2 Purpose: The Medical Professional Institute Scholarship Program has been designed to provide financial assistance to students who meet the criteria established below and want to enroll in one of Medical Professional Institute s programs for start dates between April 1, 2015 through December 31, Eligibility Requirements: In order to apply for a Medical Professional Institute Scholarship, an eligible student must: Complete the admissions enrollment process and be accepted Start the program by December 31, 2015 Submit scholarship application and necessary documents to the Admissions Department prior to your initial financial aid appointment and the given deadline. More specific eligibility requirements are included in the Scholarship Descriptions section of this application. Maintaining eligibility Those students awarded a scholarship must maintain satisfactory academic progress throughout their time at MPI. Scholarship Award: Scholarships will be prorated over the duration of a program. Only students that meet the qualifications listed above and have met the admissions requirements in order to be considered an enrolled student can be awarded the scholarship. The Medical Professional Institute Scholarship Committee, made up of faculty and/or staff, will make the final decision regarding the award. Recipients of the scholarship will be notified in writing by school administration. The notification will include the amount being awarded and the start date for the program. Additional Scholarship Information: The scholarship will not be awarded to any student who defers their enrollment past the requisite time period. The amount of scholarships offered can vary based on the number of applications and the decisions made by the committee. This award is a scholarship and does not require any form of repayment to Medical Professional Institute. Only one Medical Professional Institute scholarship can be awarded per student. *These Scholarship Programs can be suspended at any time. There would be no adverse impact on those students who were awarded a scholarship in the event that the Scholarship Program was suspended. 2

3 Scholarship Descriptions Phlebotomy & Electrocardiography Alumni Scholarship MPI students who are graduates of the Phlebotomy & Electrocardiography Technician program may apply for the MPI Allied Health Alumni Scholarship worth $1,500. The scholarship can only be applied towards enrollment into the Medical Assistant program at Medical Professional Institute. Those interested in applying for the scholarship must have successfully completed the Phlebotomy & Electrocardiography program as well as their externship. Interested candidates should see the Admissions Office for requirements and deadlines. Medical Assistant (MA) to Practical Nursing (PN) Fast Track Scholarship Medical Professional Institute (MPI) is offering an opportunity for those wishing to enter into our Practical Nursing (PN) program to gain knowledge and background in the medical field through our Medical Assistant (MA) program. Students who complete both programs are given an amazing opportunity to have two separate certificates in Medical Assistant and Practical Nursing. After completion of the Medical Assistant program at MPI with an overall grade point average of a 3.0 or higher, graduates will be eligible to apply to the Practical Nursing program without taking the Test of Essential Academic Skills (TEAS) test. In addition, accepted students who go through this process will be awarded a $4,500 scholarship toward their pursuit of a PN certificate awarded through the Financial Aid Office. Use of the Fast Track does not guarantee acceptance into the PN Program. Guidelines for the MA to PN Fast Track The following requirements are to be met in order to utilize this method: 1. Applicant must score an 70% or higher on the Pre-Application Assessment 2. Applicant must be recommended to the Fast Track program by their respective Admissions Counselor and/or the Director of Admissions. 3. Applicant must complete the Medical Assistant program fully with externship and obtain a Grade Point Average of at least 3.0 (B average) on their MPI transcript. 4. Applicants must not have any course grades of Course Withdrawal (CW), Course Withdrawal Passing (CWP), Course Withdrawal Failing (CWF), Program Withdrawal, Program Withdrawal Passing (PWP), Program Withdrawal Failing (PWF), Academic Administrative Withdrawal (AAW), Attendance Administrative Withdrawal (AtAW), Disciplinary Administrative Withdrawal (DAW), Financial Administrative Withdrawal (FAW), Documents Administrative Withdrawal (DoAW), No Pass (NP), Drop Fail (DF), or Incomplete (I). 5. Applicants must have no disciplinary warnings of any kind while attending Medical Professional Institute. Medical Assistant President s Scholarship The Medical Assistant President s Scholarship will be awarded each semester to new students entering into the Medical Assistant program. A limited amount of scholarships will be available in the amount of $2,500. To apply for this scholarship, accepted MA students must write a minimum 200 word essay about why they want to pursue a career in Medical Assisting and how this scholarship will help to achieve that goal. Deadlines to submit the essay will be posted in the Admissions Office and/or the MPI website. Selection to receive the scholarship will be based on Pre-Application Assessment score and essay. Recipients will be notified by the Admissions Office by phone and/or . *This scholarship is available for new students only. Those who receive this scholarship would not be eligible for the Medical Assistant Alumni Scholarship. Practical Nursing President s Scholarship The Practical Nursing President s Scholarship will be awarded each semester to new students entering into the Practical Nursing program. A limited amount of scholarships will be available in the amount of $3,000. To apply for this scholarship, accepted PN students must write a minimum 300 word essay about why they want to pursue a career in nursing and how this scholarship will help to achieve that goal. Deadlines to submit the essay will be posted in the Admissions Office and/or MPI website. Selection to receive the scholarship will be based on the Pre-Application Assessment, TEAS Scores, and essay. Recipients will be notified by the Admissions Office by phone and/or . *This scholarship will be available for new students only. Those who have received the Medical Assistant President s Scholarship or Medical Assistant Alumni Scholarship would not be eligible. Practical Nursing Maldonian Scholarship The Practical Nursing Maldonian Scholarship will be awarded to any graduate of Malden High School in the amount of $2,000. To apply for this scholarship, accepted PN students must provide a copy of their high school diploma from Malden High School as well as a transcript proving date of graduation. This scholarship can be awarded up until any given class start date. This scholarship can only be given once per person. Medical Assistant Maldonian Scholarship The Medical Assistant Maldonian Scholarship will be awarded to any graduate of Malden High School in the amount of $1,000. To apply for this scholarship, accepted MA students must provide a copy of their high school diploma from Malden High School as well as a transcript proving date of graduation. This scholarship can be awarded up until any given class start date. This scholarship can only be given once per person. 3
