on a monitoring visit to Ibanda, Kabale and Mbarara, to monitor interventions for Marburg prevention and control. Update on response activities Kabale: The suspect case from Ntungamo district who was admitted in the isolation ward in the evening of 7 th November tested negative for Marburg on PCR and has been transferred to the convalescent ward in Rushoroza HCIII. There were no alert calls of Marburg suspect cases from the community on 8th November. Four teams were deployed to follow up 6 contacts; all the 6 (%) contacts were traced and reported to be doing well. Thirty four (34) contacts to confirmed cases completed days of follow-up on 8th November and will be dropped from the contact follow-up list on 9th November. A cardiac blood sample from the community death from Habugarama village, Rwenyanje parish, Kamwezi sub-county (reported on 7th November ) tested negative for Marburg on PCR. A total of 6 contacts who completed -day follow-up period had their blood samples collected on 8th November for serological testing. Social mobilization P a g e
3 The social mobilization sub-committee conducted a community dialogue at Lake Bunyonyi involving 67 hotel attendants, local leaders, bar maids, waiters and managers of restaurants. A total of 3 religious and opinion leaders from Kabale district have been mobilized for sensitization on Marburg prevention and control slated for 9th November. Volunteers from Uganda Red Cross Society were deployed to Bushuro parish in Kitumba sub-county where they conducted house-to-house case search and sensitizations in 9 households; 4 hotels; 9 shops; and 9 bars. The film van was deployed to Bubare and Muko sub-counties where a total of six (6) parishes with an estimated population of,6 people being sensitized on Marburg prevention and control. Psychosocial support The psychosocial support team deployed to three areas: the first team travelled to Kyanamira where they counseled the family members and neighbors to the confirmed cases currently admitted in the isolation facility; the other two teams were deployed to Post Bank and Byamuzungu Cooperative Savings Union respectively where they provided counseling to employers and workmates of two contacts that have completed -day follow-up program. The psychosocial team has finalized compiling the list of individuals and households that are meant to benefit from the solidarity kits. The team has also embarked on a social postmortem of the probable causes of death for suspect cases that tested negative for Marburg. The coordination sub-committee is considering scaling down the workforce involved in Marburg epidemic response in light of the reduced intensity of activities such as the number of patients in the isolation ward, decreasing number of contacts to be followed, low levels of alerts and the accompanying need for ambulance and burial services, etc. A program has been drawn for the district to host the Honorable Minister of State for Health (Primary Health Care) on 9th November. Ibanda The district notification of a release of 8 million Uganda shillings. The funds will be available in the district account by the end of the week. The Chairman LCV conducted a sensitization meeting in Kikyenkye sub-county and also visited the bereaved family that had just had a supervised burial. The Minister of State for Primary Health Care, the Commissioner of Health Services Clinical and her entourage visited the isolation facility and also addressed the district leadership during the district task force meeting. There are no new admissions on the isolation facility: There are admissions in the isolation facility. The confirmed case a one year and four month old male baby died and a supervised burial was conducted today. 3 P a g e
4 The isolation facility is full and yet 8 patients are due for discharge. However, their discharge is pending the availability of a discharge package. Forty four (44) out of 49 contacts were followed up today. However, 5 new contacts of the child who died will be added to the list and will be followed up starting tomorrow. There were no new alerts. Two () blood samples were collected. Social mobilization The Social mobilization plan has been developed and will be finalized with the support of social mobilization experts from MoH headquarters and WHO. Conducted sensitization of residents of Sigirira trading centre in Kikyenkye trading centre, the locality from which the 3 confirmed Marburg cases originated. Leaflets and posters on Marburg were distributed using the film van. A radio talk show on Marburg was conducted on Voice of Kamwenge Radio last night. The district has received leaflets and posters on Marburg provided by the MoH and WHO. A sensitization of 4 Private Medical Practitioners in Ibanda district was conducted in the afternoon. Students and teachers of Nyakatukura memorial secondary school were the initial case was working were sensitized and counseled on Marburg today, 8 November. Psychosocial support The team provided counseling to the bereaved family and the surrounding community. Mbarara The draft response plan is ready and has been circulated for comments The acting Director Clinical and Curative, visited the district, toured the isolation unit and met with selected members of the DTF. He and his entourage provided valuable guidance to the DTF on technical issues. Seventy-six (76) health workers drawn from the district and Mbarara Regional referral hospital were trained on Marburg epidemiology, case management, infection control, surveillance and social mobilization. Eighteen (8) out of nineteen (9) contacts were followed up by the district surveillance focal point and all are well. 4 P a g e
5 The data for both Mbarara and Ibanda have been entered into FIMS and cleaned. However due to few cases in both districts, detailed epidemiological analysis is not warranted at this stage. Major Gaps/ Emerging Issues Kabale: No major gaps have been identified in the response on 8 th November. Ibanda The social mobilization team needs strengthening in order to plan and execute the plans more effectively There are no IEC materials for social mobilization. Mbarara The social mobilization and psychosocial care activities need to be strengthened. Lack of operational funds is hindering the response. Kampala: No major gaps Planned activities Planned activities for Kabale Sensitize religious and opinion leaders from Kabale district on Marburg prevention and control. Conduct sensitization of health care workers on Marburg surveillance protocols including the case definition, triage guidelines, and guidelines for referring suspect cases. Host the Honorable Minister of State for Health (Primary Health Care) is expected to visit Kabale district on 9th November. Continue follow-up of contacts to confirmed cases. The psychosocial support team is to continue supporting affected families and communities. Continue mobilizing and sensitizing the communities on Marburg prevention and control. Planned activities for Ibanda Sensitization of schools. Continue with door-to-door active case search and comprehensive follow-up of contacts Planned activities by Mbarara Convene a DTF meeting tomorrow Engage the district health educator and the DTF socil mob committee on scaling up Liaise with MRRH to see how psychosocial care activities can be started. 5 P a g e
