13 Patient Engagement Important focus of MU Stage 2 Patient Electronic Access > 50% of patients must have access to their electronic health information > 5% of patients must view their health information online Secure Electronic Messaging > 5% of patients must send secure messages to their EP

15 Preventive Care Objective: Send Clinical Reminders to 10% of your Established Patients Reminders must be clinical in nature and must be for care the patient is not scheduled to receive Use Clinical Parameters in the Patient Search report to identify patients and send reminders. Reminders will be sent per the patient s communication preference ( , Patient Portal, Letter).

17 Summary of Care Objective: For Outgoing Referrals, the EP should provide a Summary of Care record. The measure has 3 parts: 1. 50% of patients are provided a Summary of Care record when being referred. 2. For 10% of patients, a Summary of Care record is electronically transmitted to the appropriate provider via Direct Messaging. 3. At least 1 Summary of Care record must be sent to a provider using a different EHR vendor Exclusion: Any EP who has less than 100 Outgoing Referrals in the Reporting Period.

19 Immunization Registries Data Submission Objective: Ongoing Submission to an Immunization Registry Exclusions: > EP does not administer any immunizations > There is no immunization registry capable of accepting the data Enroll with your state, and setup an interface through CureMD Ongoing submission of data In Stage 1, it was a menu measure and only one test of submission was needed

20 Medication Reconciliation Objective: EP should perform Medication Reconciliation for patients that come from another setting of care or provider of care For New Patients > Record the patient s updated Medication List and check the Medication Reconciliation Performed checkbox For Incoming Referrals where a Summary of Care record was provided > Perform Medication Reconciliation

23 CPOE The measure has 3 parts: > Use CPOE for 60% of Medication Orders > Use CPOE for 30% of Lab Orders > Use CPOE for 30% of Radiology Orders Exclusion: Any EP who writes fewer than 100 orders of any type will get the appropriate exclusion Compared to Stage 1, higher thresholds and addition of Radiology and Lab orders
